The Frank Haith Basketball Team Camp is open to all boys

high school & club teams. It will provide an opportunity for

campers to improve their skills while playing in organized

games against great competition. The format for this camp

will include pool play games + a SINGLE ELIMINATION

TOURNAMENT on the Final Day. Teams will be categorized

into 3+ divisions (JV, Small Varsity, Large Varsity) with

each team guaranteed a minimum of 3 games per day.

1 Day Option = 3 Game Minimum on June 1 or 2.

2 Day Option = 6 Game Minimum on June 1 & 2.

3 Day Option = 5 Game Minimum on June 1 & 2 + Single

Elimination Tournament on June 3.

Who: Boys HS & Summer Teams Entering Grades 8-12.

Cost: • 1 Day: $250 per team • Saturday & Sunday: $325 per team • 2 & 3 Day: $450 per team ($400 w/ multiple teams)

• Includes free camp shirt for each participant.

• $225 cancellation fee will be deducted for refunds.

• Camp store will be available on a limited schedule during

camp for purchases of snacks.

Facilities: Games will be played on the campus of The

University of Tulsa (Donald W. Reynolds Center).

Additional facilities off campus may be used.

Camp Staff: Frank Haith’s staff will direct camp.

A medical trainer will be on site during camp at all times.

An officiating clinic will take place in conjunction with camp.

Housing: Housing is not provided during camp. Contact

office for local hotel information and rates.

• In order to FINALIZE your spot in team camp, a minimum

29, 2015. This will act as your non-refundable deposit.

Camp Philosophy

Welcome! We would love the opportunity to aid in your

development, on and off the court, in our 2018 Frank Haith

Basketball Camps. Our commitment to conducting the best

fundamental basketball camps is second only to having as

much fun as possible. Becoming a player is hard work, but we

believe the game should always remain fun!

Fundamentals, sportsmanship, teamwork, work ethic,

discipline and good listening skills are all components of

becoming a basketball player. We focus on these areas as we

teach the game of basketball in age appropriate groups. Daily

full court games add excitement to the atmosphere, build

confidence and help instill the love of competition and the

game of basketball.

Players excel during the season, but are developed in the

summer. Join us at our camps as we help you strive to become

a better person, teammate, and player.

Release and indemnity agreement to Frank Haith Basketball. I give my consent for him/her to participate in the camp programs conducted and/or sponsored by the Frank Haith Basketball Camp. I understand that participation in basketball, and related activities involve certain risks, and may result in unavoidable injuries. The injuries may include strains and tears, broken bones, and severe injuries including, but not limited to, permanent paralysis, or even death. I am fully aware of the risks and possibilities of injury involved and acknowledge that I am assuming the risk of such injury by my child’s participating in camp. I further acknowledge that I will be responsible for any and all medical and related bills that may be incurred by me for any illness or injury that my child may sustain during the camp and while traveling to and from the site for camp. I further acknowledge and authorize the employees of the Frank Haith Basketball Camp to act accordingly to their best judgment in any situation requiring medical attention, whether an emergency or not, until such time as I am contacted to make decisions concerning my child’s treatment. If in the judgment of a physician or designee it is necessary for health care reasons to proceed with treatment without delay, this treatment may proceed without prior notification of the undersigned, although every attempt will be made to notify me in the event of such injury or illness. I agree that any medication information provided to this camp shall be released to other health care providers who may be providing care. Knowing these facts and in consideration of my child’s participation in the camp program, I, acting as parent or legal guardian, agree to release and hold harmless the respective officers, directors, representatives, members, agents, employees, coaches, or agents of the Frank Haith Basketball Camp, the coaches and support staff of the Frank Haith Basketball Camp program, from any and all liability for negligence or any other claim, demand, action, judgment, loss, liability, cost and expenses arising out of or in connection with whether directly or indirectly, any illness, injury, damage or loss to

person or property that my child may incur or sustain during camp, all activities associated with camp, and while traveling to and from the site for the camp. I further acknowledge and authorize the Frank Haith Basketball Camp the right to photograph my child and use the photo and/or other digital reproduction of him/her or other reproduction of his/her physical likeness for publication purposes, whether electronic, print, digital or electronic publishing via Internet. I acknowledge that I have read this release and waiver of liability in its entirety and fully understand its contents. I am aware that this release contains an acknowledgement of my voluntary and knowing assumption of the risk or illness or injury. I further acknowledge that I have signed this document voluntarily and of my own free will.
