Registration Forms Instructions You must use Adobe Acrobat Reader to type out the Registration Form. Download it to your computer at http://get.adobe.com/reader it is free. You cannot use the default reader (e.g., Preview) on your computer if it is not Adobe Acrobat Reader. It does not work.
You must save the Registration Form to your computer first. Then open the saved copy and type in your information and chosen activities. Prices will auto insert per the quantity selected.
There are 2 requirements to attend the convention. You must be a MAFCA member and everyone listed on the registration form must sign and date the MAFCA Waiver, Indemnification Release, and adults also sign page 1. You can join MAFCA by clicking the box next to the type of membership you select. The 2018 MAFCA National Convention Registration Form is available here as a Portable Document File (PDF). Download and save the PDF to your computer. You must open the form using Adobe Reader https://get.adobe.com/reader and type it out on your computer. Simply click on the Yes & No boxes on page 1 and a check mark will appear. The Hubley Derby, Swap Meet and Fashion Boutique have supplementary pages to fill out and information will automatically be entered in the appropriate spaces on pages 2 – 4. A sales tax deposit for the State of Nevada is required for the Swap Meet and Era Fashion Boutique. Your selections of Convention Merchandise on pages 8 and 9 will automatically total and be entered on page 4. If you opt for one of the Fallon Naval Air Station Bus Tours, fill out page 11 with the personal information required by the Navy.
1. When you have filled out the Registration Form and attachments, please email a copy to the Registrar at [email protected] with MAFCA Registration in the subject line so we’ll have an electronic copy.
2. Then print all the appropriate pages and sign page 1 of the Registration Form and all those attending must sign the MAFCA Waiver, Indemnification Release Agreement on page 5.
3. Mail by USPS the completed and signed Registration Form and attachments, the signed MAFCA Waiver, and a check for the Grand Total on page 4 to

4. If you don’t use a computer, you may fill out a printed version of the form by hand. Please print clearly, sign the Registration Form and the MAFCA Waiver and mail the Registration Form and attachments to Registrar at the address above.
5. Please don't hand-fill the form unless you absolutely must. All hand-filled forms must be retyped by the Registrar once they are received. You can help speed the process by typing in the information needed on the Registration Form. Perhaps a friend can do it for you.
6. For emergency purposes you must supply contact information for an emergency contact person. That person must not be someone attending the convention. That information should be entered at the bottom of pages 1 and 5.

MODEL A FORD CLUB OF AMERICA WAIVER, INDEMNIFICATION RELEASE AGREEMENT
Waiver and Release Property of the Model A Ford Club of America Rev 05/08/13
IN CONSIDERATION OF BEING PERMITTED TO VOLUNTARILY PARTICIPATE (AT MY REQUEST) IN THE EVENT OF MODEL A FORD CLUB OF AMERICA (“MAFCA”), ITS CHAPTERS, REGIONS AND/OR SPECIAL INTEREST GROUPS REFERRED TO BELOW (“EVENT”), THE UNDERSIGNED AGREE(S) TO THE FOLLOWING:
1. THE UNDERSIGNED HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE MAFCA, ITS CHAPTERS, REGIONS OR SPECIAL INTEREST GROUPS, THEIR EMPLOYEES, OFFICERS AND AGENTS (“Releasees”) from all liability to the undersigned, his or her or their personal representatives, assigns, heirs, family members and next of kin (“Releasors”) for any loss, damage, or claim therefor on account of injury to the person or property of the Releasors, whether caused by any negligent act or omission or other fault of the Releasees or otherwise while the undersigned is participating in the Event or using any of Releasees’ facilities in connection with the Event.
2. THE RELEASORS HEREBY AGREE TO INDEMNIFY AND HOLD HARMLESS the Releasees from all liability, claims, demands, causes of action, charges, expenses, and attorney fees and all other costs resulting from involvement in the Event whether caused by any negligent act or omission or other fault of the Releasees or otherwise.
3. THE RELEASORS HEREBY ASSUME FULL RESPONSIBILITY FOR AND RISK OF PERSONAL OR BODILY INJURY, DEATH OR PROPERTY DAMAGE while upon Releasee property or participating in the Event or using any Releasee facilities and/or equipment whether caused by any negligent act or omission or other fault of Releasees or otherwise. The Releasors expressly agree that the foregoing release and waiver, indemnity agreement and assumption of risk are intended to be and shall be interpreted and applied as broad and inclusive as permitted by applicable law.
I ACKNOWLEDGE THAT I HAVE READ THE FOREGOING, THAT I AM VOLUNTARILY ENTERING INTO THIS AGREEMENT AND THAT I AM AWARE OF THE LEGAL CONSEQUENCES OF THIS AGREEMENT, including that it prevents me, my personal representatives, assigns, heirs, family members and next of kin from suing the Releasees if I am injured or damaged for any reason as a result of participation in the Event.
IF THE PARTICIPANT IS A MINOR, HIS OR HER CUSTODIAL PARENT OR LEGAL GUARDIAN MUST READ AND EXECUTE THIS AGREEMENT WHERE INDICATED BELOW. I hereby warrant and represent that I am the legal guardian or custodial parent of the minor named below, who is a minor, and agree, on my own and said minor’s behalf to the terms and conditions of the foregoing agreement.
I understand that I may talk to my legal advisor about this agreement and I have either done so or chosen not to. I understand that I have the right and have been given the opportunity to object to and bargain about the provisions of this agreement. I am voluntarily signing this agreement and intend it to be the unconditional release of all liability to the greatest extent allowed by applicable law.
BEFORE SIGNING, I HAVE CAREFULLY READ THIS ENTIRE AGREEMENT: If any accident, injury, death or damage to personal property occurs, you (by signing this agreement) will be giving up certain legal rights. If any part of this agreement is held invalid, the rest of the provisions shall remain in effect. If you do not understand anything in this agreement, you should not sign it and you should instead consult your legal advisor. Swap Meet Nevada Sales Tax Deposit
Form A fashion and parts swap meet will be available during the 2018 MAFCA National Convention. The fashion swap meet will be held indoors at the Nugget Casino Resort on Monday and the parts swap will be held both inside the Nugget Casino Resort and outside in the Northwest area of the parking lot on Monday through Wednesday. The size of the spaces will be about 20’ x 20’ indoors and outdoors.
We have been notified by the State of Nevada that sales tax will be due on ALL sales by ALL vendors, both parts and fashions. The only exception is if the purchaser takes possession of the item outside of Nevada for items like a rebuilt engine or an overdrive. Otherwise all sales are considered taxable by the State. The State holds the Convention responsible for collecting the sales taxes.
The procedure required by the State is that we have to obtain the information below from each vendor 30 days before the event. This information is sent to the State and they return a form to us for each vendor which he/she will use to declare their gross sales and the sales tax due (8.265%). You should be aware that their vendor’s form asks for your Social Security Number (SSN) or Employer Identification Number (EIN). We will collect each vendor’s completed and signed form and sales tax due amount and return a summary report and payments to the State along with a copy of each vendor’s signed declaration form.
To insure all vendors follow the procedure we are requiring that a $50 sales tax deposit be paid by all vendors 30 days prior to the Convention. At the conclusion of the event or when the vendor elects to close up shop, he/she must return the Nevada sales tax form and pay any additional sales taxes above the $50 deposit. If their sales taxes are less that the $50 deposit, we will refund the difference.

FALLON NAVAL AIR STATION TOUR - REQUIRED BY U.S. GOVERNMENT If you signed up for the bus tour to Fallon Naval Air Station, they will perform a background check on all visitors, including children, before allowing them on the base. The personal information they require from each visitor is listed below and unless we receive the information from you with the registration form, you will not be allowed on this tour. The Naval Air Station has also informed us that they reserve the right to cancel one or all of the tours during a national emergency or any other critical situation that they deem necessary without any penalty or recourse by us.

PRE-REGISTRATION IS REQUIRED No late registrations will be accepted at the check-in table.
Standard Cars: Stock car will weigh max of 21oz(1.5lb) with original wheels,
frame, axle screws, DRY lubrication only.
Modified cars must not intrude on other lanes and fit in the start gate.

Modified entrants will use the same frame and be the same weight 21oz(1.5lb) as the standard hubley car. Tires, wheels and axels DO Not need to be original. NO LIQUID Lubricants. Car must run by gravity.
