Proprioceptors in muscles, tendons and joints are deep sensitivity sensors and convey the perception of position, movement, situation and force of the body.1 They lead to motor responses through sensorimotor feedback.

If these sensory structures are damaged, the result is defective coordination, which can lead to pain and, in the long term, to wear in the joint. Superficial mechanoreceptors in the skin also assist proprioception when, during movement, the skin is stretched or compressed. Pressure or vibration acts upon the skin to stimulate these proprioceptors.

The mode of action of supports based on the example of the GenuTrain®

GenuTrain® promotes the afferent functions and the neuromuscular feedback processes. It stabilizes and relieves pressure on the knee, thereby alleviating pain.

Neuromuscular joint stabilization Supports cover a larger area of skin and, therefore, a large number of receptors. During movement, the constant intermittent compression applied by the knit stimulates receptors both on the surface of the skin and in the deeper layers of the muscles and soft tissue. Due to the increased perception of irritation, there is also a more intense perception of the joint position in the motor centers of the central nervous system, and the stabilizing musculature is activated. Coordination improves and incorrect movements are prevented.

The stimulating action of the intermittent compression massage is more effective when there is constant contact between the support and the skin. This means that the better the fit of the support and the better it adapts to a particular movement, the better the stabilization of the joint, thanks to the positive effect on the proprioception. The fit of the support therefore helps to determine its therapeutic effectiveness.

Study into the GenuTrain®

Controlled trial with the knee support (GenuTrain) on patients with non-specific knee pain due to osteoarthritis in order to investigate the clinical effectiveness of the knee support in terms of pain reduction and stabilization.

Study structure: prospective, randomized, controlled clinical study

Therapy:

a) Conservative: Relieving tension with manual massages, called “friction massages” applied at a right angle to the direction in which the tendon fibers run and extend combined with myofascial techniques. This relaxes the lateral fiber group, reducing the pain caused by the strong lateral pull on the patella. At the same time, it is important to strengthen the medial muscles, while also trea-ting the lumbar spine and hips, if necessary.The tension-relieving pad fulfills the functions of this friction massage, relaxing or reducing tension in the tensor fasciae latae muscle, and thus relaxing the apparatus of lateral restraint.

b) Surgical: If there is a strong lateralization and subluxation ten-dency, the most sensible course may be to make a notch in the lateral retinaculum (lateral release) and to tighten the medial re-tinaculum, if necessary.

Study into the GenuTrain® A3

39 patients with symptomatic osteoarthritis (Grade 1–3) with an average age of 62 years took part in the study. The study examined the effect of knee supports in patients with osteoarthritis of the knee.

The causes of knee instability are damage to one or more of the functional elements of the knee joint and a reduction in proprioception.1 However, correct sensorimotor feedback forms the basis of a physiologically coordinated movement.2

The GenuTrain S active support, with its combination of lateral joint splints and adjustable strap system, provides the knee with excellent lateral passive stability. Neoprene

BreathabilityKnit products are breathable, which means air can easily flow through the material. This provides a natural cooling effect by avoiding heat build-up and by allowing humidity to evaporate.

through the use of the brace is important because this minimizes further injury, promotes a quicker recovery and allows them to get back the full range of motion much sooner. At first, a patient might be reluctant to wear the brace, but once it is put on by the nurse and they feel the comfort and relief they have a different attitude. This results in high compliance. We’ve also found that patients like the support they get from the brace long after the procedure and continue to use it whenever they feel the need. Thousands of my patients have used, and continue to use, Bauerfeind support products as part of their treatment plan and are very pleased with them.

What is it about GenuTrain knee supports and SacroLoc or-thesis that make them different than other products on the market?Dr. Purita: I tried a knock off of GenuTrain but the product fell apart. Durability is the key. Bauerfeind products can be used a long time before they need to be replaced. Another key factor is the anatomically shaped viscoelastic inserts that provide target compression. This massaging effect enhances therapeutic treatment by stimulating the neuro-motor system and results in

a positive influence on muscles and joints by reducing the pain. Braces alone don’t do everything, but the feeling of well being that is provided by GenuTrain is all about proprioception. And, of course, the product has a modern design and is attractive. This also promotes patient compliance.

Where do you see the future of conservative treatment of dege-nerative medicine, and what do you see as the role of compres-sive products such as Bauerfeind’s GenuTrain in that future?Dr. Purita: What’s new today will be main stream tomorrow, and, in my opinion, without a doubt stem cells and platelet rich plasma (PRP) will be a part of that in the US. However, no matter what the technology, there is no question that stabilizing the tis-sue and reducing inflammation will always be a part of successful treatment. Using support products such as Bauerfeind’s Genu-Train and SacroLoc right alongside these new procedures will be a key factor in returning a patient to normal activity as quickly as possible.

Dr. Joseph Purita, renowned orthope-dic physician and pioneer in the use of the laser in orthopedic surgery, is part of an 11-physician team of orthopedic specialists at the Boca Raton Orthope-dic Group in Boca Raton, Florida, the largest and foremost provider of or-thopedic services in South Palm Beach

County. He is also founder of the Institute of Regenerative and Molecular Orthopadics, a hybrid of traditional ortho-padics and cutting edge regenerative medicine utilizing a variety of non-surgical solutions.

Dr. Purita treats approximately 500 patients a month and uses Bauerfeind products — GenuTrain knee support and SacroLoc orthesis for the pelvis and the sacroiliac joints — as part of his non invasive and post surgical treatment plans. Today, he speaks to us about the products and his patient’s positive reaction to wearing Bauerfeind braces.

What kind of injuries do you see on a regular basis, and what are the common treatments or interventions you use for these types of issues?Dr. Purita: My patients live in the South Florida area where the weather is conducive to year around outdoor activities allo-wing them to be very active. Because of that, I see a myriad of injuries and conditions including osteoarthritis, sports related injuries and sprains, bursitis of the knee and shoulder, knee tendonitis, patellar chondromalacia, commonly known as “runner’s knee,” and back injuries. My goal is to provide treat-ment that ensures the best outcome for my patients, therefore, when appropriate, I take a multi-step approach for each of these conditions or injuries. First, I use non-invasive treatments such as injections of cortisone to minimize pain and promote healing. In addition, I use GenuTrain knee supports and SacroLoc orthesis,

in the case of back injuries, to minimize swelling, stabilize soft tissue and reduce inflammation in order to return the patient to full functionality in the shortest period of time. Of course if the injury is more severe we perform arthroscopy. Here, again, support products such as GenuTrain or SacroLoc are used as part of our post-operative treatment plan.

How long have you been using Bauerfeind support products and why did you decide to try them? Dr. Purita: I have been practicing orthopedic medicine for over 30 years and about 20 years ago I saw Bauerfeind products at an orthopedic conference in Europe. Anatomically knitted to provide a precise fit, they appeared to be comfortable and easy to use so I decided I’d try them. But it’s really my patient’s positive reaction to the products and the fact that they experienced faster healing that convinced me to continue using them. The products suit our purposes very well and I’ve been using Bauerfeind ever since.
