Introduction
In January 2012 the Public health outcomes framework was published. This framework is one part of the government’s
reforms of the public health system in this country. It is also one of a suite of outcomes frameworks, which include
frameworks for: the NHS, adult social care, and at the time of writing, a proposed one for children and young people.
The purpose of the public health outcomes framework is to “…set out a broad range of opportunities to improve and
protect health across the life course and reduce inequalities in health that still persist.”1
The framework has two overarching outcomes: Increasing healthy life expectancy and reducing the differences in life
expectancy and healthy life expectancy between communities. To support these overarching outcomes are 66 further
outcomes (some of these outcomes are supported by more than one indicator), divided into 4 domains:
• Domain 1: Improving the wider determinants of health • Domain 2: Health improvement • Domain 3: Health protection • Domain 4: Healthcare public health and preventing premature mortality
Currently some of the outcomes are not specified and some of the indicators are not yet finalised. Also even when the
outcome and the indicators have been finalised the data may not be available at this time. This report also includes
data from Domain 1 (Preventing people from dying prematurely) of the NHS outcomes framework2. These have been
included as they are of relevance to public health.

Methods
This report has used the public health outcomes framework as a starting point in the development of a tool to support
prioritisation in the newly created Health and Wellbeing Board3. Each of the indicators in the framework, if the data are
available, has been assessed using 4 questions:
• How is Doncaster currently performing? • Is Doncaster improving? • Is Doncaster narrowing the gap? • Are inequalities improving within Doncaster?
Each of these questions was gauged using the following system:

Outcomes and indicators with more red ‘buttons’ are areas where the board may wish to devote more attention than
indicators with more green ‘buttons’.

Findings
It is important to note that data are not available for a number of the outcomes and indicators in the framework4. But
from the data currently collected 12 outcomes should be brought to the attention of the board. The codes in
parenthesis are the outcome reference numbers (PHOF – Public health outcomes framework; NHSOF – NHS outcomes
framework).
• Perinatal mortality (1.6ii NHSOF) – Perinatal mortality is a combination of stillbirths and infant deaths in the first 7 days of life. While perinatal mortality rates are not significantly different in Doncaster compared to national rates,
the rates have increased slightly over the last 10 years but have declined nationally. There is some indication that
the rates are rising faster in the more deprived parts of the borough.
• Low birth weigh births (2.1 PHOF) – Any baby with a birth weight less than 2500g is a low birth weight birth. Rates in Doncaster are significantly higher than England & Wales and the rates are not falling. Rates are falling nationally
and so the gap with Doncaster is widening. However there is some evidence that internal inequalities are
improving.
• Breastfeeding prevalence (2.2 PHOF) – The prevalence of breastfeeding in Doncaster is significantly lower at initiation and 6-8 weeks after birth compared to the national rate. These rates have not significantly improved
over the previous 10 quarters. Nationally rates are improving and so the gap between Doncaster and England is
currently widening.
• Infant mortality (4.1 PHOF; 1.6 NHSOF) – An Infant death is any death occurring in the first year of life. Infant mortality rates are currently only slightly higher than England & Wales, but there is no evidence that rates are

improving in the borough. Nationally the rate is falling and so this gap is widening. Within Doncaster there is some
evidence that inequalities may be beginning to increase.
• Children in relative poverty (1.1 PHOF) – Children in relative poverty are children living in households where the income is less than 60% of the national median household income (before housing costs). Doncaster has
significantly more children in relative poverty compared to England as a whole and the rate has increased
significantly. The gap with national rates is increasing.
• Excess weight in children (2.6 PHOF) – Children’s height and weight are measured at 4- 5 years and 10-11 years and are defined as overweight or obese if they are greater than or equal to the 85 centile on the British growth
chart. The rates of children who are overweight or obese at reception are significantly higher than national rates
and are climbing. A gap is opening between Doncaster and England as a whole.
• Killed or seriously injured on the roads (1.10 PHOF) – The rates in Doncaster are significantly higher than the national data. The national rate has also been falling at a significantly faster rate than in Doncaster where the
rate has remained largely unchanged for the last 9 years.
• Cancer screening (2.20 PHOF) – In Doncaster both cervical and breast screening programmes are currently performing better than nationally. However there is evidence that uptake rates are falling in both programmes.
• Suicide (4.10 PHOF) – Suicide rates are currently significantly higher in Doncaster and there is some evidence that the rate might be increasing. Because the national rate shows a slight decline there is evidence of a widening
gap between Doncaster and England & Wales. Within the borough, conversely, there is some evidence that
inequalities are improving.
• Liver disease mortality (4.6 PHOF; 1.3 NHSOF) – Deaths from liver disease are higher in Doncaster and have been increasing for the last 10 years. The gap with England & Wales has widened over this period. Inequalities within the
borough have remained largely unchanged.
• Respiratory disease mortality (4.7PHOF; 1.2 NHSOF) – Respiratory disease mortality is significantly higher in Doncaster and rates have remained largely unchanged over the last 10 years. The gap with England & Wales is
widening. Inequalities within Doncaster have remained unchanged.
• Excess winter mortality (4.15 PHOF) – Excess winter mortality is the number of deaths in December to March compared to the previous and subsequent 4 months. Rates tend to vary widely from year to year but there is
some limited evidence that rates might be falling in Doncaster. Within the borough there has been some increase
in inequality, with more deprived areas showing increasing mortality and less deprived showing reductions.
