October 13, 2015 Subject: Request for Quotation Maintenance Facility- Commission Meeting Room Design The Madison Metropolitan Sewerage District is currently in the process of constructing a new Maintenance Facility on the Nine Springs Wastewater Treatment Plant campus. This project includes what will be the new primary meeting room for the District’s Commission. The 9- member Commission provides oversight and governance of the Madison Metropolitan Sewerage District. The purpose of this letter is to request a quotation for design services for this new meeting room. The goal of these services is to produce a final plan and recommendations for the layout and setup of the new meeting room. 3. Meet with District staff to discuss recommendations and options

December-January 2015: 4. Utilize feedback from District staff to produce final recommendations. 5. Compile recommendations into a scope specification to be used for bidding. 6. Meet with District staff to discuss if necessary (include this cost in your quotation).

For your reference I have attached a subset of the construction plans for the new Maintenance Facility. On the plans, the meeting room located on the southeast corner of the building and is shown as Meeting/Training, room 102. A full set of plans can be made available electronically (PDF or DWG) upon request. Your proposal should indicate an estimated total price for the work. Also show the unit price rates (time and materials) to be used. Include with your quotation a brief summary of any relevant previous work and project team experience. Note that this project scope includes only the production of a plan. The implementation of the plan is beyond the scope of these services.
