Purpose

• LSF Health Systems (LSFHS), in collaboration with its network providers is committed to ensuring coordination with State Mental Health Treatment Facilities to assist the DCF in its goal of reducing admissions and length of stay for adults in state hospitals and return them to a community environment.

ME Role

LSF Health Systems as the NE Region Managing Entity (ME), in accordance with Section 394.4573, F.S. shall implement a continuity of care management system to provide mental health care for clients referred from the State Mental Health Treatment Facility (SMHTF) to the community.

ME Role and Responsibilities

Pursuant to LSF Health Systems’ contract with the DCF, the ME shall perform all administrative and oversight functions necessary to ensure the proper delivery of services including, but not limited to the following:

• Individuals who are currently in civil and forensic state Mental Health Treatment Facilities, committed pursuant to Chapter 394 or 916 F.S.

• Coordinating with civil state Mental Health Treatment Facilities and Network Service Providers to reintegrate into the community individuals identified as discharge ready.

Point to Remember:

The goal is the successful reintegration of individuals

discharging from a state hospital back into the community and to

reduce, divert and work to prevent re-hospitalization of this priority

population.

ME Role and Responsibilities

LSF Health Systems is responsible for contracting with local providers, who are in turn responsible for staffing (community case managers and/or forensic specialists) and providing services, as applicable, to cover the contracted counties/areas of service.

LSF Health Systems is responsible for providing support, oversight and is contractually obligated to monitor, evaluate the system, holding network providers accountable for ensuring that services are provided in accordance with all federal and state rules, regulations, and requirements.

• Clinical Care Support Specialist (CCSS) shall:• send out the monthly discharge planning meeting

schedule to all program directors on a quarterly basis,• enroll all discharges from NEFSH into LSFHS’ care

coordination program,• track provider participation in treatment team meetings, • attend all Continuity of Care and NEFSH board meetings,• assist in discharging consumers with significant barriers

to placement.
