preferibilmente 24 hr prima della PCI in assenza controindicazioni; se non
possibile per os 250-500 mg250-500 mg e.v.
Dopo PCI ASA 325 mg deve essere proseguita almeno per 1-mese dopo BMS,
per 3-mesi dopo sirolimus-eluting stents e 6-mesi dopo paclitaxel-eluting stent
Successivamente ASA deve essere continuata indefinitivamente alla
dose di mantenimento di 75 - 162 mg75 - 162 mg
Clopidogrel
Dose di carico iniziale di 600 mg 600 mg deve essere somministrata a tutti i pazienti
prima della PCI.
Dopo PCI clopidogrel deve essere continuato alla dose di mantenimento di
75 mg 75 mg almeno per 1-mese dopo BMS e per 12-mesi dopo DES (drug-eluting
stents).

ACEIs - ARBs
ACE-Inibitore per os deve essere somministrato entro le prime 24 ore di STEMI a pti con infarto anteriore, congestione polmonare o FE 40% in assenza di ipotensione (PA sistolica 100 mm Hg o riduzione di 30 mm Hg rispetto al valore basale) o di controindicazioni note a questa classe di farmaci.

I C SARTANI (Valsartan, Candesartan) nei pazienti che sono intolleranti a ACE-inibitori e hanno sia segni clinici o radiologici di insufficienza cardiaca o FE 40%.
ACEI per os entro le prime 24 ore di STEMI può essere utile nei pti senza infarto anteriore, congestione polmonare o FE 40%, in assenza di ipotensione o di controindicazioni a questa classe di farmaci.
ACEI per via endovenosa non deve essere somministrato entro le prime 24 ore in pti con STEMI a causa del rischio di ipotensione (Una possibile eccezione possono essere i pti con ipertensione refrattaria)

BB per os deve essere iniziato nelle prime 24 ore in pti che non hanno controindicazioni.

BB endovena è indicato nei pazienti con tachicardia sinusale e ipertensione in assenza di controindicazioni, perché riducono la frequenza cardiaca e quindi le richieste miocardiche di ossigeno
Beta-bloccante endovena non deve essere somministrato a pti con IM che hanno: [1] segni di insufficienza cardiaca [2] bassa portata (PA < 90 mmHg) [3] alto rischio di shock cardiogeno (fattori di rischio per SC sono età > 70 anni, PAS < 120 mmHg, frequenza cardiaca > 110 bpm o < 60 bpm), [4] bradicardia < 50 bpm, BAV I° con PR > 0.24 sec, BAV II° e III° grado[5] asma bronchiale attiva
Riducono le dimensioni dell’infarto, riducono le aritmie letali, alleviano il dolore e riducono la mortalità compreso la morte cardiaca improvvisa.

Contrast-Induced Nephropathy (CIN)
Definizione
Nuova insorgenza o aggravamento di insufficienza renale caratterizzata da un aumento della creatinina sierica (Crs) ≥ 0.5 mg/dL o ≥ 25% della Crs basale, che si manifesta entro 48 - 72 ore dalla somministrazione di mezzo di contrasto iodato, raggiunge il picco a distanza di 5-7 giorni dall’esame angiografico e nella maggior parte dei casi (75%) la Crs ritorna ai valori basali dopo 7-15 giorni.

Indicazioni fornite alla dimissione
Controllo ematochimiciCreatinina dopo 3-4 giorni, Emocromo a 1 mese, Colesterolo LDL a 1 mese
Indicazioni per la prevenzione secondaria- Astensione assoluta dal fumo (anche l'esposizione al fumo passivo è dannosa).- Pressione arteriosa deve essere mantenuta sotto 140/90 mmHg. ( sotto 130/80 mmHg. se il paziente è diabetico o ha insufficienza renale cronica ).- Colesterolo LDL deve essere mantenuto sotto 100 mg/dl; se i Trigliceridi sono oltre 200 mg/dl il Colesterolo non HDL deve essere sotto 130 mg/dl;- Il paziente deve svolgere almeno 30 min. di attività fisica 7 giorni alla settimana ( minimo 5 giorni alla settimana );- Mantenere l'indice di massa corporea fra 18,9 e 24,9 e la circonferenza vita sotto 102 cm nell'uomo e 88 cm nella donna.
Educazione sanitariaIl paziente e, se lo desiderano anche i familiari, sono invitati a partecipare ad un incontro di educazione sanitaria c/o il nostro Ospedale.

Conclusions
COGENT is the first, randomized assessment of clopidogrel and PPIs on clinical events
The data provide strong reassurance that there is no clinically relevant adverse cardiovascular interaction
between clopidogrel and PPIs
The results call into question the exact relationship between ex vivo platelet assays and clinical outcomes,
especially with respect to assessing drug interactions
– Platelet assays and observational data are not a substitute for RCT data
Further research is needed to define the optimal strategy to reduce GI events in patients on antithrombotic
therapy, though prophylactic PPIs seem very promising
