*DentaQuest offers dental services for members living in Kenosha, Milwaukee, Ozaukee, Racine, Washington and Waukesha counties. In these counties, you must go to an in-network dentist. If you live outside these counties, you may get dental services from any dentist who will take your ForwardHealth ID card. Call ForwardHealth Member Services at 1-800-362-3002 for questions about your dental benefits in these counties or to find a dentist.

Dont forget your ID cards! Its important to bring all of your health insurance cards with you to your provider appointments. This includes your ForwardHealth ID card, your Anthem member ID card and any cards from other service providers you may have.

Choosing a primary care provider As a new member, you will be assigned to a primary care provider (PCP). Your PCP is your health advocate. Your PCP will: x Get to know you and your health history x Try to find and treat minor health issues

before they become more serious x See you for well-visits once a year and

nonemergent health concerns as needed x Recommend a specialist for any special health

needs you may have

You can call Anthem anytime and tell us who you want as your PCP. Anthem will assign you to a PCP near you if you do not let us know who you want. The name and phone number of your assigned PCP will be on your Anthem member ID card. We want you to be happy with your PCP, so you can call Member Services if youd like to change your PCP.

If youre a Medicaid SSI member and your current provider isnt in the Anthem network, we will still pay him or her for up to 60 days after you enroll. This is known as continuity of care. If you havent chosen an in-network provider for yourself after 60 days, you will be reassigned to one.

OB/GYN services You have the right to see an Anthem OB/GYN without a referral from your PCP for services. To find an OB/GYN in this directory, go to the Specialist section and look for providers listed under OBSTETRICS/GYNECOLOGY for your county or a county near you.

HealthCheck program HealthCheck is a full program of checkups and health care services for babies, children, teens and young adults with Anthem. These services make sure babies, children, teens and young adults get the health care they need. Good health starts at birth. We encourage you to have your children screened. This includes dental checkups, starting at age 1.
