Supp lers Enlistment Form can be downloaded frorn the website of the Bank (www.sbacbank com).

An amount of Tk. 5001 (Taka Five Hundred) only in the form of Payment Order (Non-Refundable) must be submitted

for each item under every Category as mentioned above. Category & serial of item must be mentioned on ihe top of

the envelope.

The En lstment Form along with Payment Order must be kepi in a sealed envelope and will have lo be dropped ai the

Tender Box kepi at Head Office, Sunmoon Star Tower (Level-10), 37 Dllkusha C/A, Dhaka-1000 on April 29, 2015 from

10:00 am io 4:00 pm and will be opened at 5:00 pm on the same day and place in presence of bldders who rnay

choose to attend.
