ACKNOWLEDGEMENTS

Thanks to John Paul Gonzales, and staff of the New York City Department of Health and Mental Hygiene for the printing of the Queens Directory of Adult Day Services. Thanks also to the Queens Council on Developmental Disabilities, Bernard Fineson DDRO, Voluntary Agencies, the Department of Education and families for their participation in this project. This is the sixth edition of the Queens Directory of Adult Day Services. I would like to thank my committee members who spent numerous hours working on developing the directory. They are: Beverly Berger, parent, QCDD, Pamela Gaynor, Department of Education; Lia Nissel, Queens Centers for Progress/Bellerose; and Wendy Schoenfeld, Lifespire. It is hoped that this Directory will be an effective tool for families and professionals to access appropriate programs in an efficient manner. We encourage agencies to contact us regarding updated information. We welcome hearing about new programs. Please email [email protected]. QUEENS DIRECTORY OF ADULT DAY SERVICES

INTRODUCTION This directory is a comprehensive listing of Adult Day programs available in Queens for adults with developmental disabilities. It is meant to be a resource for families and professionals in search of services for young adults who are in transition due to aging out of special education programs. It is also a resource for adults with developmental disabilities who are in need of a day or vocational program. A developmental disability is defined as a mental or physical impairment, manifested before age 22, that continues indefinitely and results in a substantial limitation in specific areas of functioning. It includes intellectual disabilities, epilepsy, cerebral palsy, autism and neurological impairment. The programs are funded by the New York State Office For People with Developmental Disabilities (OPWDD), www.opwdd.ny.gov and are administered in Queens County by the Bernard Fineson Developmental Disabilities Regional Office (BFDDRO).

Queens Council on Developmental Disabilities

The Queens Council on Developmental Disabilities is an organization comprised of individuals, parents and representatives of most of the agencies providing services and programs in Queens. The council meets on the first Monday of each month. At these meetings, information is shared about programs and services, priorities are determined and legislative agendas are planned. This forum allows opportunities for networking.

Adult Day Services Committee

The Day Services Committee’s mission is to ensure that quality adult day program services are being provided in Queens. The Committee is made up of family members, provider agency representatives, and Department of Education transition coordinators. The Committee provides a system to ensure that individuals who are in need of day services are referred to appropriate provider agencies. The meetings also act as a forum for the group to share innovative ideas and discuss emerging issues. Members of the Committee can gain information and a greater understanding of all programs available to adults with developmental disabilities. Meetings are usually on the third Wednesday of every month and held at different locations within the Borough of Queens. See website: www.qcddny.org for information/locations of meetings.

TYPES OF ADULT DAY SERVICES Day Habilitation: This is a service that provides a range of supervised experiences in community settings designed to meet an individual’s unique choices and needs. Activities in this type of program may include purchasing goods and services, volunteering in the community, learning to travel in the community, and preparation for the possibility of eventually entering the job market in a supported work endeavor. Some agencies offer Day Habilitation Without Walls. Pre-Vocational Services: This is a service that prepares individuals for paid or unpaid employment. Services include teaching concepts such as attending to task, task completion, problem solving, and safety. Activities included in this category are not primarily directed at teaching specific job skills, but are directed at underlying goals, such as increased attention span and/or improving motor skills. Supported Employment: These are services designed to enable adults with intellectual and developmental disabilities to secure and maintain gainful employment in their community. Services may include, but are not limited to: vocational assessment to accurately determine the proper vocational course of action, on-the-job placement working with job coaching, and vocational counseling supports. Transitional employment programs, supported employment programs, and workstations in the industry are examples of programs. Supported Employment is for individuals who because of their disability cannot gain competitive employment on their own or cannot maintain the job independently. Some agencies offer: ETP – Employment Training Program and Pathways to Employment Services. Work Readiness: This is a services that addresses issues of occupational exploration, job hunting techniques, interviewing and the behaviors necessary to hold a job. A significant component is devoted to individual participation in community-based work internships. Staff works directly with each individual at a work site to further assess their work skills, preferences, and support needs. Considerable time is spent on training individuals to travel independently to work locations. The final phase of the program involves individualized job placement with intensive job coaching services. The coaching helps each participant learn work tasks and appropriate work behaviors. Coaching continues as needed to ensure job retention.
