Safety - a small investment for a rich future!
1. Heat stress - Summer Safety
Working in hot environment puts stress on body’s cooling system. When heat is combined with other stresses such as hard physical work, loss of fluids, fatigue or some pre-existing medical conditions, it may lead to heat-related illness.
This can happen to anybody (even young & fit) especially true early in the summer when people are not used to the heat. The heat source at workplace can be either from sun or equipment or processes. Humidity at workplaces also contributes to heat stress.
Your body is always generating heat and passing it into the environment. The harder your body works, the more heat it has to lose. When environment is hot / humid / has a source of heat (furnace or sun), your body must work harder to get rid of heat.
The summary of heat-stress related disorders, causes, symptoms, treatment and prevention are as below:

Controlling heat stress:
Engineering controls:
Reducing physical demands of work task through mechanical assistance
Controlling the heat at its source through the use of insulating and reflective barriers
Exhaust of hot air and steam produced by operations
Reducing the temperature and humidity through air cooling
Providing cool, shaded work areas
Increase air movement if temperature is high but below 350C dry bulb temperature. If temperature is further high, administrative controls can be employed
Administrative controls:
Increasing the frequency and length of rest breaks
Scheduling strenuous jobs to cooler times of the day
Frequently drinking water
Cautioning workers to avoid direct sunlight
Assign additional workers or slow down the pace of work
Training the workers on symptoms of heat stress and start a “buddy system”
Protective clothing:
Light summer clothing (light colored, long sleeve shirt & pants) to allow free air movement and sweat evaporation
General measures:
Acclimatization: Initially during hot days, slowly increase the exposure time rather than sudden increase. The longer you work in a hot environment, the better your body gets acclimatizes to the heat.
Eat well-balanced, light and regular meals which includes fruits and healthy vegetables
While driving, use helmet and cooling goggles to prevent dust & excess light falling on eyes
2. Lessons Learned
Incident Date & Time : 14th March 2014 & 16:30 Hours
Location : Unit # 50 Argon Room, CGPL – Mundra
Incident : Fatality
Description of Incident:
Argon cylinder fell on the worker and high pressure argon gas was released. As a result he sustained serious injury. Later on the way to hospital he expired.

Root Cause Analysis:
Human Factors:
• Violation by individual.
• Operation of equipment without authority. (Spare Argon cylinder)
• Lack of knowledge of hazards present. (Handling of Pressurized argon gas cylinder)
• Equipment or materials not secured. (Locked with Nylon rope instead of chain)
Physical Factors:
• Energized systems, other than electrical. (Pressurized Argon Cylinder)
• Mechanical hazards (Handling of Pressurized Argon Cylinder)
System Factors:
• Inadequate communication between work groups (Between maintenance & operation)
• Incorrect instructions (Hazard were not communicated)
• Improper storage of spare cylinders. (Spare cylinders kept in working area for PG Test)
• Inadequate assessment of required skills (Wrong person on the job)
• Lack of job safety analysis & PTW. (PTW & JSA not available)
Recommendations:
1. PTW system should be religiously followed.
2. PTW should be issued for inspection of critical equipments and installations. This type of
critical installation to be identified.
3. Guidelines should be made for authorized entry in restricted area. List of authorized person
should be made & should be approved by Head-O & M.
4. All spare pressurized cylinders should be tightened with chain and caped.
5. Training for handling Argon cylinder should be conducted.
6. Safety aspects such as access control, maintenance facility and Handling of Argon cylinders
(Trolley) to be provided inside Argon room.
7. Apply and obtain license for gas cylinders storage as per “Gas Cylinder Rules 2004.

3. Safety Training - Mar’14
‘Supervision’, ‘PPE Inspection’, ‘Housekeeping’ ‘First Aid’ & ‘Safety in yard activity’ trainings
were conducted at Visapur, Poolavadi, Agaswadi, Sadawagapur & Gadag Wind Farms
respectively
Safety standards and procedures, first aid and fire fighting, DMP awareness, Electrical safety,
defensive driving, IMS awareness, E-PTW trainings were conducted at Trombay
4. Emergency Exercises-Mar’14
Emergency exercise on ‘Road Accident’, ‘Natural calamity’, ‘Fire’ & ‘Lightening’(table top) were
conducted at Khandke, Nivade, Sadawagapur & Supa wind farms respectively
5. Corporate Safety Highlights – Mar’14
Corporate OHSF participated in interview panel to certify O&M Engineers prior to taking
independent charge.
Conducted first training program on Tata Group safety standards (Contractor Safety and
Working at Heights) at Mumbai which was arranged by TQMS.
Corporate OHSF participated as part of the team for investigating fatal accident of CGPL,
Mundra.
Prepared office & fire safety posters and coordinated with Corp. Communication for
displaying the same on Sangam. Hard copy of Safety posters were also circulated to all
divisions.
DMP certification as per NDMA guidelines received from consultant for MO & Eastern region
except DCS & CGPL.

7. Security Highlights
Trombay
16 ,20 & 23 March.2014 – Training was conducted for 14 Coal Jetty security staff as a part of
ISPS code compliance and Port Security Rules, duties, handling of gadgets etc by Dy PFSO
Vijay Desai.
30 March.2014 – OJT training imparted by Security Officers by Mr. P S Bani and Rajiv Kurup
on physical security at Main gate for Security/QRT staff.
24March.2014 – 21 Students of Stanford Graduate School of Business visited the plant at
1351 hrs and departed at 1525 hrs. Necessary security and traffic arrangements were made
during the visit.
25 March.2014 – A meeting of security service providers of
Mumbai operations area with Tata Power CFT Members from
Security/CC/Admin was held at 1030 hrs at Chinar Conf for
awareness on Performance Linked Product Based Contract
(PLPBC).
29 March.2014 – A monthly meeting of Trombay security officers
chaired by Col. P B Dalvi Head - Security was held in the conference hall from 0830 hrs to
1145 hrs at Security Complex.
30 March.2014 – Emergency Exercise based on Intrusion was
carried out at Station - B Plant Area as a part of OJT training of
security staff.
24March.2014 – 21 Students of Stanford Graduate School of

31 March.2014 – A Farewell function organized for Mr. B M Dubey Senior Head Guard
(173983) who superannuated from Tata Power Service after completing 20 year 09 months
at Security Trombay.
JOJOBERA
Operational achievements - Successfully Conducted a number of training programmes to
enhance the operational skills of the guards and also improve active involvement in Emergency
Exercise.
Training activities - A)Security awareness lecture/training were given to 597 contract
workmen by the security officers of Tata power to make the contractors workmen aware of the
Do's and Don’t's in respect to security, while working inside the Plant area. B) 64 security
personnel of SIS Security were imparted training on “firefighting”. “Mock drill on emergency
preparedness” “Deployment for event management”. And “Traffic management”

Social activities – A)Mr. Sunil Kumar, Head – Security was a part of team led by Mr. D. S. Kudalkar- Station Head who visited School of Hope meant for mentally challenged children on 6th March 14. B) Sunil Kumar also actively participated in organizing Homeopathic camp for villagers of the nearby villages on 28/03/14.

Mr. Sunil Kumar, Head Security was recognized for "Imparting weekly training to stake holder employees"
Mr. A K Srivastava, Lead Security was recognized for “Taking initiative and excellence performance during chairman visit”. He was also rewarded by safety department for “Weekly training for stake holder employees" and “Training on safety to security personnel". He was also rewarded by office waste management committee for ”Taking initiatives for plastic free environment".
01 ASO, 03 SI, 06 Supervisors and 18 guards of SIS were rewarded for their performance during March 14.

Security Meeting- Mr. Sunil Kumar, Head Security along with Heads of Security of other Tata Group companies met with SSP, Jamshedpur on 12/03/14.The deteriorating law and order situation in the area was discussed in the meeting. Mr. Sunil Kumar. Head –Security attended meeting with Dy SP (Traffic) and DTO Jamshedpur on 26th March 14. Head - Security met Circle Inspector, Telco on 1st and 7th March 14. Meetings at regular interval were attended with OC PS Govindpur by Head Security.
Security Related Functions/ Events Founders Day and Safety week was celebrated in befitting manner. Team security actively participated in both the events. Team Security actively participated in the mock drill on "Emergency Preparedness".

Achievements - Security Department received 33 rewards/ recognition during the month of March,
2014. Meetings between Head - security and Police/ Govt. officials took place at regular
interval. Team Security actively participated in the Mock Drill on "Emergency Preparedness" and
gave desired results.
Transmission & Distribution Training & Mock drill: The training workshop for security guards of T & D was conducted at Parel receiving station on 04 Mar and Techno Polis on 08 Mar 14 respectively. Total 62 security personnel’s attended the workshop. The following was covered during the training:- (a) Security process. (b) Job responsibility of security guards. (c) How to use of communication radio sets (practical) training. (d) Practical training on frisking/checking by HHMD (e) Documentation at security gate. (f) Knowledge sharing.
SECURITY MOCKDRILL AT MAHALAXMI DISTRIBUTION DIVISION 20 MARCH 2014
Security Mock drill was carried out with the help of NM Joshi Marg police station at Mahalaxmi on 20 Mar 2014 at 1230hrs. The aim of this exercise was to streamline the procedures and action to be initiated during emergency in our location. Police Inspector & security staff seen briefing the employees on security issues.
Encroachment REMOVAL. Mankhurd - Air-conditions units removed by AET to avoid electrical accidents under the 110 Kv Chembur – Khopoli Lines .The Air condition units mounted on permanent hut roof which reduced distance from Transmission lines.
Mankhurd – The encroacher was trying to occupy the space in front of Mankhurd open plot by putting temporary temple were removed by AET .
CGPL:
Mr. SN Rudrappa, Chief-Security & Vigilance visited CGPL and reviewed security on 23 -24 March.
On 27/3/14 A Security Mutual Aid Meeting of Mundra-Anjar region was conducted by HMEL at Gandhidham.
DIG Coast Guard (NW Region), Police Officials and senior security professionals of various
industries in the Mundra-Anjar region participated in the aforesaid meeting.
The agenda of the meeting was:
Present security scenario of Mundra-Anjar region.
Improvement in sharing of information/intelligence and resources.
Improvement in liaison with local Govt. authorities.
Strengthening mutual aid policy. Mr. Virendra Dogra and Mr. Nand Kishore Bhatt of CGPL Security actively participated in the
meeting.
Salient Points:
1. Importance of the local intelligence. 2. Service legal agreements of the Security Service providers. 3. Details of the different vendors engaged in different Industries. 4. Common Email Id for sharing of Communication. 5. Different Acts on which the law breakers can be booked in the court of Law. 6. Contract Labour antecedents verification. 7. Police verification of the labour engaged.

MPL –MAITHON:
Operational Achievement: On 13.03.2014 on specific information from intelligence sources Security personnel (ASO – Birbal Modi, SS- Sk. Sarfuddin, SS- Amit Kr. Singh & SG – Sk. Jainul) recovered scrap materials approx 3 quintals worth Rs 6400/- from nearby village Kamardih ,about 200 meters from the plant. As soon as the person carrying scrape saw security personnel he any how managed to flee leaving the material with tricycle.
There were reports that a scrape vender comes to the village to get the house hold items but he used to takes away the scrape also. Security personnel put and ambush and recovered the items.
Details of material:-
1. Iron Scrap- 320 kg 2. Plastic Scrap- 05
kg 3. PP Rope – 1 bundle
Tricycle(Thela ) vehicle – 1 no.
On 29/3/14 about 60kg iron materials recovered by ASO Narendra Singh, SS
Rajmangal, SG Rajendra & QRT 1 from 06 ladies of Purni village in BHEL yard Maj (Retd) Manish Bahuguna has joined as Head Security, MPL on 15 March, 2014.
