A: There are 14 pairs of exit type doors used for this project. 12 pairs are to be demo'd, these can be used in the new construction, 3 new pairs will be needed. The door hardware should be the same or equal to the hardware in use on site. Hardware subcontractor should verify the hardware set used. There are 3 single exit doors and 5 on the demo so there is no need for new doors. 2 doors are not required to be exits. The Owner has classroom doors that can be used for these. 4) Sheet A700 does not offer detail on the base or head of the interior partitions, rated walls, non-rated walls, acoustical walls, chase walls and 2 hour walls. Fire Caulking is shown at the top of Type 2A walls and we are unclear if the intention is for rating or acoustical purposes. Sheet A101.1 shows openings that would compromise any rating. A: The drawings are incorrect the sealant should be acoustical only on non rated walls. After further review I believe there will not be a required two hour wall so A101.1 will be a 2A not 4A. 5) Wall type legend on A700 shows fire caulking at top of Type 2A walls (as if the walls are fire rated). We believe these are non-rated walls and the caulking is for acoustical purposes only. Please clarify. NOTE: On page A101.1 the walls are shown with openings that will compromise the rating if the intent is for these walls to be fire rated. A: A101.1 will be corrected with first review by city.

6) The carpet tiles and VCT is OFOI – will the Vinyl Base be by the Owner also?

A: Vinyl Base is OFOI.

7) The finish schedule states the ACT-1 is Owner Furnished – who is doing the install of

this material?

A: Contractor to provide and install grid and ACT provided by owner.

8) Will the metal stud framing as shown on structural drawings S607 need to have

engineered shop drawings?

A: Shop drawings for the stud framing on S607 are not needed. The stud framing details provided on S606 & S607, with combination of the notes and plans in the structural drawings should be sufficient.

9) Will the Owner be submitting the plans for the building permit as well as paying all associated fees?

A: Yes, the owner will be paying for the permit and associated fees.

10) Please clarify Unit Price 4 in the bid form - It states Miscellaneous and Structural Steel $ per unit. This cannot be priced as stated as there are many types of structural steel as well as misc. items and they are all different prices? Not clear what you are looking for.

A: Sort out the Miscellaneous and Structural Steel. Giive individual pricing on the type or classification of the material, plus the cost of erection. (i.e. Description, quantity, price per ea./TN/SFS/LF etc.) Use additional sheets to list or GC can provide Matrix on bid documents of these items.

11) The Unit Price form in the bid form starts with Unit Price 3. Are there Unit prices 1 & 2? A: Referring to “Unit Prices” in spec book, Address Letters A-D (3-5). Additionally, address Unit pricing through the RFP to bidders. This is a more complete Bid Form Document is required to be filled out completely.

12) On sheet D121 Demo note 4 calls out that all exposed fireproofing be treated with

Silicate Hardener – Please specify which product would be required for this work? A: The intent of this matter, is that the owner does not want the existing fireproofing flaking off after construction is complete. If the existing fireproofing is already dense enough to not and the new paint added to the exposed construction, then this note is not required.

13) All demo items listed as remove and salvage to owner – will all this material be taken

to a storage area on the church property or will we be required to transport it offsite to a remote storage location? A: Note on D121 is incorrect. Do not salvage existing ACT nor grid. Seating storage provided by owner under owner’s roof, seating to be removed and stored at owner’s direction under owner’s roof.

14) Are there any particular doors that we are to use for access to the work area?

A: Please plan to use the Door Entry 3 for access for workers. There may be times that other doors can be made available by coordinating with owner. Please address this issue if required in the Bidding GC presentation.

15) RFP Page 2 Item 12….What are the CCC Lender Requirements and the NRS Project disbursement company rules and regulations? A: Lender requires a standardized Pay Application from the GC, to be broken out by standardized 16 CSI Divisions. Selected GC for project will go over this detail with Central Christian Church Construction Team once selected. Project disbursement company (TBD), typical rules & regulations apply for working with a Nevada Company.

16) Is there an asbestos survey already completed on this project?

A: Please see attached Asbestos Report from CCC Phase 1 Construction.

17) Which party will be responsible for the furnish and installations of the following items? A: A701 is correct, the medium density fireproofing is acceptable.

20) Building section D1 on sheet A200 illustrates that the existing roof is less than 20’ AFF. Is the existing structural frame & roof assemble already fireproofed? If not, please clarify what grid lines you wanted fireproofed on sheet S104

A: Fireproofing will need to be added to bays between C & E.

21) Bidders have identified the existing glazing systems are Interplane high performance glass, they are no longer in business. A close match may be available from Viracon. Are the building record glazing specifications available for the bidders to aid in figuring their glazing cost.

LIMITATIONS The conclusions and results contained herein are based solely on the information presented in this report and are for the sole use of the Client exclusively and are subject to the terms and conditions of “The Contract”. The sampling, testing, and observations described in the report represent conditions only at the specified times and locations. Additional materials or information which were hidden, undiscovered, inaccessible, or are not a part of the finding presented herein, would result in the modification of the conclusions and recommendations made herein. Any response action guidelines are based solely on the findings contained herein. The response action guidelines are presented as a courtesy and are not to be considered a complete or detailed set of specifications. Dominion performed these services using the degree of care and skill ordinarily exercised under similar conditions by reputable members of the profession practicing in the same or similar circumstances. Please note, with regard to laboratory analysis of suspect bulk asbestos-containing materials: The Environmental Protection Agency (EPA) Method 600/R-93-116, Polarized Light Microscopy (PLM) is used for the testing of bulk building materials for asbestos by performing a visual estimation. This method is the most widely used method for estimating asbestos in bulk building materials and works well for most sample types. However, it might require a more detailed method such as point counting for accurate estimation of asbestos in samples with low asbestos concentration. This method is also not applicable for samples containing large amounts of fine fibers below the resolution of the PLM (<0.3 microns). Typically, the asbestos fibers present in non-friable organically bound (NOB) materials are often less than 0.1 microns in diameter and therefore may cause the PLM method to yield low estimates or even false negative results. Dominion suggests that Transmission Electron Microscopy (TEM) Chatfield Semi-Quantitative EPA Method 600/R-93-116 be used to confirm all negative results of NOB materials. This method is particularly useful to reduce the possibility of false negatives in NOBs and to provide additional quantitative data for samples which contain low levels of asbestos. The TEM method is available at additional cost and is done only at the client’s request. Dominion’s scope of service for this report did not include the inspection for, or identification of fungi, lead paint, or any other hazardous or controlled substances not specifically identified herein. Dominion is not responsible for the accuracy of information provided by others, or for conditions or consequences arising from relevant facts that were withheld, concealed, undiscovered, or not fully disclosed. Dominion is not a law firm and therefore, makes no representations regarding any potential liability of any person or entity for site conditions. Further, Dominion is not qualified to present medical advice. If any present or future health issues are in question, it is recommended that the findings in this report be presented to a qualified medical professional for evaluation.

SECTION 2

EXECUTIVE SUMMARY

EXECUTIVE SUMMARY INTRODUCTION This report presents the results of a limited AHERA-style asbestos inspection conducted for Wallin Construction, LLC. The limited asbestos inspection was conducted at the Central Christian Church Auditorium, located at 1001 New Beginnings Drive, in Henderson, Nevada. Dominion was authorized by Mr. Justin Wallin, with Wallin Construction LLC, to perform the limited asbestos inspection via verbal authorization on October 1, 2014. BACKGROUND INFORMATION Dominion was informed that during renovations at the church, Clark County Air Quality had arrived on site and requested that asbestos sampling be performed. ASBESTOS SAMPLING The limited asbestos inspection of the auditorium was conducted on October 1, 2014, by Mr. Christopher Webster, an EPA-accredited AHERA asbestos building inspector and Nevada OSHA-licensed asbestos inspector. Mr. Wallin outlined the areas and the materials being disturbed during the renovation activities, instructed Dominion to collect bulk samples of the building materials, and instructed Dominion to have those samples analyzed for the presence of asbestos fibers by a qualified laboratory. A total of 42 bulk samples were collected from the suspect building materials within the outlined areas of the auditorium. The suspect bulk samples were submitted to EMLab P&K, LLC (EMLab) for analysis using Polarized Light Microscopy (PLM) methods. EMLab used the methods prescribed in Method 40 CFR, Part 763, in the Code of Federal Register in analyzing bulk samples. EMLab is certified by the National Institute of Standards and Technology (NIST) as adhering to and in compliance with the National Voluntary Laboratory Accreditation Program (NVLAP) and holds the certification number 500031-0. The suspect asbestos bulk samples were collected and submitted to the laboratory using established chain of custody procedures. Bulk samples were collected utilizing safety and health practices as required by the Occupational Safety and Health Administration (OSHA). Following are the suspect building materials that were sampled during the limited asbestos inspection:

The building materials sampled were reported by laboratory PLM analysis to be “none detected” for asbestos. RECOMMENDATIONS Based on the previously referenced laboratory analytical data, there are no asbestos recommendations at this time. If any un-sampled suspect building materials are discovered during the renovation project, those building materials should be properly sampled by a certified inspector prior to their disturbance. All documentation is provided to Wallin Construction, LLC., and should be retained in a permanent record. GENERAL NOTES/EXCLUSIONS • Mr. Justin Wallin, with Wallin Construction LLC, outlined the areas and the materials currently disturbed

or scheduled to be disturbed during the renovation activities, instructed Dominion to only collect bulk samples of the outlined materials, and to have those samples analyzed for the presence of asbestos fibers by a qualified laboratory.

• This was a limited inspection and those areas not specifically identified in this report were not inspected;

nor any area outside the specific area described herein. • If any un-sampled suspect building materials are discovered during the renovation activities, those

materials should be properly sampled by a certified inspector and analyzed by a qualified laboratory prior to their disturbance.
