Director’s Update — Margaret Whoriskey
Programme update - The main focus over the past two months has continued around refining the
process and priorities for Years 2 and 3 of the TEC Programme, in consultation with a range of stake-
holders and finalisation of the associated guidance.
This has now been issued and is attached as an annex to this report. The six monthly reviews with
all funded areas and organisations have been completed and further information is contained in Board papers. The second tranche of
funding has been issued to most areas with funding still to be allocated to a few areas pending further progress – these areas will be reviewed again in January 2016.
Digital Health & Care Week—In addition to the main national conference on 9th
December there were a number of other national and local events taking place. This year's conference theme was “Using innovative technology to enable more integrated, sustainable & person-centred health and care”. A mix of workshops and plenary sessions were held to provide delegates with the time and space to discuss current
knowledge base on a range of topics; network with colleagues from across the relevant
sectors; and actively consider application to their own practice.
The DHI hosted one of their ecosystem events on the 10th December. This had a focus on
health & social care integration. We also arranged for the Cabinet Secretary to visit one of our TEC-funded projects on the 10th – the primary care-based high blood pressure home
monitoring in Lothian. The Health and Social Care Alliance held an event on 8th December and launched a report on
Realising the benefits of Digital Technology.
Other events—We have presented at a number of events at home and away. I spoke at the European Telemedicine Conference in Denmark at end of October. The national TSA
(Telecare Services Association) conference in Wales involved a number of TEC contributions. For the first time there was a dedicated award for individual contribution called the Billy
Graham Crystal award Delivering Excellence for TEC. Billy Graham was the Scottish rep for
one of the TEC suppliers and was sadly killed along with his wife during the Tunisia terror attack early this year. Doreen Watson who is working on our TEC Telecare programme was
the winner of the award for her work supporting partnerships across Scotland.
Business Modelling—One of the action points and conclusion of the Critical Success
Factors of Successful Adoption (MOMENTUM) workshop, held on April 16 2015, was to run and enable the development of new business models for deploying Technology Enabled Care
(TEC) at local level.

As part of this, we held a workshop with EHTEL on 15 September on business planning. The objective of the workshop was to address this action point by “training trainers” in order to enable them supporting local teams in
refining their TEC local business models. The workshop set out to:
Look at how to apply business modelling to TEC projects;
Test out business modelling during the day, with two local use cases;
Provide support material that can fit the Scottish context, i.e. a background document associated to a
PowerPoint template to propose to local teams. Improvement Programme—Building on the work of the TEC improvement programme, Delivering our Ambitions, we are offering improvement and financial support to Health and Social Care Partnership teams who are willing to
develop and carry out Tests of Change.
The aim of this is to demonstrate and share measurable improvement in outcomes brought about by the Implementation of Technology Enabled Care, and to complement and inform the future 2016-18 TEC programme
development. Four priority areas:
1. Measuring Telecare and Telecare Response Outcomes;
2. Measuring how TEC contributes to supporting people to remain at home (avoiding unnecessary hospitalisation
or long stay placement);
3. Measuring the impact of embedding TEC in assessment pathways;
4. Measuring the impact of training and awareness raising on the use and uptake of TEC.
Measurement Tool - A "Task and Finish" group is finalising data definitions and refining the measurement tool,
and the updated tool launched in January 2016.
Events—Improvement Programme - East/West and Remote and Rural Regional learning events took place in November and December - focusing on "Making Connections", sharing experience and knowledge of TEC between
organisations. The next national learning event scheduled for 16th March 2016 and we will be approaching some Board members to contribute /chair sessions.
Show and Tell—We are planning some ‘show and tell’ sessions for Board members and TEC workstream leads to
demonstrate what is happening in some areas that are in receipt of TEC funding. The first such session is going to focus on work in the Lothian area with Professor Brian McKinstry and will be held at Bioquarter near the Royal
Infirmary of Edinburgh. Provisional dates are 10th, 15th, 17th, 22 or 23rd February. Michelle Brogan and Sue Scotland
are organising this session and I hope we can fix on a date that will suit at least a few Board members.
Evaluation—We reached agreement at the last Board Meeting to commission an evaluation of the TEC Programme
in two separate phases. Phase one, year one findings and initial scoping work in relation to the full evaluation of the
TEC programme, is still currently sitting with procurement before being released to tender.
European funding opportunities - The Scottish Government is in a strategic partnership with the Governments
of Northern Ireland and the Republic of Ireland within the Cross Border Cooperation Programme (INTERREG V-a) 2014-2020. This covers the border region of Ireland, Northern Ireland and Western Scotland (Dumfries & Galloway,
East Ayrshire and North Ayrshire mainland; South Ayrshire; Lochaber, Skye & Lochalsh, Arran & Cumbrae and Argyll
& Bute and Eilean Siar/Western Isles). The first stage call is now closed and there have been 21 applications with many featuring Scottish interests. The evaluation will be undertaken in January 2016.
New arrangements for TEC - I updated Board members about the changes around the integration of JIT with
HIS and the change to my role. I will continue to have responsibility for the Technology Enabled Care Programme and wider telehealth and telecare activity within Scottish Government. We are setting up a Technology Enabled Care
and Digital Innovation Division sitting under the Director of Finance, eHealth and Analytics and working closely with colleagues in eHealth Division and Digital Directorate. Given the change to my role and my ‘executive’ responsibilities
around delivery of the TEC programme John Matheson has nominated John Burns to take over as chair of the TEC
Programme Board from January 2016. We will review the Terms of Reference at our next meeting of the Board.
Forward Look—Deloittes is hosting a round table event on 14th January focusing on ‘Connected Health: how digital
technology is transforming health and social care’ on 28th October. This will help us shape the future Delivery Plan for Telehealth & Telecare, possibly under the wider umbrella of Digital Health & Care. This new Delivery Plan will
continue to provide the strategic direction for the development of TEC.

Development of the National Model is progressing—current focus on ‘The Case for Adoption’ and Model
Pathways
Developing elements of ‘The case for HMHM’ Demand & Capacity, Return on Investment guidance etc.
Applying the Model Pathway to Heart Failure and Diabetes

Video Conferencing
Infrastructure for Guest, Care Homes, SWAN—initial Guest testing completed. Awaiting installation of

VC roadmap agreed. Deployment of Acano solution to provide integration with MS Lync and WebRTC
solution to provide virtual clinics agreed
Digital Platforms
Draft definitions and services report circulated to stakeholders on 25th November for review. Date being
sought for January project Board to sign off.
Living it Up
The MyAccount integration with the Living it Up platform was successfully implemented in October 2015
and is now operationally live.
From the end of October, we now have 20,000 full users and members registered with Living it Up.
Analogue to Digital Telecare
Firstly, the team would like to say a big thank you to all those organisations whom completed their questionnaires in supporting the Analogue to Digital feasibility activity. The information provided is vital to the work currently being undertaken in this workstream. Workshop held with Farrpoint to review approach and deliverables
Revised approach will merge products 2&3, and will provide a more detailed analysis of the 3 options to
include cost
Engagement with Sweden’s digital lead around standards (SCAIP) has progressed to further inform study
Expansion of Telecare
Technology Chart for People with Dementia was published 30th November 2015

Improvement & Suppor t Update
Two new Improvement Advisors have joined QuEST and will be supporting the national technology enabled care improvement programme.
Lesley Middlemiss has just joined the team from NHS Forth Valley and will be closely aligned to
the national TEC improvement programme. Lesley has held a wide range of planning and improvement roles over the last 10 years within outpatients, ambulatory care, rehabilitation,
community hospitals and health and social care integration. For the last 4 years she has been service improvement manager with Forth Valley CHPs supporting the Reshaping Care for Older
People programme and as Integrated planning lead for Older Peoples Care Hub model within
the proposed new Stirling Care Village.
Michael Fox joined the team in October. He has worked within healthcare for over 20
years, in lead planning, performance and service improvement roles across community care, acute care and central government sectors. Michael has a specialist knowledge in the
application of improvement methodologies including Lean, Theory of Constraints and 6 Sigma.
TEC Measurement Tool
The TEC measurement tool has been issued to partnerships and returns requested for the last few months. We know this has been a challenge for many and we have noted these challenges and sought further feedback from
partnerships, to understand how the tool can be refined to support partnerships demonstrate improvement and scaling of TEC.
Work is on-going through the Task and Finish group to refine the tool, clarify some of the definitions and work
with partnership representatives to adjust and simplify the current tool. The new tool is due to be launched in time for January 2016 submissions.
Improvement Network
The improvement programme group on the Knowledge Hub has some new members, total membership is now
sitting at 93 people. The ‘Events’ tab is up and running, please take some time to visit the page and check what’s on over the next few weeks. Gordon Black has also put up a couple of Forum threads, feel free to add your
comments! The Knowledge Hub library is now packed full of resources that have been uploaded by the TEC team,
if you have something that you think would be value or interest to other partnerships please upload to the library cache. If you are uncertain as how to do this, or have any general queries about the Knowledge Hub please

TEC National Learning Event, 2nd October 2015
Over 90 delegates attended the last TEC national learning event was held on the 2nd of October in Stirling. All of the great work shared can be found within the Knowledge Hub.

On 16th November, TSA (the industry body for telecare and telehealth) hosted their annual awards as part of their conference. For the first time, there was a dedicated award for individuals called the ‘The Billy Graham Crystal Award - Delivering Excellence for TECS’ (Billy Graham was the Scottish rep of one of the suppliers, who
was sadly killed along with his wife Lisa during the Tunisia terror attack earlier this year).
Our very own Doreen Watson, who has been working as our telecare expert through the Action Group for a few
years now, was presented this award by the daughter of Billy and Lisa for all of her work supporting partnerships across Scotland.
A true reflection of Doreen’s work, we are sure you will all join us in saying congratulations. Well done to her, and
richly deserved!
Our Video Conferencing Workstream Lead Hazel Archer has been shortlisted for the ‘Digital Public Services Award’ in the 2015 Scottish Public Service Awards.
This award recognises the use of technology to deliver improvements in public services to the citizen.
The NHS Scotland National VC (Video Conferencing) Service has grown from a simple teleconferencing facility to a national service. It now offers a net saving of £5.1m each year and a further reduction in Scotland's carbon
footprint of 1.9k tonnes.

NHS Western Isles report that their very first Home Health Monitoring patient has experienced a remarkable change in health and wellbeing.
A 16 year old with diabetes, was admitted to hospital with exceptionally poor insulin control and history of
persistently failing to attend routine clinic sessions.
It transpired that the patient had lost all interest in checking their blood glucose levels and as a result had
stopped taking their insulin. This had resulted in the patient experiencing several prior admissions for Diabetic
Ketoacidosis.
The patient was offered an SMS based service called "Florence" as something that could potentially help.
6 weeks later and the patient now attends their clinic weekly and has a very much improved insulin control. The
patient reports that they 'love' having Florence in their life and that they are now determined to reach their HbA1c target level. This greatly reduces their risk of complications such as increased risk of Coronary artery disease and
stroke, Diabetic Retinopathy (blindness) and Chronic Kidney Disease.
The Florence service reminds the patient to test their blood sugars at specific times of the day and to reply with readings. Florence then responds to readings with clinically appropriate (algorithmic) guidance. Florence also
gives medication reminders and provides educational tips to the patient on managing hypos. The Diabetes team
also use Florence to send appointment reminders, the patient has not missed an appointment to date.
The Florence system is web based and as a result the specialist nursing team can access the latest readings from
anywhere in the country via their mobile device, this means that patient contact can take place virtually anywhere.
The team at NHS Western Isles aim to replicate these kinds of results across the island communities in the
coming months.

Renfrewshire Health & Social Care Partnership is delighted to announce that through the Technology Enabled Care Programme, Home Health Monitoring will be expanded into the community. The aim is to build upon the
current services in order to further reduce demand on acute hospital services by improving discharge
arrangements, improving self management, reduce face to face contacts in Primary Care, and provide alternatives to unnecessary hospital admissions.
To support these demands the Partnership will develop and upscale regular home health monitoring to service users who suffer from Chronic Obstructive Pulmonary Disease and Heart Failure. It is expected that people who
meet the inclusion criteria, will be referred by acute clinicians, primary care and community services to the Home
Health Monitoring programme. People will be monitored daily, Monday to Friday, by nurses who will support the person to manage their condition. We are also delighted to announce that two nurses have been appointed and
will commence recruitment on 1st of December 2015.

Telehealth and telecare are the focus of a new project based in Dundee Voluntary Action. Working with partners in social work and health, DVA has employed a communications officer to spread the word about what’s possible now and
what is coming soon. The postholder is Jimmy Black, formerly a PR with national voluntary organisations such as Citizens Advice Scotland.
He says, “I’ve been shown epilepsy sensors and safety beams at Dundee’s Social Care Response service; PAMIS have
shown me their Digital Passport for people with learning disabilities; I’ve met computer games companies making healthcare apps; and Blackwood’s Concept House will soon feature in a new housing development in the city. There’s a
lot going on.” Jimmy has also been analysing the websites used by local GP surgeries. “GP websites offer huge potential to improve services and save time. One surgery I spoke to handles over 1,300 repeat prescription requests online, and
patients can make and cancel appointments. That’s really good, but there’s scope for much more.”
The project is led by Senior Social Work Manager Alexis Chappell, who chairs the city’s Technology Enabled Care Group,
bringing together professionals from many disciplines within Dundee City Council, NHS Tayside and voluntary
organisations.

The aim of the Dementia Dog project has been to demonstrate the ways that trained dogs can support and
enable people to live well with their dementia.
It provides people in the early stages of dementia with a fully trained assistance dog. The dog is a companion, helper,
social connector and reminder for key daily tasks as well as delivering joy, delight and unconditional love.
The project has demonstrated the impact of the dogs in
three pilots:
Assistance Dog - trained and matched dogs live with the families to undertake tasks and companionship, matched to
the unique needs of the family.
Intervention Dog - a trained dog with a support worker offering a smaller outcome focussed session with families.
Typically visit lasting around 3 hours.
Dementia Dog Days - a group of families meeting with several dogs acting as a catalyst for conversation and joy.
Benefits and the unexpected benefits which in some ways became more powerful than those they had hoped to
achieve
• A marked improvement in the couple’s relationships (a greater sense of equality).
• Communication was greatly improved, a marked decrease in word searching.
• Motivation to continue with routines and interests that were declining, but since have been refreshed.
• A greater sense of self.
• Postponed need for more formal care arrangements in all couples.
• Dogs becoming intuitive and perceptive of individual needs.

Survey Feedback on Year 1 Activity Thank you to all who participated in the Survey Monkey
following the National Event in October 2015. The full document around lessons learned from Year 1 can be found on the
Improvement Programme Knowledge Hub page. Please see below a few comments that came out of the survey.
“The workshops where all partnerships get together is great - it is very
useful to find out what other partnerships are doing - what's worked or not
worked for them. Also being able to share our progress with other
partnerships is useful for them, I hope. Having a local focus on TEC and
having funds available to improve and develop services is great.”
“The programme allows the service to reach more vulnerable people than would be
possible based on the mainstream funding we have at present within our service. It has
also given use the opportunity to get out and promote the service to the wider community
allowing people to remain independent within their own home where previously the only
option was LTC. The outcomes show we are sustaining people at home longer.”
“We could probably develop better cross partnership links. Encourage more
working together between partnerships that are working on similar solutions -
this might result in economies of scale and cross fertilisation of ideas.”
