Open Enrollment Process Steps …

Go to the Human Resources “Open Enrollment Resource”

page for information

Review summary documents, rates, coverage changes, and

new plans

Review any additional benefits (e.g. 401k, 403b, 457, Flex Spending (Healthcare and

Dependent Care), Supplemental Life, etc

Complete the forms to make changes OR make a new

enrollment in additional benefits but if opting out ..

Complete the forms for the benefit you are opting out of

(Medical or Dental)

[ you can not opt out of vision]

Complete the Flex Cash Form to request cash for premiums not taken along with proof of

“other insurance”

[ this cash is taxable income to you]

Leverage the Human Resource Benefit drop in times with

questions OR just return your completed forms to Human

Resources by October 11th Don’t forget to sign all

documents and supply

SSNs where applicable
