  • Bernd Kutzscher, MD Pacific Eye Specialists Annual Educational Meeting October 6, 2016

Summary of Course Topic: Since its FDA approval in 2009, the femtosecond laser has taken on a growing role in cataract surgery, here in the U.S. and overseas. Much of the most high quality work with the Fs laser has been done in Europe. The question remains as to whether the Fs laser is a technology that truly adds quality and safety to cataract surgery. This remains a somewhat controversial topic.

In my talk, I will address the pros and cons of Fs laser cataract surgery. I will also review the basic technology of the Fs laser along with recent upgrades in the technology. The bulk of my talk will be oriented to steps in cataract surgery using the Fs laser that make it different than conventional cataract microsurgery. Finally, I will review the key features of Fs laser cataract surgery that should be known to any optometrist who is active in management of cataract surgery patients.

Outline of Key Topics: Questions to be addressed: 1. How does the femtosecond laser function and why is this beneficial in cataract

surgery? 2. How does the imaging system that guides the femtosecond laser make cataract

surgery more accurate? 3. What are the specific steps of cataract surgery for which the femtosecond laser can

be used? a. Centration b. Corneal incisions c. Limbal relaxing incisions which can be combined with toric IOLs. d. Anterior capsulotomy e. Lens fragmentation

    4. What is the impact of the femtosecond laser on lens implantation? 5. What are the specific areas in which the femtosecond laser benefits the cataract

surgery patient? a. Corneal edema b. Length of surgery c. Accuracy of lens implantation d. Amount of phaco power used e. Prevention of capsulotomy difficulties f. Reduction of astigmatism

    6. What can we expect next in laser-assisted cataract surgery?

September 15, 2016 To: State Board of Optometry attn.. Kristina Eklund From: Bernd Kutzscher, MD Pacific Eye Specialists

Dear Ms. Eklund, Thank you for your letter regarding our CE Course Approval Request. To give

some background, our practice has hosted this meeting annually for nearly ten years. It is always well attended, well liked and a great opportunity to collaborate with our colleagues in optometry. Our practice consists of two optometrists and three ophthalmologists, and the meeting is organized as a collaborative effort. This year, our meeting on October 6 has 38 optometrist attendees confirmed to date. We are, of course, very eager to be able to offer CE credit to these attendees who are dedicating an evening after work to join us.

  • There may be some misunderstanding about the concept of the course. Though we will be discussing three separate topics, there is only one meeting. We have followed the same format over the years that we have hosted this course, and it gives us the opportunity to address a number of separate topics at the same meeting. The speakers, discussants and attendees will be exactly the same for the three topics, as will the date, time and venue. This format is exactly the same as any larger meeting where there would be a long list of different topics discussed at the same single meeting.

We will, of course, be happy to comply with the requirement for a detailed summary, a course outline and presentation materials.

Would you please contact me personally if there are any issues. Based on past experience over many years, we have promised CE credits to attendees in our meeting invitation, and they are all accustomed to receiving those credits in prior years. I would feel personally responsible if CE credits are not available. My email address is: [email protected] My cell phone number is: 415-290-9720

    Thank you, Bernd Kutzscher, MD

  • Femtosecond Laser for Cataract Surgery

    Cataract LASIK

    16

  • Femtosecond Laser

Uses the same type of laser energy as for flap creation in LASIK.

Femtosecond laser operates with pulses at a duration of 1015 of a second

Thats one quadrillionth of a second A femtosecond is to a second as a second is

    to 31.7 million years

    17

  • Femtosecond Laser

Unusual properties when laser energy is delivered in this way.

If the laser is slower than a femtosecond, there is time for heat to spread and for energy to dissipate.

At femtosecond pulse duration, the pulse is absorbed before heat diffusion takes place.

Nearly 100% of the laser energy goes to the removal of tissue without heat damage. 18

Less corneal trauma Less inflammation Less risk of posterior capsule break with

vitreous loss Future advances such as new lens implants

    32

  • Femtosecond Laser for Cataract Surgery Regulatory Issues

Cannot use femtosecond laser to perform the usual steps of cataract surgery and charge patients for the higher cost (golden scalpel rule)

Can use the laser to add additional steps not covered by Medicare such as refractive steps (LRI) and intra-operative OCT imaging

    33

  • Femtosecond Laser for Cataract Surgery

Financial Added cost from price of laser and from

per click fee to laser manufacturer Approximately $900/case in added costs

more than the entire Medicare allowable for cataract surgery

Patient is charged $1,500 for added services

    34

  • Femtosecond Laser for Cataract Surgery Co-Management

Additional attention to refractive result especially astigmatism

Ability to open LRI post-op Usual co-management fee structure applies

following Medicare principles: 20% for post-op care

    35

  • Co-Management Fees - 2016 Procedure Patient fee

per eye Professional fee for co-management calculation

    Co-Management Fee Per Eye

  • The Future for Optometry and
