Registered Nurse Interview Scenario “What would you do to manage the following situations?” Please read this scenario and discuss what actions you would take in the following situations. You are working as a registered nurse in a 28 bedded frail elderly unit in a 60 bedded care home; you are on a twelve hour shift. Working with you is a team leader and three care assistants. From 8am to 2pm there is a second registered nurse on duty; you are in charge. 1. At 9am the emergency nurse call bell sounds as Mrs B has been found on the floor in the lounge. She has a large skin tear on her shin and is holding her head. No one witnessed her fall. Mrs B appears to seem confused and is unable to tell you what happened. Things to consider 1) Most important you are in charge so there is no one more senior to you to ask for advise, so you must take charge 2) What do you know about this resident? Is she allergic or respond badly to any medicines or treatments? 3) Does she need hospitalisation? If she is suffering from Concussion or needs stitches she may need an ambulance 4) If she doesn’t need hospitalisation, Get her comfortable, put her in bed- re assure her, calm her down, arrange for someone to make her a drink. 5) Is she in shock – She may need emergency resuscitation or hospitalisation? 6) Where are the Care assistants, do they have any information to help you to find the cause? 7) Don’t leave her alone, arrange for someone to sit with her for a couple of hours if she does not require hospital treatment. Things to consider

1) Most important you are in charge so there is no one more senior to you to ask for advise, so you must take charge

2) What do you know about this resident? Is she allergic or respond badly to any medicines or treatments?

3) Does she need hospitalisation? If she is suffering from Concussion or needs stitches she may need an ambulance

before? 10) Do we need to amend the care plan to take in consideration that she may not be safe to

be left alone.11) Review with Deputy for future prevention of problems. We need to make sure this

doesn’t happen again.12) Very important to make sure that this situation has not occurred due to abuse by a staff

member or neglect by the staff in charge

2. You have delegated the morning medication round to the other RN on duty as you need to contact the GP surgery to arrange a GP visit to the home. At 10am she comes to tell you that she has given the medication prescribed for Mrs G to Mrs F. This medication was Aspirin, Furosomide, Omeprazole and Prednisolone. She had already given Mrs F her medication earlier which was Bendrofluazide, Ibuprofen, Tamsulosin and Paracetamol.

3. While doing your rounds of the residents, you go and see Mrs Y, a frail lady who has nursed in bed all week with a lower respiratory tract infection. She remains poorly. Her nutrition and fluid intake has reduced. The carers are assisting Mrs Y with her personal care and you can see her pressure areas. You discover that she has a pressure ulcer that you knew nothing about. The lady is lying on a medium risk foam mattress. Nothing was discussed during handover and she has no specific supporting documentation or care plan for prevention of pressure ulcers or for the management and healing of the pressure ulcer.

Please write a care plan for Mrs Y for prevention of pressure ulcers using the enclosed form.
