The Guardian Connect Continuous Glucose Monitoring (CGM) system is right by your side - helping you see what's really going on with your glucose levels.

The Guardian Connect transmitter works with a small sensor to continuously monitor your glucose levels and sends this information directly and conveniently to the Guardian Connect App that sits on your compatible smartphone.

CONNECT

CGM uses a small sensor to continuously measure glucose levels in the interstitial fluid just below the skin. It can take readings all day and night and send them to you via a small wireless transmitter, so you can see how you’re doing anytime.

The Guardian Connect system can also send the information it receives to your Medtronic Carelink® account online, which your healthcare professional can look at before your appointment. The system also enables care partners to be set up to receive SMS alerts at predetermined values for added reassurance*.

WHAT IS CGM?

*Healthcare professionals access requires the user to grant permission. Care partner alerts are subject to Guardian Connect Internet connection (Wifi or mobile network) and mobile reception of the Care Partner. It is important you and your care partner do not rely exclusively on these alerts as part of your diabetes treatment plan.

Safety InformationContinuous glucose monitoring systems are intended for monitoring interstitial glucose levels in persons with diabetes. These systems provide information regarding potential high and low glucose levels and can provide an indication of when patients should obtain a glucose reading using a home glucose reader.

The information provided by continuous glucose monitoring systems is intended to supplement, not replace, readings from a home glucose meter. Insertion of a glucose sensor may cause bleeding or irritation at the insertion site. The Guardian Connect app requires that you have a functioning mobile electronic device with the correct settings (such as a supported operating system and Bluetooth® enabled) for accurate operation.

The patient's smartphone (running with Guardian Connect app) must be connected to the Internet via WiFi or a mobile data network (e.g. a 3G or 4G network) to enable a care partner to receive SMS alerts. If the patient receives an alert when the Guardian Connect app can't connect to the Internet, the care partner will not be able to receive an SMS alert.

It is important you and your care partner do not rely exclusively on these alerts as part of your diabetes treatment plan.
