Everyone has an SCR that says:
What medicine you take
If your medicine means you need to be careful about other things
If you are allergic to anything
Who has a Summary Care Record (SCR)?
We think SCRs should say morethings, like:
If you have a Learning Disability
Contact details for next of kin or an advocate
What communication needs you might have
Who has a Summary Care Record?
At your Annual Health Check your GP should ask if you would like a Summary Care Record with extra information
This is new question and has been added to the Annual Health Check Template that GPs use on their computers

SCR is for people in Hospitals, Community Care, and Ambulances to use so they know important things about you
Being able to see more means people can care for you better
The GP Practice needs to keep the SCR up to date when they get letters from hospitals or community care
You can’t access the SCR online.
If you want to see it, ask your GP to show you what it looks likes at your next annual health check or GP appointment
You can ask for a print out of your SCR to keep
It can be used everywhere in England
It can be used if you have to go to hospital
It can be used if you get medication from the Pharmacy
It can only be seen by Health professionals who are involved in your care

Lawrence has difficulty hearing and his speech can be hard for people to understand. He can get anxious with people that he doesn’t know.
Lawrence has an annual health check at his GP practice.
He is with his sister who is his next of kin
Lawrence’s story
His love of football and his fear of needles is recorded by his GP
Lawrence and his sister talk to the GP and they decide to add these to his Summary Care Record (SCR)

Six months later Lawrence goes to A&E with a carer
He has been refusing to eat for the last few days and clutching at his chest
The Hospital access Lawrence’s SCR
Lawrence’s story
Lawrence’s SCR:
• Shows that he has a Learning Disability
• It shows contact details for his sister
• It shows that he likes football
• It shows medication that his GP has given him, and that he has had stomach pain
Lawrence needs a Blood Test
At first Lawrence is anxious
The hospital staff talk to him about football which makes him feel much less anxious
How did it help Lawrence?
Lawrence agrees to have the tests with some pain relief cream. He is found to be anaemic (not enough iron in his body)
An endoscopy (a type of test) is arranged. His sister helps him so he is less anxious
He is found to have a stomach ulcer but he is started on treatment and sent home
How did it help Lawrence?
The SCR told the hospital staff that Lawrence had a learning disability so they knew to make reasonable adjustments
The SCR helped the staff know what would help Lawrence feel calm (football)
The SCR meant that the staff knew what tests to do
How did it help the hospital staff?
The SCR meant that Lawrence could be more involved in his care because the hospital knew to call his sister to support him
The SCR saved time because the staff did not need to phone the GP to ask for information
