PURPOSE

The purpose of this manual is to provide guidance and direction to District and campus personnel charged with

the maintenance and accuracy of student records for the purposes of identifying, reporting, data validation of

student dropouts, leavers and students withdrawing from school; it does not replace the Data Standards

published by Texas Education Agency (TEA). The focus of an audit is to examine student records for purposes of

verifying the accuracy of the leaver information reported through the Public Education Information Management

System (PEIMS) and Appendix D, using agreed-upon procedures. The authority for the annual audit of leavers is

provided in Texas Education Code (TEC), §39.055. This manual includes procedures for coding leavers using the

appropriate leaver codes and for verifying the accuracy of the dropout information reported through the Public

Education Information Management System (PEIMS). These policies and procedures have been established in

partial fulfillment of House Bill 3459 and Senate Bill 894 requirements.

Summary: Both bills direct the agency to audit dropout records electronically. It authorizes the agency to

conduct on-site monitoring, based upon an analysis to risk factors.

This manual is a “living document” and is subject to be updated at least once a year based on releases from

Region 1 or TEA. Campus leaver audits will be performed twice a year by the Dropout Prevention Department to

ensure accuracy of leaver reporting and uniformity of procedures.

DEFINITIONS

Public Education Information Management System (PEIMS) collects and reports school district data systematically

through PEIMS. For purposes of information systems management and reporting, TEA requires four categories of

data collection from school district: 1.) organization; 2.) district finance; 3.) staff; and 4.) students. For each

category of data, a record layout formats is specified in the annual TEA publication, “Public Education Information

Management System: Data Standards.” Campus and district procedures for ensuring data quality for all record

layouts and submission are a shared responsibility between campus and district personnel.

Education Code 39.052 (b), (d); Board Policy AIA (Legal)

Under National Center for Education Statistics (NCES) definition, a dropout is a student who is enrolled in public

school in Grades 7-12, does not return to public school the following Fall, is not expelled, and does not graduate,

received a GED certificate, continues high school outside the public school system, begin college or die.

As required by the Texas State Legislature, the Texas Education Agency has adopted the standards of the National

Center of Educational Statistics for determining Completion Rates and for identification of Dropouts. The

standards used are those which are reported through PEIMS for accountability purposes.

Leavers are students who were served in grades 7 – 12 during the prior school year, but have not enrolled in the

district during the current school year. This includes graduates, court ordered GED, returns to home country,

enrolled in school out of state, enter a private school, enrolled home schooling, entered college, expelled,

removed by Child Protective Services, incarcerated in state or federal facilities, died or dropped out. Accounting

for these students is the shared responsibility of campus and district personnel and is accomplished through

PEIMS’ Leaver Records. The leaver record requires the district to assign a reason code explaining why the student

did not re-enroll in the district in accordance with established documentation standards. For accountability

purposes, TEA uses leaver records to calculate a district’ dropout rate and completion rate. Leaver data is

reported in PEIMS Submission 1 using a 203 record. A 203 record must be reported for all students who

graduated during the prior school year, including the summer of the prior year. Leaver data can be corrected until

the resubmission of FALL PEIMS data in mid-January. Once records are finalized, they cannot be changed. Leaver

records are reported for all students who graduated during the prior school year, including summer graduates up

through August 31st. Students who have previously completed all graduation requirements in one school year,

but do not pass End of Year Courses until the next year are reported as graduates in the year in which the state

assessment test was passed. Students who complete a GED program are not reported as graduates.

Key point: Leavers require documentation.

Movers are students who move or officially transfer from one Texas public school district to another and is

reported to PEIMS by another district and/or ends up at a private school. Movers include students who enrolled

in another Texas public school, earned a GED certificate, withdrew to a health care facility outside the district,

incarcerated in a Juvenile Probation Office. These are students 17 and younger where the facility will provide

mandatory education for them. Key point: Movers require verification.

Underreported student is a student in Grades 7-12 served the previous year for which the district has not

submitted either enrollment records for returning students or leaver records for graduates, dropouts, and other

leavers. A list of underreported student is available after the completion of the FALL PEIMS Submission deadline.

The PEIMS department will deliver the reports to the campuses for review. Any corrections that need to be made

to student leaver data will be made. PEIMS leaver records will be resubmitted to TEA. Final leaver reports and

underreported information will be available at the completion of the FALL PEIMS resubmission in mid-January.

Campuses must clear all applicable students from its underreported list.

Over reported student is a student for whom leaver records are received when none are expected.

Personal Identification Database/System (PID) is used by the Texas Education Agency (TEA) to manage and store

identifying data on individuals who are reported to TEA. The PID database should contain one and only one

record for each person with the following demographic data: 1.) Social Security Number/Alt. ID; 2.) First, Middle,

and Last Name, Generation Code, Maiden Name; 3.) Date of Birth, Sex Code, Ethnic Code.

Accountability Rating System is a system used to measure the performance of school districts. This rating system

integrates the state’s public education information for state level reporting. The dropout information is an

essential component in the rating system.

COMMON MISTAKES IN LEAVER REPORTING

District does not submit a leaver record.

District does not submit an enrollment record.

District does not follow up on a student who intended to enroll elsewhere to see if he or she actually

enrolled.

District withdraws a student using a leaver code that is no longer valid.

District does not document why a student left.

District knows why a student left but submits the wrong code on the 203 record.

District creates a duplicate PID and the student becomes underreported because records do not match.

District does not use the PIEMS Data Standards Appendix D Leaver Reason Codes and Documentation

Requirements.

DROPOUT RATE

Once the leaver reporting process is completed, TEA calculates the annual dropout rate for each campus and

district with Grade 7-12 enrollment, for all students. These rates, together with the State of Texas Assessments of

Academic Readiness (STAAR) and attendance rates, serve as an academic excellence indicators and are used to

determine an accountability rating for each district and campus.

Only students reported with selected PEIMS leaver codes are defined as dropouts. Once all districts have made

data submission, TEA runs an automated check against other state data sources to attempt to locate reported

dropouts in other educational settings. District and campuses are held accountable for their official dropouts such

as those whose records are not excluded by this process. For example, if a reported dropout is discovered to

have remained enrolled in a public school somewhere in the state, received a GED certificate, or graduated from a

Texas public school, the record is excluded from the official dropout count. TEA then determines, on a district by

district basis, counts or returning students, over-reported and underreported student records, graduates,

dropouts, and other leavers. Underreported student records, as percentages of reported students, are also

calculated.

CALCULATING ANNUAL DROPOUT RATES AND LONGITUDINAL RATES (COMPLETION RATES)

Annual Dropout Rate definition is the percentage of students who drop out of school during one school year.

Calculation definition is the number of students who dropped out during the school year divided by the number

of student enrolled during the school year.

Completion Rate definition is the percentage of students from a class of beginning 9th grade (cohort) who

complete their high school education by the anticipated graduation date. The cohort includes student who

transfer in during the second, third and fourth years.

Completer definition is a student who graduates, continues high school in the fall after expected graduation, or

receives a GED.

Continuer definition is a student enrolled in the school-start window in the fall after expected graduation (5 th year

of high school).

Longitudinal Dropout Rate definition is the percentage of students from the same class who drop out before

completing their high school education.

The annual dropout rate is the count of dropouts summed across all grades, 7-12, divided by the number of

students summed across all grades 7-12.

It is calculated as follows:

Number of students who dropped out during the school yearDivided by

Number of students who were in attendance at any time during the school year.

Note that a cumulative count of students is used in the denominator as well as the numerator. This method for calculating the dropout rate neutralized the effects of mobility by including the denominator every student who enrolled at the school throughout the school year. If the student dropped out, the student was counted as a dropout for the district last attended (as well as for the campus where the student was enrolled in that district).

RESPONSIBILITES

District Responsibilities

The Superintendent of School, his/her Assistant, and Executive Director for Student Services shall assume the

responsibility of compliance for the leaver and dropout data for all secondary campuses. In conjunction, the

District Dropout Prevention Coordinator and PEIMS Specialist will support the Executive Director for Student

Services in the documentation and compliance of the leaver and dropout data. Computer Services shall provide

the Dropout Prevention Coordinator timely reports of all students who attended a campus in grade 7 th through

12th for the previous year and who have not attended a district campus in the current year. A duplicate copy will

be provided to the Executive Director for Student Services. The Campus Principal with the Dropout Prevention

Coordinator will distribute the reports and assist campus staff and Campus PEIMS Coordinators, specialist and/or

clerk with the documentation and verification of the leaver information. The District PEIMS Specialist will provide

no less than three PEIMS training sessions per year, usually two in early September for the Fall Submission and

again in March. The district will develop an orientation program for PEIMS Data Standards. Orientation to be

held before the school start window. Tentatively, the as of school start window is August 21, 2017 until

September 29, 2017. For the summer submission, the District PEIMS Specialist shall assist the Campus PEIMS

Coordinator and clerks with training on the student administrative software (Skyward) for or coding/data entry of

documentation on leaver and dropout data. Central Office personnel may not edit codes, or add/delete students

from Student Accounting System or PEIMS databases without a signed computer request from the campus

principal or Campus PEIMS Coordinator. The Assistant Superintendent of Student Services in conjunction with the

Executive Director of Student Services will establish a District Task Force Committee to ensure that campuses are

kept up to date on latest information received from TEA and collaborate with other departments to reduce the

dropout rate. The Dropout Prevention Department will conduct staff development for Campus Graduation Task

Force members. The annual leaver training is completed for secondary campuses and will include elementary

campuses as of October 25, 2017 in an effort to provide consistency across all campuses. The training includes a

review of leaver codes, documentation expected in student folders, and data entry training for withdrawals.

Internal audits will be conducted in the fall and spring as needed to review data quality and required

Campus Responsibilities

Accurate reporting of leavers results from a campus level coordination of administration, attendance personnel,

registrars, data clerks, counselors, teachers and contacts with the student or the student’s family and

acquaintances, as well as any other parties with knowledge of the student’s status, location or intentions. While

campus coordination of efforts is essential, the importance and impact of the accountability ratings attached to

completion and dropouts cannot be overstated. Complex situations will need team work and organization to

resolve. Since withdrawals occur throughout the school year, regular collaboration must be used for accurate

coding, research and identification of dropouts and possible dropouts.

The ultimate campus responsibility for accurate Leaver Reason records rests with the principal who confirms

PEIMS submissions. The principal of each campus with the responsibility for verifying and assuring that accurate

leaver data is reported in the PEIMS Fall submission. The principal will ensure that the procedures, outlined in this

document, are followed. In addition to the procedures outlined here, the principal will ensure that all of the

leaver data is processed according to the PEIMS Data Standards. PEIMS Data Standards provides applicable

accordance codes and supplemental Appendix D to the Data Standards describes the extent of the documentation

required. These documentation standards are to be strictly followed. Data quality is paramount and must be

DESIGNATED STAFF

The Principal shall, at the beginning of each year, designate the professional staff member who will be responsible

for tracking leavers and leaver documentation. In the event that the staff member is not available, an alternate

professional staff member will be designated. This information will be published to staff and made a part of

campus procedures. The designated staff member will act as the authorized representative of the superintendent

for signing withdrawal forms and other leaver documentation.

Those individuals assigned the responsibility of overseeing the leaver process must thoroughly familiarize

themselves with the PEIMS Data Standards as they related to the leaver documentation process. All questions

and concerns shall be routed through Campus PEIMS Coordinator. Should there be a need for further

clarification; the Campus PEIMS Coordinator will contact the Executive Director for Student Services.

Administrators assigned this responsibility must assure not only their own proficiency in this process, but should

also assist, and monitor all involved in the process. The District PEIMS Coordinator, specialist, and clerk will

provide training through the “Training of Trainers” method. Campuses PEIMS Coordinators will take all

information disseminated at the training sessions and instruct appropriate campus personnel. The Campus PEIMS

Coordinator and campus Principal shall be responsible for ensuring that all assigned data clerks are cross-trained

at the campus level.

The principal shall assign designated staff to participate in the Campus Graduation Task Force Committee to

ensure all required steps are taken during the withdrawal process, proper signatures are obtained,

documentation is created and withdrawal process is completed. In addition, the social worker at each secondary

campus shall be conducting campus leaver audits on a weekly, bi-weekly or monthly basis. Campus Graduation

Task Force Committee will be delegated the task of dropout recovery. The campus PEIMS attendance

clerk/registrar will be primary charged with coding withdrawals and/or making updates on Skyward after the

administrator in charge of leaver assigns a leaver code. Education Code 11.255; Board Policy BQA (Legal)

IMPORTANCE OF SCHOOL START WINDOW

The act of not showing up the first day of school does NOT make a student a dropout. A student has until the end

of the school start window (September 29, 2017) to be enrolled within public education in Texas, or to classify for

a Leaver Reason. It is especially important at the start of the year to identify and investigate students who do not

return immediately. If they can be enrolled before the end of the school start window, then the designation of

“Dropout” can be avoided. Even if a student returns after the school start window and graduates that same year,

he or she will counted as a dropout due to not being enrolled during the window and not be classified under a

non-dropout leaver reason. Students who return after the school start window should be questioned regarding

their enrollment status DURING the school start window. If an approved leaver reason can be documented, the

student’s last leaver record (following his/her last withdrawal) may be changed to reflect enrollment in another

educational setting and avoid the designation of dropout.

IMPORTANCE OF CONTACT INFORMATION

Every effort should be made during the school year to collect as much contact information for students as

possible. Multiple contact numbers and names of family members and other acquaintances who could be

referenced if necessary. Returned mail should be investigated for a change of address.

DATA QUALITY STEPS

No Shows must be posted to the file in a timely manner by the end of the first day of the school year.

Failure to delete PET events weekly may cause a dropout leaver to be overlooked.

Check the spelling of the name and the social security number against the birth certificate and social

security card. If a request for records is received, be sure to check the name and social security number

against your records. If the name or PEIMS number is different, check the TEA Person Identification

Database (PID) to make sure that the same student is not listed twice. Contact the Office of Evaluation

and Accountability to help in resolving a PID error.

Check the PID by entering the last name and birth date to see if there could be duplicates for a student.

Check the PET Leaver Report for possible dropouts.

A student counted as a dropout could have enrolled after the school start window of the first day of

school through the last Friday in September. If a returning LJISD student enrolls after the school start

window, school personnel should check to verify the student’s whereabouts during the school start

window and adjust the leaver record to reflect where the student was.

Check requests for records of student transferred to other LJISD schools. If no record is requested, verify

that the student was not really a No Show.

Pay special attention to reliable and stable contact information for At-Risk students, migrant students,

recent immigrant students, homeless students and other students with a history of mobility, truancy or

disaffection.

Communicate frequently with parents, students, and shelters about the importance of informing the

school of any possible changes in residence, attendance or enrollment.

Special consideration of students should be made at the time of referral to HOPE Academy or before any

know period of interrupted attendance. A very common category of dropout exists among such students.

Prior to withdrawal from the referring campus, a plan should be developed for successful re-entry to the

referring campus or transition to a different school. Regular communication with such students should be

maintained throughout the referral.

CAMPUS GRADUATION TASK FORCE COMMITTEE

The Campus Graduation Task Force Committee is the single most important entity in dropout prevention at the

campus level. The importance of this Professional Learning Community, which assumes the role of monitoring the

successful engagement and progress of the campus’ at-risk population, cannot be understated. Campuses with

proactive, well-organized, and motivated committees regularly experience success in their dropout prevention

efforts. Campus with committees that meet at irregular intervals, have few if any defined and assigned roles, or

that simply focus on reacting to specific situations often are unsuccessful in meeting dropout reduction goals. The

membership and action of the committee is an indicator of the level of importance and urgency that campus staff

have regarding dropout prevention. It is the responsibility of the Campus Graduation Task Force Committee to

assess during their regularity scheduled meetings that the documentation requested is acceptable and follows

guidelines established in Appendix D. Education Code 11.255; Texas Education Data Standards: Appendix D;

Board Policy BQA (Legal)

EVALUATION OF LEAVER RECORDS BY CAMPUS GRADUATION TASK FORCE COMMITTEE

Merits of leaver documentation are assessed at the time the documentation is requested during a data inquiry

investigation by the Campus Graduation Task Force Committee. Determination of the acceptability of

documentation is made by the Campus Graduation Task Force Committee conducting the investigation. These

guidelines describe the most common types of documentation the audit would expect to find supporting use of

each leaver reason code. Other documentation that represents good business practice and shows a good faith

effort on the part of the campus/district to properly report leaver status will be evaluated on case by case basis.

COMPLETENESS OF DOCUMENTATION

Withdrawals documentation shall be considered incomplete without a date, signature(s), and destination.

Documentation will not be deemed insufficient when information is missing because the parent or parents refuse

to provide information requested by the district. A district should document at the time of the conversation that

the information was requested, and the parent refused to provide it. Appropriate documentation of a parent

refusal to provide information includes the date, content of conversation, name of the person with whom the

conversation was conducted, and the signature of the school official verifying the conversation. Education Code

11.255; Board Policy BQA (Legal)

WITHDRAWL PROCEDURES

A student should be withdrawn from the attendance accounting system (Skyward) on the date the campus

becomes aware the student is no longer a member of the district. With proof of enrollment in a different

district/campus, retroactive withdrawals are permitted to the day a student enrolled in another school. All

attendance accounting records affected by such a change must be updated. By law, each district is required to

transfer student records within 10 working days of receipt of a request by the receiving district, and the student

record must be transferred via the Texas Student Records Exchange (TREx). Refer to the District Student

Attendance Guidelines.

The district may withdraw a student who is at least 19 years old and is voluntarily enrolled in school when he or

she accumulates more than five unexcused absences in a semester. The district may revoke the enrollment of

such a student for the remainder of the school year, except that a district may not revoke enrollment on a day on

which the student is physically present at school. A student who is removed from school under this provision will

be considered a dropout for accountability purposes unless the student returns to school during the school-start

window the following fall. (Secondary School Completion and Dropout in Texas Public Schools)

The following procedure will be used by every campus to record leavers:

Student who withdraw during the school year. Completion of an exit review with the student’s parent/guardian, or the adult student conducted by an

authorized campus staff member. During the interview all pertinent information as it relates to any new

educational setting and contact information should be obtained and documented in the appropriate

forms.

Completion of withdrawal forms recording the reason for the student’s leaving and as much future

contact information as possible. The withdrawal form must be signed by either parent/guardian or adult

student, if available and school personnel designated as authorized to do so at the time of the withdrawal.

Documentation on the withdrawal form must be complete, signed in black ink and dated. The

documentation standards in the PEIMS Data Standards are to be firmly adhered to. Every campus will use

the computerized Student Accounting System withdrawal form.

Designated campus personnel should begin written and oral inquiry to confirm a leaver’s enrollment in

another educational setting should acceptable documentation not be received within 10 days following

the departure of a student whose stated intent was to enroll in another district.

Any student who was enrolled for at least one day last year is a potential true dropout.

Any student who does not enroll the first day of school is considered a “No Show” and potential true

dropout.

Student who do NOT withdraw but quit attending school: Withdraw the student in accordance with District policy.

The Campus PEIMS Coordinator will complete a withdrawal form and oversee the data entry coded

through Student Accounting System.

Every effort will be made to determine the whereabouts of the student. Designated personnel will begin

written and oral inquires to confirm a leaver’s enrollment in another educational setting.

If the student’s whereabouts cannot be ascertained, this student will be recorded as a dropout.

A list of potential dropout and numbers will be provided to Central Office Staff by the campus principal’s

dropout prevention designee on a daily basis during as of start window.

All graduates including those who graduate early during the school year and summer graduates: These students graduation status must be documented on their AAR record. The cumulative record must

have graduation seal, the graduation type plan, EOC results and sufficient credits.

Designated Personnel must review the additional PEIMS fields for these levers (special education

indicator, LEP, migrants, economic disadvantaged, and graduation/completion date and college entry.)

Student who fail to return at the beginning of the next school year: “No Show” student are deactivated 2nd period in the student attendance accounting system by the end of

our first school day. There is no option to this entry. No show is those students who attended all last year

but failed to return for the next school year.

Designated personnel should make every attempt year round to locate these students and assign an

appropriate leaver code. If the student’s whereabouts cannot be ascertained, this student will be

recorded as a dropout (leaver code 98-unknow.)

A leaver tracking form will be used to document all attempts to location of the student.

Refer to the PEIMS Data Standards regarding record types, status codes and leaver code application.

Student who returned at the beginning of the next school year, but are NOT Enrolled on the snapshot date:

DATA ENTRY

Campus PEIMS Coordinators are responsible for assuring that PEIMS leaver codes are transmitted to the assigned

staff. This information should be provided in a timely manner so that periodic reports may be generated.

Assigned personnel must be trained in entering, changing and deleting data from the student software.

All entries, changes and deletions require supporting documentation. Any changes made to original

documentation must be initialed and dated by the person making the change.

Coding decisions are not made by data clerical personnel. Coding decisions are made by authorized campus

professional personnel only. Education Code 39.052 (b), (d); Board Policy AIA (Legal)

MONITORING LEAVER

Each campus will generate a final “NO SHOW” list no later than 5:00 p.m. current day containing the same

information as shown above. Each name on this list must have a leaver tracking form on file.

Each campus will generate a final leaver report that contains all leavers from the current school year.

This will be an alpha Leaver 203 report containing the following information:

Social security number or Alternate identification number;

Student's full name;

Leaver reason;

Dropout indicator;

Date of graduation; and graduation type (if applicable);

Economic disadvantaged code;

Special education indicator;

Sex; and

Ethnicity

Each campus will, on a six weeks basis during the school year, print a withdrawn student’s report and conduct an

audit of leaver documentation. The administrator managing the leaver process will conduct regular internal

audits of the documentation process. Monitoring a student leaver to the point of acceptable documentation

should involve all personnel assigned this responsibility and may involve home visits by personnel responsible for

such activities.

If local leaver reason codes are applied, all data will be reviewed to assure that crosswalks to PEIMS leaver codes

have been accurately converted. The District PEIMS Coordinator will maintain PEIMS tables up to date. These

tables are updated at the beginning of each PEIMS submission and on and as needed basic throughout the school

year.

Software systems including the extracted file (file sent to TEA) are reviewed to ensure that the proper PEIMS

codes are be used.

REVIEW OF PEIMS EDITS AND QUALITY ASSURANCE REPORTS

District PEIMS Coordinator, social worker and PEIMS clerk will review all PEIMS errors concerning the 203 records

for data accuracy and provide corrective feedback to each campus, staff and/or central office personnel. The

PEIMS edit reports will be distributed to campuses for review and possible corrections. If the errors cannot be

corrected, the personnel will annotate the reason. These remarks will be maintained for audit purposes.

District PEIMS Coordinator, social worker and PEIMS clerk review the list of under-reported and over-reported

leavers generated in the PEIMS system. PEIMS edit reports will be distributed to campuses for review and

possible corrections. In each instance where the data cannot be corrected the designated personnel will annotate

the reason. These remarks will be maintained for audit purpose. (Refer to page 12)

If the status of a student found in the list of underreported cannot be determined, the student will be assigned a

leaver code of “98”.

District PEIMS Coordinator, social worker and PEIMS clerk will assist campuses with the review and correction of

all Personal Identification Database (PID) errors. PID errors that require manual intervention by TEA will be

forwarded to the Central Office for processing no later than the Education Service Center’s due date. All errors

will be corrected no later than the campus deadline for validating data.

In addition to general documentation requirements, requirements specific to leaver reason codes also apply.

Signatures on documentation are required. Documentation must be signed and dated by an authorized

representative of the district. The district should have a written policy stating who can act as an authorized

representative for purposes of signing withdrawal forms and other leaver reason documentation.

Withdrawal documentation should also be signed by a parent, guardian, responsible adult as recorded in school

records, such as foster parent, or a probation officer, or qualified student. A qualified student is one who, at the

time he/she stops attending school is married, or is 18 years or older, or has established a residence separate and

apart from the student’s parent, guardian, or other person having lawful control of the student.

An original signature is not required on withdrawal forms received in the district by fax. Withdrawal forms

received by e-mail do not need to be signed by the parent/guardian or qualified student. Written documentation

of oral statements made in person or by telephone by the parent/guardian or qualified student is acceptable

documentation in some situations if it is signed and dated by the district representative. Education Code 39.052

(b); (d); Board Policy AIA (Legal)

STUDENT LEAVER RECORD

Accurate record keeping and maintenance of the student leaver records are of utmost importance. These records

are auditable by the Texas Education Agency as well as other organizations. The following guidelines will help

campuses maintain accurate records.

1. PERSON/S RESPONSIBLE FOR MAINTAINING LEAVER AND MOVER RECORDS: The attendance clerk is responsible for the maintenance and filing of leaver and mover records. However,

the principal is the custodian of records for their campus. (FL-local policy)

2. PREPARING LEAVER RECORDS:

The attendance clerk will create a leaver record for each student classified as a leaver using:

a straight cut, letter size, red folder

11/2” X 11” labels (Avery White Full Sheet Labels Product No. 5353)

Labels must include student’s legal name, grade level, date of birth, student identification,

campus name, leaver code, school year.

3. DOCUMENTS IN THE RED FOLDER:

Documents in the Red Leaver Folder should be filed chronologically with the most recent information at

the top.

4. LEAVER RECORDS MUST INCLUDE THE FOLLOWING:

Most recent leaver code using the LJISD code change form (if applicable)

Completed withdrawal form

Completed exit interview form

Documentation to support assigned leaver code as noted in Appendix D and the district

guidelines.

5. DESIGNATED STAFF:

Each campus will designate a “Campus Graduation Task Force” to work on identifying and re-engaging

students who have not enrolled in school. The campus principal will appoint members to the Campus

Graduation Task Force Committee. Recommended team members include but are not limited to: Campus

Administrator, Attendance Clerk/s, Social Worker, Migrant Strategist, Special Education Counselor,

*Truancy Officer, *ESL Specialist. (*) High School personnel. While the “Campus Graduation Task Force

Committee” does most of their work during the School Leaver Start Window (the first day of school to the

last Friday in September), the job of re-engaging students continues throughout the year.

6. LEAVER CODE CHANGE:

When working with Leavers, there are times when a code change is needed. If your latest documentation

supports a code change, use the district form titled “La Joya ISD Code Change” found in Section entitled

“Forms” of the District Leaver Manual. Fill out the information requested, including reason for code

change. Indicate who is making the request for change and ensure an administrator authorizes the

change by signing the form. Update the code on the Red Leaver Folder Label to reflect the latest

information.

7. RESPONSIBILITIES FOR DOCUMENTATION OF LEAVERS/MOVERS:

The social worker and/or attendance clerk who work with the Leaver Tracking Process must adhere to the

PEIMS Data Standards for leaver documentation. Social Workers will continue to clear code 98’s weekly.

Forms must be signed by the campus staff auditing folder and dated using complete names and titles.

See Section 4 of the Leaver Manual – PEIMS Data Standards Appendix D Leaver Reason Codes and

Documentation Requirements. Refer to leaver codes.

8. CAMPUS AUDITS AND REVIEWS :

Campuses will conduct weekly or bi-weekly audits of leaver folders using the Leaver Audit Guide. If an

error is found, it should be documented on the “Discrepancy Form” which must be signed and dated by

the person auditing the record.

Campuses will review leaver records for: Most recent leaver code using the LJISD code change form (if applicable),

Completed withdrawal form,

Completed exit interview form, and

Supporting documentation for assigned leaver code as noted in Appendix D and the district

guidelines.

When conducting an exit interview, or completing a leaver tracking form and/or home visit form,

administrator must sign if it will support a leaver reason code as per Appendix D.

For Example: Code 16 (Returning to Home Country) document must include an exit interview, or

leaver tracking form and must indicate the city, state or country that the student is returning to.

An administrator must sign indicating the Leaver Reason code.

9. DOCUMENTING LEAVER RECORD DISCREPANCY : Document any “concern” regarding a leaver record using the “Leaver Record Discrepancy Form”. State

the problem and write the corrective action the campus will take to ensure the same mistake/s do not

happen again. Leaver Record Discrepancy Form can be found in Section entitled “Forms” of the District

Leaver Manual.

10. LEAVER RECORD REVIEWS: The attendance clerk is responsible for creating a Leaver Record Review Log form to document individuals

requesting to work on leaver records. When an administrator, graduation specialist, social worker or

counselor need to review a leaver record, use the Leaver Record Review Log form. If the campus is

conducting a regular campus audit, keep a copy of the records reviewed. Each reviewed folder should

contain a copy of the audit review form signed and dated by the auditor.

11. LEAVER RECORD TRANSFERS: A Transfer of Leaver Records form must be completed by a campus administrator before a record is

transferred to another campus. Keep a copy of the signed and dated transfer form in a binder. This

binder is to be maintained by the attendance clerk in charge of leaver records. The administrator and/or

attendance clerk receiving or transferring the leaver folders must sign-out or sign-in the folders. The

receiving campus will document when the leaver folders are received. (See Transfer of Leaver Records

from Campus to Campus form)

12. STORING OF STUDENT LEAVER RECORDS: All student leaver records must be stored in a safe and secure environment with lockable filing cabinets.

Access to leaver records is restricted to authorized school personnel only at the principal’s discretion. Use

the “Storage Container Transmittal Form” found in Section entitled “Forms” of the District Leaver Manual

to label each box. All folders must be arranged by code and placed in appropriate boxes. All leaver

records must be kept on campus for a period of seven years and be readily available for auditing

purposes.

processing under the guidelines of Records Consultants Inc. during the yearly record retention pickup

campaign.

14. CENTRAL OFFICE SUPPORT STAFF : Dropout Prevention Coordinator, Public Education Information Management System (PEIMS) Specialist,

and Graduation Specialists for each area are available for support.

DROPOUT PREVENTION AND RECOVERYDropout Recovery is the act of re-engaging a student in school after the student has ceased attending.

Dropout Prevention is the act of identifying students at-risk of dropping out and applying effective safeguards,

monitoring and interventions to assure continued attendance in school until graduation.

DROPOUT RECOVERY BEST PRACTICES

Students with BarriersOften student who have chosen to not return to school are dealing with serious barriers affecting their ability to

attend school. Such barriers may include family economics, pregnancy, child-care, substance abuse,

transportation, health/medical issues, psycho-social issues, and safety/security issues. Some students may be

encouraged by parents or other family members to leaver school for the purpose of supporting the family in some

way.

InterventionSocial services or referrals should be made for student with barriers so that the issues can be remedied, allowing

the student to return to school. School counselors, social worker or nurse should have a list of agencies working

directly with the school district who could provide assistance. (Refer to Out of District Referral List in Appendix)

Disengaged StudentsDisengaged students include those who are over-age and whose peer group (cohort) has already graduated,

students who have been separated from regular schooling due to incarceration, truancy, expulsion or

victimization. Students who have career interests other than those offered by the school often become

disengaged through boredom or perceived lack of relevance. Other disengaged students include those who have

experienced a profound lack of success in school and/or had traumatic experiences at school with staff or peers.

Students who have become involved in certain subcultural groups such as gangs, cults, thugs, goths or stoners

may depreciate academic success.

InterventionCounseling, family involvement, mentorship and scheduling options should be explored to identify appropriate

interventions for disengaged students which may include transferring to different campuses and exploring other

school options outside the district (Refer to Out of District Referral List in Appendix)

Students with Unknown Whereabouts’Most often than not, dropouts are students who are unable to be located.

Interventions:After telephone calls and home visits have failed to locate the student, and perhaps his/her family, the following

search measures should be taken:

Check School Records

Inspect Cumulative Folder/Permanent Record for other contact information.

Review siblings’ records for contact information.

Check with school nurse for emergency contact numbers.

Check with Community

Ask neighbors about missing students and their families.

Ask the apartment leasing office or Housing Authority about any forwarding information for the family.

Check with former employers.

Check with Peers

Check with classmates.

Try to contact known friends or former coworkers.

Social Networking Media such as Twitter, Instagram etc.

Check with Government Resources

Check Jail records

Check with County and Municipal Court/Probation Records

Check with Police Department including reports for runaways

Effective cohort management will result in a list of students who have dropped out of the cohort during each

of the cohort’s years of existence. A continuous effort should be made to locate or communicate with all

missing members of the cohort. Education Code 29.918; TAC 89.1701; Board Policy EHBC (Legal); FEA (Local)

FERPA

In accordance with the Family Educational Rights and Privacy Act 34 CFR § 99.35, federal, state, and local

educational authorities may access educational records as part of an audit, evaluation of a state or federally

supported education program, or for enforcement of or compliance with the legal requirements of these

programs. Thus, this provision applies to the auditors as authorized representatives of the state and local

educational authorities. Auditors will maintain confidentiality of the records.

FERPA gives parents certain rights with respect to their children’s education records. These rights transfer to

the student when he or she reaches the age of 18 or attends a school beyond the high school level. Students

to whom the rights have transferred are “eligible students”.

Parents or eligible students have the right to inspect and review the student’s education records

maintained by the school. Schools are not required to provide copies of records unless, for reasons

such as great distance, it is impossible for parents or eligible students to review the records. Schools

may charge a fee for copies.

Parent or eligible students have the right to request that a school correct records which they believe

to be inaccurate or misleading. If the school decides not to amend the record, the parent or eligible

student then has the right to a formal hearing. After the hearing, if the school still decides not to

amend the record, the parent or eligible student has the right to place a statement with the record

setting forth his or her view about the contested information.

Generally, schools must have written permission from the parent or eligible student in order to

release any information from a student’s education record. However, FERPA allows schools to

disclose those records, without consent, to the following parties or under the following conditions

(34 CFR § 99.31):

School officials with legitimate educational interest;

Other schools to which a student is transferring;

Specified officials for audit or evaluation purposes;

Appropriate parties in connection with financial aid to a student;

Organizations conducting certain studies for or on behalf of the school;

Accrediting organizations;

To comply with a judicial order or lawfully issued subpoena;

Appropriate officials in cases of health and safety emergencies; and

State and local authorities, within a juvenile justice system, pursuant to specific State law.

Schools may disclose, without consent, “directory” information such as a student’s name, address, telephone

number, date and place of birth, honors and awards, and dates of attendance. However, schools must tell parents

and eligible students about directory information and allow parents and eligible students a reasonable amount of

time to request that the school not disclose directory information about them. Schools must notify parents and

eligible students annually of their rights under FERPA. Board Policy FL (Legal)

RECORD RETENTION AND STORAGE SAFEGUARDS

Campuses will keep a red folder labeled “Withdrawal Form/Leaver Documentation” in their possession for 7

years after the end of the completed school year. The folder label must include the student name, ID#,

campus, grade level and school year. All folders must be alphabetized (by grade level) placed in boxes and

labeled accordingly with the “Storage Container Transmittal form found in Section entitled “Forms” of the

District Leaver Manual. Boxes are to be stored in as safe and secure location within the campus. In case of

internal audits or audits conducted by TEA, the folders will be available for inspection for 7 years at the

campus. After the 7th year retention period, the boxes will then be forwarded to the Office of Admissions &

Records for processing under the guidelines of Records Consultants Inc. during the December district-wide

records pick-up campaign.

SKYWARD STUDENT ACCOUNTING SYSTEM

The information in the Skyward Student Accounting System is stored. A full backup of the entire system

assures that records are stored in a manner that is readily accessible in order to meet audit requirements.

For storage of paper documentation, campus staff will use the documentation standards as outlined in the

Attendance Accounting Handbook and in the PEIMS Data Standards. PEIMS records are retained and

subsequently destroyed in accordance with State requirements. PEIMS records are maintained for a period

of 7 years.

Our district uses the Skyward Student Accounting System and is PEIMS compliant. Education Code 39.052

(b), (d); Board Policy AIA (Legal)

COMPUTER SYSTEM SAFEGUARDS

Security software is used to monitor access to the computer system and the database that a user wishes to

access. The Student Accounting System and PEIMS database are password protected and allow only those

users who have been given access to these databases to logon to the computer system. The security

software monitors the time that a user is on the system and exits anybody off of the computer when there

has been no activity for over an hour. This safeguards the database from the user who walks away from their

station and leaves the connection open to the computer for one hour.

APPENDIX D

Leaver Reason Codes and Documentation Requirements

The attached table provides expanded definitions and specific documentation guidelines for each of the leaver reason codes listed in Code Table C162 of the Texas Education Agency (TEA) PEIMS Data Standards. The table is organized into the following broad categories of leavers:

completed high school or General Educational Development (GED) program moved to other educational setting withdrawn by school district other

Leaver records are not submitted for students who enroll in other Texas public school districts or charters and students who obtain GED certificates at Texas examination sites by August 31. Students who move to other Texas public school districts are considered movers, not leavers. Documentation as described in Appendix D is not required for movers; districts may wish to develop local policy on documentation for movers. The Person Identification Database (PID) Enrollment Tracking (PET) may be used to establish tentative local documentation that students are movers. The final determination of whether students are movers will be made by TEA.

Local policy in this appendix refers to a district's or charter's leaver policies and procedures.

General Documentation RequirementsDistricts must document the withdrawal of students and maintain on file the appropriate paperwork associated with student withdrawals. Documentation is required for all leaver reason codes. Documentation requirements for leaver codes are provided in the Documentation Requirements by LEAVER-REASON-CODE section. Documentation supporting use of a leaver reason code must exist in the district at the time the leaver data are submitted, i.e., no later than the PEIMS Submission 1 January resubmission date.

Timelines for establishing leaver reasons and obtaining documentation.

Students who leave during the school year.

For students who leave during the school year, leaver reasons apply at the time of withdrawal, and documentation should be obtained at that time. For example, for students who are withdrawn by Child Protective Services (CPS), LEAVER-REASON-CODE 66, documentation would be obtained when the student is removed.

Students who fail to return in the fall.

For students who fail to return the following Fall, leaver reasons apply on the first day of school or its approximation, the school-start window. The school-start window is the period of time between the first day of school and the last Friday in September. Districts should use the LEAVER-REASON-CODE that most appropriately describes the student's whereabouts during the school-start window. For example, to use LEAVER-REASON-CODE 60 student withdrew from/left school for home schooling, a district would establish that a student was being home schooled at

some point during the school-start window. The district could obtain the documentation to support the leaver code at any time up until the PEIMS Submission 1 resubmission date.

Students who were attending and were withdrawn under LEAVER-REASON-CODE 83 when the district discovered that the student was not entitled to public school enrollment in the district.LEAVER-REASON-CODE 83, not entitled to public school enrollment in the district, applies to students who are attending and are withdrawn by the district because the district discovers, when verifying enrollment information, that the student was not entitled to enrollment in the district because the student was not a resident of the district or was not entitled under other provisions of TEC §25.001 or as a transfer student. It is not for a student who was a resident and stops attending because he/she has moved out of the district.

This code is also used for those rare situations in which the student has not met the requirements under TEC §38.001 or a corresponding rule of the Texas Department of State Health Services for immunization, provisional enrollment, or exemption. Leaver reasons for LEAVER-REASON-CODE 83 apply at the time of withdrawal and documentation showing due process supporting the withdrawal should be obtained at that time.

Signatures on documentation. Documentation must be signed and dated by an authorized representative of the district. The district should have a written policy stating who can act as an authorized representative for purposes of signing withdrawal forms and other leaver reason documentation.

Withdrawal documentation should also be signed for the student by a:

parent, or guardian, or responsible adult as recorded in school records, such as a foster parent or a probation officer, or

qualified student. A qualified student is one who, at the time he/she stops attending school:o is married, oro is 18 years or older, oro has established a residence separate and apart from the student’s parent, guardian, or

An original signature is not required on withdrawal forms received in the district by fax. Withdrawal forms received by e-mail do not need to be signed by the parent/guardian or qualified student. Evaluation of documentation Merits of leaver documentation are assessed at the time the documentation is requested during a data inquiry investigation. Determination of the acceptability of documentation is made by the professional staff conducting the investigation. These guidelines describe the most common types of documentation the investigator would expect to find supporting use of each leaver reason code. Other documentation that represents good business practice and shows a good faith effort on the part of the district to properly report leaver status will be evaluated on a case-by-case basis.

Completeness of documentation Withdrawal documentation shall be considered incomplete without a date, signature(s), and destination. Documentation will not be deemed insufficient when information is missing because the parent or parents refuse to provide information requested by the district. A district should document at the time of the conversation that the information was requested, and the parent refused to provide it. Appropriate documentation of a parent refusal to provide information includes the date, content of conversation, name of person with whom the conversation was conducted, and the signature of the school official verifying the conversation.

Changing LEAVER-REASON-CODEs Once a district meets the documentation standard that supports the leaver reason code used, the district is not required to obtain additional information on the student. Local policy will determine if an existing leaver reason code is updated for a student when additional information is received. The policy should be clearly stated in the district's published guidelines on leaver procedures. For example, if a district assigned LEAVER-REASON-CODE 60 student withdrew from/left school for home schooling for a student and later received a request for transcript information from a private school for the same student, the district may change the LEAVER-REASON-CODE to 81 enroll in a Texas private school but is not obligated to.

Graduated or received an out-of-state GED

01 Student graduated from a campus in this district or charter

Definition and use: Use for students who meet all high school graduation requirements (which includes passing the state assessments required for graduation) at any time during the prior school year, including the summer (through August 31) following the close of the prior year.To graduate, a student must satisfy the requirements under 19 TAC Chapter 74, Subchapter B. Special education students must satisfy requirements under 19 TAC §89.1070. Students who complete all course requirements for graduation in one school year but do not pass the state assessments required for graduation until a later year, are reported as graduates in the school year in which the state assessments are passed, and the diploma is issued.

Documentation requirement: Transcript showing sufficient credits, successful completion of the state assessments (including testing dates) required for graduation, graduation seal, school official signature, and date of completion.

85 Student graduated outside Texas before entering a Texas public school, entered a Texas public school, and left again

Definition and use: This code may be used for students who graduated in another state or country before entering Texas public schools. This code may also be used for students who graduated from Texhoma High School, Texhoma, Oklahoma.

Documentation requirement: Transcript showing sufficient credits, date, and school official signature, and a diploma with a graduation seal.

86 Student completed the GED outside Texas

Definition and use: This code may be used for students who earned GED certificates outside Texas, including students living in Texas and earning GED certificates online from a testing company in another state, before enrolling or after leaving Texas public schools.

Documentation requirement: Acceptable documentation is a copy of the GED certificate or some other written document provided by the testing company showing completion of the GED. Written documentation from the testing company must include the date of GED completion, location, address, and contact information of the company.

90 Student graduated from another state under provisions of the Interstate Compact on Educational Opportunity for Military Children

Definition and use: Per TEC §162.002, student lives in the household of an active-duty

military serviceperson, transferred into Texas public schools at the beginning of or during his or her senior year, did not meet requirements to graduate from Texas public schools, did meet requirements to graduate from a school in the sending state, and, under provisions of the Interstate Compact on Educational Opportunity for Military Children, graduated from a school or district in the sending state.

24 Student entered college and is working towards an Associate’s or Bachelor’s degree

Definition and use: This code is for students who leave secondary school to enter college early. It should be used for students who are enrolled full-time (at least 9 credit hours per semester). This code is also for students who leave school to enter a dual-credit program established by the Texas Legislature at the Texas Academy of Mathematics and Science at the University of North Texas, the Texas Academy of Leadership in the Humanities at Lamar University, the Texas Academy of Mathematics and Science at the University of Texas at Brownsville, the Texas Academy of Mathematics and Science at the University of Texas Rio Grande Valley, and the Texas Academy of International Studies at Texas A&M International University.

Documentation requirement: Documentation of enrollment in a college or university must indicate that the student is enrolled full-time in an academic program. Per federal requirement, it is not permissible for a district to document that, at the time of withdrawal, the student intended to enter a post-secondary educational setting. Beginning with students leaving in the 2011-12 school year, a district must document that the student has actually entered a post-secondary educational setting.

One of the following types of documentation is required to verify enrollment:Transcript request. Acceptable documentation of enrollment in college is a records request from the college in which the student is enrolled. Telephone requests must be documented in writing, including the date of the call, the name of the college requesting the records, the name of the person making the request, and the name of the person who received the call. Telephone requests should appear on a standardized, district-approved form. The original of the form should be included in the student's permanent file. Documentation of the method of records dissemination also must be included in the student’s permanent file (e.g., copy of fax activity log, certified mail receipt, encrypted email receipt confirmation, or postage/mail log with complete address information for requesting school).

Verification by an authorized representative of the college. A signed letter from the college verifying enrollment is also acceptable documentation. The letter must state the name and location of the college in which the student is enrolled and the date of enrollment. Other acceptable documentation is written documentation of an oral statement by a representative of the college providing the name and location of the college and verifying that the student is enrolled, signed and dated by an authorized representative of the district.

Verification by the parent/guardian or qualified student. Acceptable documentation includes a letter, signed and dated from the parent, guardian, or qualified student stating that the student has enrolled in college in a program leading to an associate’s or bachelor’s degree.

60 Student is home schooled

Definition and use: Student is being home schooled. This code may be used only for a student whose parent/guardian confirms that the student is pursuing, under direct supervision of the parent/guardian, a curriculum designed to meet basic education goals. The district is not required to obtain evidence that the program being provided meets educational standards.

Documentation requirement: A district must document that the parent/guardian is home schooling the student. Per federal requirement, it is not permissible for a district to document that, at the time of withdrawal, the student intended to be home schooled. Beginning with students leaving in the 2011-12 school year, the following documentation is required to verify enrollment:

Verification by the parent/guardian. A letter, signed and dated, from the parent/guardian stating that the student is being home schooled is acceptable documentation. Letters from parents/guardians must indicate the actual date home schooling began.

66 Student was removed by Child Protective Services (CPS) and the district has not been informed of the student's current status or enrollment

Definition and use: This code applies only to Child Protective Services. Private agencies that provide asylum for students do not have the legal authority to remove students from school.

Documentation requirement: Acceptable documentation includes due process documentation supporting the withdrawal; a written statement, signed and dated by the CPS officer, including the CPS officer's name and contact information; or written documentation of an oral statement by a CPS representative that the child was removed, including the CPS representative's name, the date of the conversation, and the signature of the school official.

Student enrolled in a public or private school outside of Texas

Definition and use: Student is enrolled in a private school in Texas (code 81), the Texas Job Corps Diploma Program (code 81), or a public or private school outside Texas (code 82). Documentation of actual enrollment is required. This code is also used when a student moves from the district without withdrawing but the district receives a records request. If the student enrolls in another school in the district or another public school district in Texas, a leaver record is not submitted.If the district did not assign code 81 or code 82 when the student stopped attending, the district can change the original code assigned to the student when the records request or communication from the parent/guardian or qualified student is received. If the original withdrawal date for the student is later than the date the student enrolled in the other school, the withdrawal date must be changed and all attendance accounting records affected by this change must be updated.Documentation requirement: Per federal requirement, it is not permissible for a district to document that, at the time of withdrawal, the student intended to enter another educational setting. Beginning with students leaving in the 2011-12 school year, a district must document that the student has actually enrolled in a private school in Texas, the Texas Job Corps Diploma Program, or a private or public school outside Texas. Acceptable documentation includes a letter, signed and dated, from the parent/guardian or qualified student stating that the student has enrolled in a private school in Texas or a private or public school outside of Texas leading to the completion of a high school diploma.

87 Student withdrew from/left school to enroll in the Texas Tech University ISD High School Diploma Program or the University of Texas at Austin High School Diploma ProgramDefinition and use: Student was withdrawn from school and parent/guardian or qualified student indicated at time of withdrawal that the student has enrolled in the State Board of Education-authorized Texas Tech University ISD High School Diploma Program or the University of Texas at Austin High School Diploma Program.

Documentation requirement: The district must receive either a) a records or transcript request from the high school diploma program or b) a letter from the high school diploma program stating that the student is enrolled.

Withdrawn by school district

78 Student was expelled under the provisions of TEC §37.007 and cannot return to school

Definition and use: This code may only be used when: the student was expelled under the provisions of TEC §37.007, and the term of expulsion has not expired or the student’s failure to attend school is

due to court action.This code may only be used for a student who was expelled for an offense included in TEC §37.007. This code is not intended for use by districts which assign students to a Juvenile Justice Alternative Education Program (JJAEP).

Documentation requirement: Due process documentation supporting the expulsion.

83 Student was attending and was withdrawn from school by the district when the district discovered that the student was not entitled to enrollment in the district because a) the student was not a resident of the district, b) was not entitled under other provisions of TEC §25.001 or as a transfer student, or c) was not entitled to public school enrollment under TEC §38.001 or a corresponding rule of the Texas Department of State Health Services because the student was not immunized.Definition and use: This code is for situations in which the district discovers when verifying enrollment information that the student is not entitled to enrollment in the district because the student is not a resident of the district or is not entitled under other provisions of TEC

§25.001 or as a transfer student. It is not for a student who was a resident of the district and who stops attending because he/she has moved. This code is also for rare situations in which the student has not met the requirements under TEC §38.001 or a corresponding rule of the Texas Department of State Health Services for immunization, provisional enrollment, or exemption.

Subject to the exceptions in TEC §38.001(c), a student is required to be fully immunized against disease as required by the Texas Department of State Health Services (TEC §38.001(a)). A student may be provisionally admitted if the student has begun the required immunizations and continues to receive the necessary immunizations as rapidly as medically feasible (TEC §38.001(e)). Except as provided by TEC §38.001(c) or by rule of the Department of State Health Services, a student who is not fully immunized and has not begun the required immunizations may not attend school. For further information about enrollment procedures, please see the Student Attendance Accounting Handbook. For further information about immunization requirements, immunization exemptions, and immunization documentation, please contact the Texas Department of State Health Services.Documentation requirement: Due process documentation supporting the withdrawal. All district actions to withdraw a student must be documented or the documentation for use of this leaver reason code may be considered insufficient. For purposes of leaver reason code 83, due process is defined as completion of the following steps:

1. District provides oral or written notice, appropriately documented, to the student’s parent, guardian, or qualified student him- or herself of intent to withdraw the student, reasons for the withdrawal, effective date of withdrawal, and date of hearing or conference at which the parent, guardian, or qualified student will have an opportunity to respond to the allegations that the student is not entitled to be enrolled in the district. Steps 2 and 3 are not required for cases in which the parent, guardian or qualified student agrees that the student is not entitled to enrollment in the district. Step 4 is always required for charter districts.

2. District provides a hearing or conference at which the district presents the reasons for withdrawal, and the parent, guardian, or qualified student is given the opportunity to respond to the reasons for withdrawal.

3. District provides a written report to the parent, guardian, or qualified student that contains the findings of fact and district decision following the hearing or conference. The written report shall include notice of the parent’s, guardian’s, or qualified student’s right to appeal the district’s decision.

4. Charter districts shall notify the school district in which the student resides within three business days of withdrawing a student from a charter school, per Texas Administrative Code §100.1211.

Other reasons

03 Student died while enrolled in school or during the summer break after completing the prior school year

Definition and use: Self-explanatory.

Documentation requirement: Acceptable documentation includes a copy of the death certificate or obituary, a program from the funeral or memorial service, a written statement from the parent or guardian, and written documentation of an oral statement by a parent or guardian stating that the student has died.

16 Student returned to family’s home country

Definition and use: Use for students who are leaving the United States to return to their home country. A student may be leaving with or without family members to live with his or her family, immediate or extended, in the home country. The citizenship of the student is not

relevant in assigning this code. This code can also be used for foreign exchange students.Documentation requirement: Acceptable documentation is a copy of the Transfer Document for Binational Migrant Student completed at the time the student withdraws from school, signed and dated by an authorized campus or district administrator. Acceptable documentation is also a copy of the withdrawal form signed and dated by the parent/guardian or qualified student and a campus or district administrator. The withdrawal form should indicate that the student is leaving school because the student is returning to the home country and should specify the destination. An original signature is not required on withdrawal forms received in the district by fax. Withdrawal forms received by e-mail do not need to be signed by the parent/guardian or qualified student. A signed letter from the parent/guardian or qualified student stating that the student is leaving school because the student is returning to the home country is also acceptable documentation. Acceptable documentation for foreign exchange students includes a written, signed, and dated statement from the student's host family or the foreign student advisor verifying the student's return to his or her home country. Other acceptable documentation is written documentation of an oral statement by an adult neighbor or other adult with knowledge of the family's whereabouts, signed and dated by an authorized campus or district administrator.

88 Student was ordered by a court to attend a GED program and has not earned a GED certificate

Definition and use: This code is for students who are court-ordered to attend a GED program and have not earned a GED certificate at any time during the prior school year, including the summer (through August 31) following the close of the prior year.

Documentation requirement: Acceptable documentation is a copy of the court order stating that the student has been ordered to attend a high school equivalency or GED program. Documentation must include the name of the student, the date of the order, the name of the judge making the order, and the county in which the judge presides. The order should state that the court is ordering the student to attend a high school equivalency or GED program or to take a high school equivalency or GED exam.

89 Student is incarcerated in a state jail or federal penitentiary as an adult or as a person certified to stand trial as an adult

Definition and use: Student is incarcerated in a state jail or federal penitentiary as an adult or as a person certified to stand trial as an adult.

Documentation requirement: Acceptable documentation is one of the following: 1) Oral notification from a law enforcement agency, the office of the prosecuting attorney, or the jail or penitentiary, to an authorized representative of the district, that the student is incarcerated. The written statement of the oral notification shall be signed and dated by the authorized representative. 2) Written notification from a law enforcement agency, the office of the prosecuting attorney, or the jail or penitentiary, that the student is incarcerated.

98 Other (reason unknown or not listed above)

Definition and use: This code is used for students who are withdrawn by the school district after a period of time because they have quit attending school and their reason for leaving is not known. It is also used for students who withdrew from/left school for reasons not listed above.

First Day Procedures-Student Enrollment/No Show

Purpose: Have accurate student enrollment data on the first day of school. Have accurate lists of students present and students NOT present on first day of school.

Details: “No Showing” is a one day event. Texas Education Agency (TEA) allows Campuses /districts to “no show” students until 12:00 midnight on the first day of school. First day enrollment data will be used by campuses to determine next steps in re-engaging students who are not enrolled day one. This data will ultimately be used to determine the campus and district annual dropout rate.

First Day Student Enrollment data affects Federal and State Accountability Systems in two major categories and for different student groups such as ELL and Special Education students.

**First Day Procedures Plan must be submitted to the office of Dropout Prevention the Thursday prior to the first day of school.

1. Develop procedures for tracking and accounting of Leaver Reason Codes and Leaver Records.

Process and Procedures:1. Ensure that leaver tracking form is used consistently to document any follow ups

within 10 days of withdrawal. Ensure the withdrawal reason is specific. (For Example: Moving to Granbury, Texas) For each contact made, ensure that a leaver tracking form is used. Do not add on to an existing form. **

2. Ensure that required signatures and dates are on all documents.3. Ensure that DVM Leaver Checklist is attached and appropriate code checked off to all

withdrawal folders. **4. Ensure that updated forms are being used and old forms are discarded**5. Do not use white-out to correct mistakes. Draw one line across the mistake and initial

and date. **6. Ensure that Administrator or designee conducts Exit Interview and assigns correct

and that the label reflects the correct Leaver Code assigned. When changing a Leaver Code on a label, it must be initialed and dated.

7. Use Code Change form to indicate new code assigned.8. Leaver Code 16 requires city, state and country on Exit Interviews.9. Leaver Code 80 requires two modalities to support the code. Refer to DVM.

10. Dates on computerized withdrawal forms and signatures must match. If dates do not match, use a discrepancy form to explain.

SPECIAL PROCEDURES - ENROLLMENT DROPOUT RECOVERY STUDENTS

** This form MUST be kept by the Attendance Clerk

STEP #1 When a student with a code (98) comes to enroll, the attendance clerk MUST use this form to proceed with enrollment. – USE A GREEN FOLDER.

STEP #2 This student MUST proceed to the counselor so that the enrollment procedure is facilitated.

STEP #3 The counselor issues a schedule and returns the GREEN FOLDER to the attendance clerk. The attendance clerk delivers the enrollment form and folder to the ATTENDANCE ADMINISTRATOR who will make sure the 2-day verification of enrollment, as outlined on form, is completed.

STEP #4 Attendance Administrator returns enrollment form and green folder is returned to

attendance clerk who will make sure form is properly filed.

STEP #5 GREEN FOLDER must be recycled. It will need to be used with the next student enrolling (who has a code 98). NOTE: If attendance clerk receives 10 neon green folders, then at the end of each day clerk must make sure she/he has accounted for each one of those folders. Extreme care must be taken so that the GREEN FOLDER is only used when a code 98 student is enrolling. This will help to maintain the commitment that the campus has made to facilitate the enrollment procedures for these students.

Step 1: If a student is leaving the home campus to attend an alternative campus (after first meeting eligibility criteria), then the home campus will make contact with the alternative campus notifying them of the new student. Counselor from the comprehensive high school will prepare an ISPL (Individual Student Learning Plan) for the alternative campus.

Step 2: Student will acquire Calendar Change form from attendance clerk and counselor will complete Calendar Change form for student.

Step 3: Receiving school will verify ISLP folder and complete Calendar Change form to enroll student at the campus.

Step 4: Administrator will forward Calendar Change form to the Attendance Clerk to input into Skyward.

Returning to Home Campus

Step 5: Administrator/Counselor will complete the Calendar Change form to initiate process to return to home campus.

Step 6: Upon arriving at home campus, student will report to the attendance office for admittance and for administrator review.

Step 7: Attendance clerk will input calendar change into Skyward.
