– Mileage 25¢ per mile• Please put your complete address on the form• Please put your council/committee name on the line for committee• Only one car per local council or state committee
– Lodging• One room per council that is 100 miles or more from meeting site
• Reimbursement forms• Please identify on the form or on receipt which budget (Class #) is to be
charged. If someone other than the chairperson is sending in a reimbursement, the chairperson needs to send Rita an e-mail to OK the reimbursement.
• If a statement is to be sent from a company directly to treasurer please send me an e-mail including:
– Company’s name– Item(s) purchased– Budget (Class #) to be charged, If you do not know your budget class
please see me– Bill will not be paid without the above information
