Suggested Marking Technique

In my experience with implanting toric IOLs, I have found that most intraoperativeaxis markers are 13.5mm in diameter which is much larger than most corneal diameters. Thismakes the markers difficult to use on patients with hyperopic eyes, deep orbits, and prominentbrows predominantly found in Asians and similar ethnicities. In order to use the markers, onemust manually stretch the wire speculum apart, causing the patient discomfort and furthermore causing him to squeeze his lids together and worsen the exposure. Another problem is that often these large markers obscure the preoperative horizontal marks entirely,defeating the purpose of marking altogether. Or the marker is designed in such a way that oneneeds to turn the marker over in order to position the axis points, making the procedure rathercumbersome. With the small markers I have seen, they either require the use of both hands ordo not make lasting marks.

The *Modi Intra-Op Toric Marker (AE-2848) that I havedesigned with ASICO overcomes these difficulties, and I havehad great success with this marker on my patients. With thisnew marker, the outer diameter is only 12.5mm. This allows the outer preoperative marks to be easily seen forconsistent alignment and works well on smaller corneas.Unlike other smaller markers, this instrument requiresonly one hand to manipulate. The outer barrel,which contains the actual axis marking plates is

knurled so that it can be easily rotated with twofingers. In addition, the marking plates have been designed to create clear and long lastingmarks. For consistently good alignment marks, it is important to remember to center thecorneal light reflex in the instrument's central opening.

When inking the marking plates,mark perpindicularly to the plateto optimize ink transfer.
