Voluntary Life Insurance:The annual re-enrollment period for the Lifestyle Protection Plan is now open. This is an opportunity for employees to change their Lifestyle coverage or join the plan. Employees who are currently enrolled do not need to complete new enrollment forms to keep current coverage amounts. The enrollment period will end on June 30, 2013. The following information applies to the Lifestyle Protection Plan: Why re-enroll? Employees who are currently enrolled may increase their coverage amounts without Evidence of Insurability up to the non-medical maximum ($50,000 for employee; $25,000 for spouse; $10,000 for each child). Employees not currently enrolled may elect coverage. If that is the case, the employee must complete an application and an “Evidence of Insurability” form, provided by the Office of Human Resources. How to re-enroll? Employees need to complete, date, and sign their applications during the re-enrollment period, showing the total amounts of coverage desired. Total amount is the current coverage amount plus the desired increase. When are coverages effective? Amounts at or below the non-medical maximum will begin at 12:01 a.m. on the first of the month coincident with or next following the date Unum approves and employee’s and/or family member’s Evidence of Insurability form. The Pre-existing Conditions Exclusion on the increased amounts will be effective at this time. Any amounts over the non-medical maximum will be medically underwritten. Therefore, the additional coverage will become effective when/if approved. What is the maximum coverage allowed? The maximum cover is up to 5 times the amount of your salary (rounded to the next $10,000) to a maximum of $500,000 for employee and spouse (with approval); $10,000 for child. If you have a child on the plan, he or she is covered up to age 19, or 25 if a full-time student. If you wish to add or increase the Lifestyle coverage, please call me at ext. 2103. Remember that the College provides a $10,000 life insurance policy to all .75FTE and full-time employees. The life insurance mentioned above is voluntary and is paid for by the employee through payroll deduction.

Employees who are currently enrolled do not need to complete new enrollment forms to keep current coverage amounts. The enrollment period will end on June 30, 2013.

The following information applies to the Lifestyle Protection Plan:

Why re-enroll?

Employees who are currently enrolled may increase

their coverage amounts without Evidence of Insurability up to the non-medical maximum ($50,000 for employee; $25,000 for spouse; $10,000 for each child).

Employees not currently enrolled may elect coverage. If that is the case, the employee must complete an application and an “Evidence of Insurability” form, provided by the Office of Human Resources.

How to re-enroll?

When are coverages effective?
