Summer Academic Enrichment Program IMPORTANT DETAILS

● The BCS Summer Academic Enrichment Program takes place in the BCS Daycare room(Ministry Center, room C107). If students are out of the class on an adventure, a notewill be posted on the door stating the children’s location.

● You MUST sign your child in with your full signature, no initials, including the time, atmorning drop-off, and sign him/her out at the end of the day. This is a state regulation,and helps to ensure the safety of all the children.

● The summer program staff will have copies of your Emergency Information, whichprovides the list of people who have permission to pick up your child(ren). NO studentwill be released to anyone not on this list without prior written approval from you! Ifthe teacher does not know the person picking up your child, a picture identificationwill be required before your child will be released. WE WILL NOT ALLOW A CHILD TOLEAVE WITH ANYONE WHO IS UNABLE TO PRODUCE THIS FORM OF IDENTIFICATION.

● For non-Bethany students, there is a $75 non-refundable Application fee/per studentdue at the time of registration for the Summer Academic Enrichment Program.

● For ALL summer program students, there is a $100 non-refundable Enrollment fee/perstudent due at the time of registration for the Summer Academic Enrichment Program.This fee will be applied to the student’s summer tuition.

At Bethany Christian School, your child’s well-being is our highest priority. Enclosed in this packet are important information forms that will assist our staff in making sure your child’s experience with us is fun, fulfilling and safe.

Please complete and return the forms PRIOR TO your child’s first day of attendance.

BETHANY CHRISTIAN SCHOOL Summer

Academic Enrichment Program

Each two-week session of the Summer Academic Enrichment Program has been assigned themes that we will explore. Educational activities have been chosen with this in mind.

SNACK AND LUNCH

Parents will need to provide snacks and lunch Monday through Friday. There is no microwave service available so please do not provide a lunch that requires heating.

DEVOTIONALS

Devotionals will occur on a daily basis and will focus on our theme verse as well as other Biblical concepts.

WATER DAYS

We will be having swim days at Waterworks Aquatics every Tuesday and Thursday (Pool days to be determined upon availability; finalized in May). Swimming will begin at 12:00pm. Girls must wear a one-piece swimsuit. If we feel a bathing suit is inappropriate, we will, if available, put a t-shirt on your child or he/she will be unable to participate in the swim activity for that day. Your help and understanding are greatly appreciated.

Please label all WATER DAY items and keep them in a backpack.

● Summer Program Application Fee of $75, for non-Bethany student,is non-refundable, non-transferable and DUE at the time of registration

My child’s enrollment in the Summer Program will be automatically terminated for payments not received by the published due date(s) or any unpaid balance comprised of additional fees incurred.

A discount will not be applied for days not attended which includes sick, vacation, and holidays.

After June 15, 2018, no refunds will be given for weeks/days not attended.

Late pick-up is strongly discouraged. I will be charged $1.00 for each minute my child remains past his/her contracted pick-up time.

I will be charged a $30.00 fee if my check is returned to Bethany Christian School unpaid, in addition to any late charge(s) that may apply. If a second check is returned unpaid, I will be required thereafter to pay with cash, money order, cashier’s check or certified check.

I am aware that my student’s behavior and conduct, both at BCS and school sponsored events, is expected to meet the standards established by the school for maintaining an atmosphere conducive to learning. Students whose behavior and conduct consistently make it difficult to maintain such an atmosphere will be subject to dismissal or denial of re-enrollment. The Program Director will make such decisions after careful consideration, which may include a conference with me, teachers and my student.

I hereby give permission for my child’s photographs to be used for program or promotional purposes, i.e. school news, school brochure, newspaper articles, web site, etc. Such material shall remain the exclusiveproperty of Bethany Christian School and shall be used without notice or compensation.

I understand that in signing this contract for the Summer Program, I am agreeing to accept the rules and policies of Bethany Christian School.

Summer Academic Enrichment Program Field Trip / Sunscreen Permission Form

FIELD TRIP PERMISSION

I the undersigned, being the parent or guardian of , give my permission to Bethany Christian School

to take my child on outings and field trips during the summer program. Transportation for such trips may be by public carrier or private vehicle.

I also authorize the designated leaders of such outings and field trips, in the event of an emergency and if contact cannot be made with myself, pursuant to Section 25.8 of the California Civil Code, to consent to any x-ray examination, anesthetic, hospital or medical treatment for my child which is deemed advisable by and to be rendered by or under the supervision of a physician licensed under the provisions of the medical practice act in which state treatment is rendered. I accept liability for the cost of any such treatment.

This consent is to remain in effect for the summer term 2018 unless sooner revoked.

SUNSCREEN PERMISSION

I hereby give the staff of Bethany Christian School permission to apply sunscreen on my child as needed. I understand that it is my responsibility to provide sunscreen with my child’s name on it to the summer staff. Due to the potential of allergic reactions to certain sunscreens, Bethany Christian School cannot provide the sunscreen for me.

WATERWORKS AQUATICS

WAIVER/RELEASE OF LIABILITY (201.4 V.1)

Please read carefully before signing! This is a release of liability and waiver of certain legal rights.

I, _________________, the parent/guardian of the participant and/or the participant agree and understand that

swimming is a hazardous activity. I recognize that there are risks inherent in the sport of swimming, including,

but not limited to, a risk of slipping on wet decks, orthopedic injuries, brain injuries, paralyzing injuries and/or

death.

I am aware that when lessons are being taught, it is extremely difficult or impossible for the instructor and /or

lifeguard (if on duty} to supervise all children at all times. I hereby agree that I shall use extreme caution and

supervision on behalf of me and my children at all times including "playtime" while on the premises of

Waterworks Aquatics.

The parent/guardian of the participant and/or the participant hereby agrees to participate in the

Waterworks Aquatics Swim Program and hereby agrees to indemnify and hold harmless Aqua tic Ventures,

LLC and Waterworks Aquatis, its teachers, officers, directors, agents and employees against any liability

resulting from any injury that may occur to the participant while participating in the Waterworks Aquatics

Swim Program. The parent/guardian of the participant and/or the participant also agrees to indemnify

Aquatic Ventures, LLC and Waterworks Aquatics for any damages incurred arising from any claims,

demand, action or cause of action relating to the participant.

The parent/guardian of the participant and/or the participant authorizes any representative of Waterworks

Aquatics to have the participant treated in any medical emergency during their participation in the

Waterworks Aquatics Swim Program. Further, the parent/guardian of the participant and/or the participant

agrees to pay all costs associated with medical care and transportation for the participant.
