about access to care, quality of care, necessity of prenatal & well child care

– Each home visit integrates conversation about needed referrals for mom and baby

infant care, SIDS risk reduction, tobacco cessation– Safety Checklist for environmental risks/dangers – Assessments for child health, can access PHN for

any medical concerns to co-visit & assess– Assessments for parents include risk of

abuse/neglect
