section 3 on unsure patients, and the patient factsheet and opt in/out form now points out that following
a decision by a patient to withdraw their record from the system, that SCR will be hidden to other
viewers. The factsheet also includes a national helpline provided by CfH for patients seeking information
or who need access to translation services to help them further.
It is hoped that this will ease some of the workload arising from the higher profile which the SCR is now
receiving in the media as a result of the profession’s concern. With regard to that workload, whilst it is
important to recognise the impact on practices of the SCR scheme, it is also important to recognise the
need to balance the risks alluded to in the practical guidance, as a consequence of the scheme’s inherent
opt-out approach to consent.

2 Dealing with violent patients – a reminder for GPs and practice managers
General practices should publish a policy statement in their practice leaflet which makes it clear that they
will not tolerate violence towards healthcare workers. Every practice should have a policy on handling
violent patients based around paragraph 21 of the nGMS Regulations, which deals with the removal of
violent patients from a practice list, as well as local PCT guidance.
Please note that the Regulations cover acts of violence against:
(a) the contractor where it is an individual medical practitioner
(b) in the case of a contract with two or more individuals practising in partnership, a partner in that
partnership
(c) in the case of a contract with a company, a legal and beneficial owner of shares in that company
(d) a member of the contractor's staff
(e) a person engaged by the contractor to perform or assist in the performance of services under the
contract, or
(f) any other person present:
(i) on the practice premises, or
(ii) in the place where services were provided to the patient under the contract
An act of violence against any of the specified people in the list should be treated with the same
seriousness, regardless of whether they are partners in the practice or not.

3 Launch of the Londonwide buying group
The Londonwide Buying Group, part of the Federation of Buying Groups run successfully by Nottingham
LMC, was launched last week. The Buying Group will offer all member practices access to a number of
pre-approved suppliers offering discounts on a wide range of goods and services from medical equipment
to vaccines and more.

4 Integration of Sutton and Merton adult community services – a message from the PCT
The development of the concept of the Virtual Community Hospital (VCH) for Adult Community Services
began in 2009, aiming to deliver excellent co-ordinated care within the heart of the community through
staff leadership, clinical excellence, patient involvement and promotion of self care and personalisation.
The concept of the VCH relies on principles from acute hospitals, managing patient care and throughput.
This has enabled the improvement of the referral, admission and initial patient allocation processes, and
allowed community services to see more patients through efficiencies.
The Single Point of Referral (SPR) was launched in April 2009 to remove the complexity and simplify the
access points into adult community services. This was done by replacing the Trust’s existing referral
processes of having at least 75 telephone numbers for referrals to one number and one process. It also
ensured that clinicians had adequate information at referral enabling more efficient assessment of the
patient at the initial assessment. It removed confusion for referrers, who did not always know to whom
they should refer and for what service. In some cases patients were referred unnecessarily to more than
one team.
Subsequently it became apparent that a number of clinical services were providing very similar clinical
input, particularly when supporting hospital discharge and preventing unnecessary hospital admission. In
order to improve this inefficiency, work has progressed to a next phase of organising the way services
are delivered, to follow clear and simplified pathways with agreed clinical outcomes, which will improve
productivity and create a better patient experience.
The project has indentified the need to provide services along three main pathways: Prevention of
Admission; Maintenance at Home; and Supported Hospital Discharge with a matrix of specific clinical
pathways running across all three eg UTI, IV antibiotics. In order to support the way staff deliver clinical
intervention along these pathways the line management of the staff has also been reviewed.
Progress to date
By April 2010 all intermediate care nursing staff will have been realigned into Community Nursing
services. The majority of staff have gone into community nursing clusters with two intermediate care
practitioners moving into the Rapid Response team to support unnecessary hospital admissions to St
Helier A&E. This has enabled the ‚intermediate care‛ function to be embedded within wider community
services, and nursing to be delivered more efficiently by one service.
Robust mechanisms have been introduced to support reduction in length of stay within intermediate care
beds. All patients now have an estimated date of discharge on admission which is regularly reviewed
with the community nursing clusters adjacent to the bed bases providing any community nursing care to
ensure consistent high quality nursing care.
A Prevention of Admission pathway has been developed and was launched on 6 April 2010. This pathway
supports how referrals are managed for those patients who have a health need that is best met outside
of an acute hospital. Referrals will continue to be made via SPR direct to community nursing clusters.
This will enable patients to receive a comprehensive holistic assessment that can be shared with the
team using the RiO electronic record. The pathway promotes the use of comprehensive care planning
and delivery of appropriate care and rehabilitation. The patient experience will be enhanced by the
introduction of ‘Key Workers’ supporting excellent coordinated care and communication. The formalised

Supported Discharge Pathway and Maintenance Pathway will be launched at our Nursing and AHP
conference in June.
It is anticipated that experience for referrers will remain efficient with communication between primary
care and community services continuing to improve. This model of care delivery promotes excellent
communication between teams, using opportunities to liaise with Primary Care Teams on a regular basis
to plan and review patient care, with the patient being at the centre.

5 Dr Tony Stanton’s retirement
At the end of October 2010 Dr Tony Stanton will be retiring as Joint Chief Executive of Londonwide
LMCs and Londonwide Enterprise Ltd, along with his role as LMC Secretary/Medical Director to our
LMCs in SE and SW London. We shall shortly be advertising for an LMC Secretary/Medical Director to
perform that role. Please watch out for the advertisement if you are interested in applying.

6 Conference gives confidence to the capital’s GPNs
Londonwide LMCs held its first, exclusive conference for practice nurses on Wednesday 17 March 2010.
This highly successful, well attended event provided practice nurses in London with an opportunity to
share good practice, celebrate successes and network with colleagues in the profession. The morning
sessions gave a national overview of changes, highlighted London’s unique challenges and gave practical
advice on how to develop services and have an impact back in the practice. Delegates also saw the
launch of the exclusive online training programme and one lucky nurse won a free course worth £450 in a
prize draw!
The afternoon focused on practical, interactive masterclasses on clinical topics and gave advice and
solutions to issues faced by practice nurses on the ground.

7 Londonwide LEAD (Learning Education and Development) – providing training for the capital’s
practice teams
March was a month of ‘firsts’, with Londonwide LMCs’ first ever practice nurse conference and a
taxation webinar.
The GPN conference was a huge success and is covered elsewhere in this newsletter. The very popular
taxation webinar was led by the Customs Investigations and Litigation Team from Vantis. The lunchtime
webinar on 23 March 2010 covered the new HM Revenue and Customs (HMRC) Tax Health Plan (THP)
for health professionals. If you were not able to participate in this particular event, further information is
on the HMRC website. There is also a frequently asked questions document that you may find useful.

percentage of doctors need help and encouragement with career development. The day will include

to stand would be eligible. Don’t forget that we welcome representation from all GPs including partners,
salaried, sessional and freelance GPs.
Londonwide LMCs, following its commissioned report on diversity, conducted independently by Judy
Gilley Associates, established a diversity strategy, and we are fully committed to representing and
supporting all GPs and their practice staff. One of our priorities is to continue to improve engagement
with our sessional GP colleagues. We recognise that to maintain quality and choice in general practice for
its patients that the profession should work together in equity regardless of the different roles that are
available for a working GP.
We would therefore particularly welcome nominations from those GPs who are underrepresented on
current LMCs. Dr Junaid Bajwa, a PMS Salaried GP and a member of Greenwich LMC says: ‚Salaried
doctors can really benefit from being a member of the LMC as it provides a vital outlet for raising issues
and concerns that may not affect some of our colleagues in different contractual positions. It is so
important to be involved in these changing times for general practice, so regardless of your involvement
in medical politics, or your level of understanding of GP issues, you will be welcomed and fully supported
as a new LMC member.‛

9 LMC statutory levy 2010/11
The LMC Statutory Levy has been reduced from 52.1p to 51.5p per patient per annum from 1 April 2010,
as a result of the increased patient population in London. We work very hard to keep our expenditure as
low as possible. Other than a modest increase in funding to support LMC members in carrying out their
representative roles, our budgets have been maintained at last year’s levels.

10 GP earnings and expenses 2007/8 final report
The NHS Information Centre has published the GP Earnings and Expenses 2007/8 Final Report. This
publication supersedes the GP Earnings and Expenses 2007/08 Provisional Report, released on 16
September 2009. The publication contains all data from the provisional report, with additional information
on earnings and expenses by SHA (Strategic Health Authority), practice size and rurality.

11 DDRB: 39th report 2010
The letter to the profession on the Doctors' and Dentists' Review Body's (DDRB) 39th report for 2009
from the GPC was published on 11 March 2010.
Regrettably, the Government has not accepted the DDRB’s recommendations in full and has decided to
reduce the uplift awarded to GP contractors. Further details are set out in the letter.

12 Focus on the new tax brackets and other financial considerations
The Finance Bill 2008-09, which came into effect on 6 April 2010, introduced significant changes that
affect those earning over £100,000 a year. The General Practitioners Committee (GPC) has produced
‘Focus on’ guidance to highlight potential ways to mitigate the effects of the tax changes which will
affect doctors earning over £100,000. The GPC does state that this guidance is not comprehensive and
GPs should consider their own case in the light of their personal circumstances and take advice where
necessary from an accountant.

13 Directed enhanced services 2010/11 – new directions
The Primary Medical Services (Directed Enhanced Services) (England) Directions 2010 provides details of
the DESs that apply for the new financial year. The following time-limited DESs are extended into the
new year:
extended hours access
alcohol related risk reduction
ethnicity and first language recording
learning disabilities health check
osteoporosis diagnosis and prevention.

14 The extended hours access scheme 2010 for England
The General Practitioners Committee (GPC) and NHS Employers have agreed that the Extended Hours
Access Scheme will continue for a further year from 1 April 2010.
The associated DES Directions, which came into effect on 1 April 2010, can be found on the Department
of Health website as can the directions amending the SFE.
The main change from the existing arrangements will be that practices will be required to indicate by
30 June 2010 whether they are proposing to participate in the new Extended Hours Access
Scheme (or equivalent local arrangements) in 2010/11. This is so that PCTs are clear early in the financial
year which practices will be involved.

15 General Medical Council (GMC) consultation on revalidation
The GMC consultation, which will end on Friday 4 June 2010, focuses on the following four issues:
how revalidation will work
what doctors, employers and contractors of doctors’ services will need to do
patient and public involvement in revalidation
how and when revalidation will be introduced.

16 Amendments to GMS, PMS and NHS performers' list regulations
These regulations have been amended by SI 2010 No 578. A copy of the regulations can be found on the
Office of Public Sector Information (OPSI) website.
The GMS standard contract and a variation document taking account of amendments are currently being
prepared and will be published on the DH website along with revised alternative providers of medical
services (APMS) and primary care trust medical services (PCTMS) directions.

17 Health emergency badge (HEB) scheme
The HEB can be used by people involved in the delivery of primary healthcare who attend medical
emergencies in patients' homes. All London Boroughs have agreed to the scheme, although it is not a
mandatory provision and it is offered entirely at the discretion of London parking authorities. The badge
has no legal status.
When the badge is clearly displayed and used in accordance with the conditions of use, badge holders
should not receive parking tickets, be clamped or towed away.
Any practice may apply for badges if they employ staff whose work involves visiting patients in their
homes to provide emergency health care. This includes doctors, nurses, midwives and health visitors.
The badge can only be used when visiting a patient to provide emergency healthcare in their home.
'Emergency healthcare' may include when:
A situation in which a patient needs immediate treatment to avoid possible loss of life or where life
saving equipment in the home has failed
A patient needs immediate treatment to alleviate acute pain or other distressing symptoms
Childbirth is imminent or immediate post-natal treatment is required
A child is in danger or a person is at risk of violent attack
A patient is suffering a mental health emergency and poses a risk to themselves and/or others

18 Fees to access and copy health records
The Data Protection (Subject Access) (Fees and Miscellaneous Provisions) Regulations 2000 sets out the
fees a patient may be charged to view their records or to be provided with a copy of them. These are
summarised below.
To provide copies of patient health records the maximum costs are:

Health records held electronically: up to a maximum £10 charge.
Health records held in part electronically and in part on other media (paper, x-ray film): up to a
maximum £50 charge.
Health records held totally on other media: up to a maximum £50 charge.
All these maximum charges include postage and packaging costs. Any charges for access requests
should not be made in order to make a financial gain.
To allow patients to view their health records (where no copy is required) the maximum costs are:
Health records held electronically: a maximum of £10.
Health records held in part on computer and in part on other media: a maximum of £10.
Health records held entirely on other media: up to a maximum £10 charge, unless the records have
been added to in the last 40 days in which case there should be no charge.
Please note that if a person wishes to view their health records and then wants to be provided with
copies this would still come under the one access request. The £10 maximum fee for viewing would be
included within the £50 maximum fee for copies of health records, held in part on computer and in part
manually.

19 Fit notes are operational now!
As we have previously reported, the ‘fit note’ comes into effect this month.
The ‘fit note’ can be used to advise patients whether they can return to work earlier if supported by their
employer. It also replaces the Med 3 and the Med 5 sickness certification forms. Detailed guidance for
GPs has been produced and the Department of Works and Pensions (DWP) has published a sample ‘fit
note’.

20 IB/ESA 113 form gets an electronic template
The DWP’s IB/ESA 113 form no longer has to be completed by hand as an interactive electronic version is
now available. GPs may be asked to complete the form if the DWP thinks that a patient may have a
severe health condition or disability, but does not have enough information to be sure.
Please download the form onto your practice system for future use. Please also read the guidance notes
before using the form.

21 New mental health resource for relatives
South London and Maudsley NHS Foundation Trust has teamed up with the Institute of Psychiatry to
launch www.mentalhealthcare.org.uk. The website provides the carers, families and friends of people
suffering any form of psychosis with information on the different aspects of treatment and care. The site

The Advisory Committee on Cervical Screening (ACCS) has produced new guidance for primary care
covering the management of young women (aged 20-24) who present with gynaecological symptoms.

voluntary sector.

23 Latest version of the NHS Constitution
The latest version of the NHS Constitution now includes a new right to start non-urgent treatment within
18 weeks. It also stipulates the right to see a specialist where cancer is suspected within two weeks of
referral, or for the NHS to take all reasonable steps to offer a range of alternative providers where this is
not possible.

Invisible patients specifically focuses on the health issues facing regulated health professionals and
identifies some priorities for addressing them. It looks at how ill health in health professionals may affect
their professional practice, the difficulties they face in seeking help and the role of the health profession
regulatory bodies.

25 The consultation on the choice of practice closes on 28 May 2010
This Department of Health (DH) consultation is seeking views from the public, healthcare professionals
and from other staff working in the NHS on new proposals that give patients a greater choice of GP
practice.
The consultation sets out the different options for organising healthcare for patients, and the potential
implications of their choices if the current system of GP practice boundaries is removed.

26 GP2GP – interim guidance on erroneous transfer from the GPC
Guidance has been developed by the GP2GP project and the Joint GP IT Committee to advise practices
how to reduce the risk of making an erroneous record transfer request and advising practices and PCTs
how to manage such erroneous requests when they do occur. Erroneous record transfers usually occur

when patients are incorrectly identified when registering with a new GP practice. This may result in a
request being made for the wrong record via a GP2GP transfer and a patient being inappropriately
deducted from their true registered general practice.
Registering new patients – Correctly identifying and registering new patients on the demographics
database (PDS) is absolutely key in reducing the risk of erroneous GP2GP record requests being
made. When completing the GMS1 registration form, practices should carefully check the accuracy
of patient data and try to provide as much information as possible, preferably including the NHS
number when it is available. The GMS1 should be checked in the presence of the patient to check its
legibility, completeness and accuracy. If a patient cannot be positively identified, practices might
consider asking registering patients to provide formal identification and proof of recent address to
ensure that correct GP2GP record transfer occurs. If in doubt, registration should be deferred and
advice sought from the PCT/Patient Services Agency (PSA).
Erroneous transfers and the sending practice – Most erroneous transfers come to light when the
patient contacts their practice (the sending practice) for an appointment or prescription, to be told
they are no longer registered. Sending practices should contact their system supplier helpdesk to
report the erroneous transfer. The practice should also contact their PCT/PSA to request that the
patient’s registration be reinstated and consider informing the patient what has happened.
Erroneous transfers and the receiving practice – The practice requesting the record (the receiving
practice) may also identify that they have the wrong record, or be informed by their supplier helpdesk
that they have registered the wrong patient. Practices should contact their PCT/PSA to advise them
of the erroneous transfer and with the support of the PSA and supplier helpdesk, arrange to delete
the erroneous NHS number and ‘roll back’ the clinical system so that the erroneous incoming GP2GP
record is deleted and no patient details remain in the receiving practice. The receiving practice might
consider whether an erroneous transfer should be considered as a practice critical incident, to reduce
the risk of further such errors.

Life clubs are practical and structured 90 minute weekly self-improvement workshops around London.

Supervision skills for clinical teachers: conversations inviting change, 14 April, 28 April and 12

Course details: Good supervision means looking after people, not just looking over their shoulders. It
is about creating time and space to help people reflect on their encounters with patients, colleagues
and managers. Supervision skills depend on active listening and often on the focused use of
questions in order to challenge people to think about new ways of dealing with stuck or difficult
situations. Supervision skills are useful in vocational training, appraisal, mentoring, self directed
learning groups, and in enhancing your own consultation skills. This three-day course offers an
extended introduction to 'conversations inviting change'. It is suitable for people who have
completed a one-day workshop at their trust or the Deanery, or who want to do the course because
of an interest in conversational skills for supervision. As well as providing opportunities for skills
practice, there is some theoretical teaching and a certain amount of set reading before the course
and between sessions. The course is based on participants' own working experience. Please come
expecting to talk about current cases and dilemmas with patients, trainees, colleagues and teams.
This is not a comprehensive course in how to be a clinical or educational supervisor, but it will equip
