NHS Orkney is looking for a flexible enthusiastic Registered Midwife to join our Integrated Midwifery team which was recently awarded the Midwife Awardrsquo at the Scottish Health Awards 2021
The successful candidate will have a high level of autonomy across the whole range of Maternity care This includes the Community settings the midwife led unit which provides antenatal intrapartum postnatal and triageday care Our model also includes support from local Obstetricians and access to tertiary Obstetric care predominantly in Aberdeen
The postholder will hold their own caseload working alongside women their families and the multidisciplinary team in a continuity of carer model in line with the Best Start ambitions There is a requirement to work flexibly covering day and night rotation over a seven-day week including on call and air ambulance transfer duties

1 JOB PURPOSE
As an Integrated Midwife the post holder will
bull Provide full range of Preconception Antenatal Intra-partum Postnatal and Neonatal care to women babies and families in both hospital and community settings based on individual needs
bull Provide early pregnancy triage and day care services
bull Provide care of the deteriorating patient maternal and neonatal including neonatal resuscitation and stabilisation for transfer
bull Provide a range of Public Health advice to women and their families as part of the multi-professionalmultidisciplinary team
Acting as an autonomous practitioner within the relevant scope of practice the postholder will
bull Facilitate care in accordance with the philosophy and policies of the NMC NHS Orkney and Maternity Unit guidelines
bull Act as a mentor for less experienced midwives students and support staff
bull Participate in full shift rotation including On Call cover and road and air ambulance transfers
In the absence of Senior Charge Midwife
bull Provide leadership and supervision of junior staff amp students

2 DIMENSIONS
The post holder will
bull Ensure appropriate use of resources by ensuring high quality cost effective care
bull Work to support the professional development of the Maternity team which encompasses both registered and support staff
bull Maintain an environment in which effective learning can take place
bull Co-ordinates and advises on midwifery care and involvement of the multidisciplinary team
bull Participate in the audit process
3 ORGANISATION CHART
4 ROLE OF THE DEPARTMENT
All Maternity services are based within the Maternity Unit and provide Preconception Antenatal Intrapartum Postnatal and Neonatal care to women babies and families in both hospital and community-based settings
Sonography Day Care amp Early Pregnancy Assessment services are also provided
Approximately 200 women are booked per year receive antenatal and postnatal care locally and approximately 75 of women remaining in Orkney for intrapartum care
The Unit is UNICEF Baby Friendly accredited and Breast-feeding rates are amongst the highest in Scotland

The model of care is Midwifery led with Obstetric Support for women who have additional risk or if complications arise This model enables additional services based on robust pathways and risk assessment on an individual basis to be provided locally This includes where appropriate Augmentation Induction of Labour and Vaginal Birth
Women requiring Obstetric led care in a tertiary facility are transferred to a Consultant led Unit off Island

6 DECISIONS amp JUDGEMENTS
The post holder will be constantly required to make judgements involving a range of facts and evolving situations which require the analysis of data from a variety of sources and comparison of a range of options and to review the judgement of others The post holder will be clinically and professionally expected to make autonomous decisions on a daily basis advising on a range of issues to patients and maternity staff and supporting the multidisciplinary team in decision making
bull Responsible for counselling women with abnormal results
bull Function as an autonomous practitioner working within clinical and professional guidelines
bull Make autonomous decisions in planning and prioritising patient care and delegating to team members
bull Recognise own limitations in the provision of clinical care referring to other professionals

The post holder will be responsible to the senior charge midwife for clinical guidance and professional management work review and formal appraisal of performance The post holder is required to

bull Participate in the day to day management of the clinical area and deputise for the Senior Charge Midwife in her absence

Care of women in pregnancy labour and postnatally where complications arise

Managing the demand of direct patient care within existing resources
On callcall outs including air ambulance transfers off island with no immediate return
Coordination of care with a wide range of specialist staff utilising their skills effectively
Coping with highly charged emotional situations when breaking bad news to parents
Informing women of abnormal screening results
Individualised counselling of women with high risk screening test results

Management obstetric emergencies especially out of hours when assistance is on an ON Call basis
Management of neonatal emergencies and ongoing stabilisation while awaiting retrieval

The postholder is required to develop the knowledge and skills to competently and
safely use and if appropriate interpret the data from a number of items of

Lying off the northern coast of Scotland between John OrsquoGroats and the Shetland Isles Orkney is

approximately 22000 with most people living on the Orkney Mainland Kirkwall the capital with its
spectacular red sandstone 12th-century cathedral and a population of 7500 is the administrative

All Maternity services are based within the Maternity Unit and provide Preconception Antenatal Intrapartum Postnatal and Neonatal care to women babies and families in both hospital and community-based settings
Sonography Day Care amp Early Pregnancy Assessment services are also provided
Approximately 200 women are booked per year receive antenatal and postnatal care locally and approximately 75 of women remaining in Orkney for intrapartum care
The Unit is UNICEF Baby Friendly accredited and Breast-feeding rates are amongst the highest in Scotland
