Logo & Tagline

This is the over-arching brand, used to unify our centers under a common umbrella.

The CommuniCare Family of Companies logo is the core visual element used in effectively communicating our brand, both internally and to the public.

It must be used consistently and correctly in order to build awareness for our brand and reinforce its legal protection as our identity. The logo should also be applied to all materials in accordance with the following standards and guidelines to ensure maximum visibility and conformity to our overall brand.

Our logo consists of multiple elements, which should never be altered or changed in any way, unless specified within this document.

While we must remain steadfast in our use of the approved CommuniCare logo, we have developed an individualized version for each of our locations. Due to a broad range of application requirements, the logo is vulnerable to misuse.

Even the most well-intentioned treatment of our logo can be considered a misuse. In most cases, this occurs due to a lack of understanding about our standards. That is what we hope to correct and/or prevent with this document. To ensure our identity is not compromised, use only approved logo artwork and never alter, edit, re-create or distort the logo in any way.

What is the billing process?

Our Business Office mails out the bills on the 20th of every month. Payment must be paid in full by the 1st of the following month. Payment may be made by cash, check or credit card. If you have any problems or questions, please contact the Business Office Manager immediately to avoid any late fees. Our facility bills one month in advance at all times. Any refunds will be made within 30 days.

We offer free Wi-Fi to all of our residents. You will need to speak with someone in the Admissions Department to get set up. All laptops and electrical devices must be locked up at all times. Falling Water is not responsible for any lost items.

PersonAL cAre items

Upon admission, each resident will receive a bag with all toiletry items needed. Any personal care items that are brought in from home MUST be cleared through the nurse. Please be sure to label all other personal items (glasses, dentures, hearing aids, wheelchairs, walkers, etc.).

Please bring in enough clothing for 7 days. All clothing should be comfortable for the resident. Please make sure that ALL clothing is labeled with a permanent marker. Falling Water provides laundry service free of charge. Please be sure to let your nurse know if you would like us to provide this service or if the family will be taking the laundry home to be washed.

When are visiting hours?

8:00 am – 8:00 pm

hoW often is therapy?

Our therapists are here 7 days a week. Therapy will be given as often as your insurance authorizes. Please speak with someone from the Management Team to give you the specifics.

When are activities offered?

Falling Water has activities 7 days a week! Each resident has an activity calendar in his/her room upon admission. We like to keep all of our residents busy at all times. If your loved one is not one to participate, no need to worry. Our activities staff goes to each individual room to encourage the resident to take part.

are there any additional costs?

Everyone’s insurance is different. It is best to call your insurance company and find out exactly what they cover in order to avoid surprises. The Admissions Office will also verify your benefits prior to admission. They will be able to tell you if you have any co-pays or out of pocket costs.

Upon admission, there will be admission paperwork that needs to be signed. Please stop by or call the Admissions Office to set up a time to sign in. This must be completed with 24 hours of admission.

Medicare card, Managed Care card, secondary insurance card, Medicaid card and prescription cards. We will not be able to bill your insurance without copies of these cards. Please stop by the Admissions Office upon admission so copies can be made.

Power of Attorney / Living wiLL

Please give all documents to the Admissions Office upon admission to have copies made. The hospital will not transfer these documents. Without having these documents on file at Falling Water, we will be unable to follow the resident’s wishes.

Privacy notice

POLICy INtErPrEtAtION ANd IMPLEMENtAtION

upon admission to the facility, residents will be provided with a copy of our facility’s Privacy Notice relative to protected health information:

Privacy notices are provided in an easy to read language and contain, as a minimum, the following elements:

1. A statement indicating how medical information about the resident may be used and disclosed and how the resident can obtain access to such information

2. A description, including at least one example, of the types of uses and disclosures that the facility is permitted to make for purposes of treatment, payment, and healthcare operations, with sufficient detail to place an individual on notice of the uses and disclosures permitted or required

3. A description of each of the other purposes for which the facility is permitted or required to use or disclose PHI without the resident’s consent or authorization,

with sufficient detail to place an individual on notice of the uses and disclosures permitted or required;

4. A statement that other uses or disclosures will be made only with the resident’s written authorization, and that the authorization may be revoked in accordance with the policy on authorization;

5. A statement of the resident’s rights with respect to his/her PHI, and a brief description of how the resident may exercise those rights, including:

a. The right to request restrictions on certain uses/disclosures of PHI, and the fact that the facility does not have to agree to such restrictions

b. The right to receive confidential communications of PHI

c. The right to inspect and copy PHI

d. The right to amend PHI

e. The right to receive an accounting of disclosures of PHI and

f. The right to receive a paper copy of the privacy notice ;

6. A statement of the facility’s duties with respect to PHI, including statements:

a. That the facility is required by law to maintain the privacy of PHI and to provide residents with notice of its legal duties and privacy policies

b. That the facility is required to abide by the terms of its current effective privacy notice

c. That the facility reserves the right to change the terms of the notice and make the new notice provisions effective for all PHI maintained, along with a description of how the facility will provide residents with the revised notice;

7. A statement that residents may complain to the facility and to the Secretary of the U.S. Department of Health and Human

Services about privacy rights violations, including a brief statement about how a complaint may be filed and an assurance that the resident will not be retaliated against for filing a complaint (See policy entitled Privacy Complaints and Investigations.)

8. The name or title, and telephone number of the facility’s HIPAA Compliance Officer to contact for further information

9. The name, telephone number, and address of the person designated by our facility to receive complaints regarding our facility’s privacy practices

10. The effective date of the notice, which may not be earlier than the date printed or published.

A copy of Privacy Notices issued to residents will be retained for a period of not less than six (6) years from the later of the date of creation or the last effective date.

A copy of our Privacy Notice will be posted in the lobby and an electronic copy will be posted on our facility’s web page (as applicable). the HIPAA Compliance Officer is responsible for prompt distribution of changes to our privacy notice or policies and procedures.

Oral review of Privacy Notices will be made by the HIPAA Compliance Officer when such request is made by resident.

residents will be required to sign a written acknowledgement of receipt of our Privacy Notice. should a resident (or representative) refuse to sign the acknowledgement, a notation must be entered into the resident’s medical record of the facility’s effort to obtain the signature and the reason why the acknowledgement was not obtained.

WHAT IS PART A AND PART B?

Premium for Part A: If you have fewer than 30 quarters of coverage, you pay $441 or $485.10/mo. For 30-39 quarters of coverage, you pay $243 or $267.30/mo.

2. A benefit period starts the first day you receive a Medicare-covered service in a qualified hospital. It ends when you have been out of the hospital (or other facility that provides skilled nursing or rehab

services) for 60 days in a row. It also ends if you stay in a facility (other than a hospital) that provides skilled nursing or rehab services, but do not receive any skilled care there for 60 days in a row. If you enter a hospital again after 60 days, a new benefit period starts.

* After you pay $147 for covered services, the Part B deductible does not apply to any other covered services you receive for the rest of the year.

** Federal law limits charges for physician services. *** Part B pays for home health care only if you do not have Medicare Part A.**** The 3 pint blood deductible (donated or paid for) may be met by

1. PRIOR TO ADMISSION The process of admission to any facility

can be overwhelming. If you desire, one or more members of our friendly team of professionals will meet you at the hospital or your home to address any concerns you may have about your care or that of your loved one. We will also provide you with a list of items to bring to the facility.

2. ADMISSION Our Admissions Coordinator will sit with

you and/or your responsible party to fill out the required forms and discuss the payment options available to you. To aid in the process, we kindly ask that you bring copies of all of your insurance cards.

3. PLAN OF CARE During your stay, the interdisciplinary team

will meet to discuss the individualized plan of care for you or your loved one. You and/or your family are always invited to attend these meetings, which address both physical and emotional needs. Progress reports can be provided to personal physicians to ensure continuity of care.

4. DISCHARGE PLANNING Upon discharge, one of our professional social workers will assist you with a safe transition to your home environment.

Our facilities use an interdisciplinary team approach to address the physical and emotional needs of our residents. Following a thorough assessment, we develop a personalized wellness plan and guide you to success in all of your health care goals.

Berea Alzheimer’s Care Center is more than a hall or a wing, we are an entire building dedicated to the well-being of our exclusively Alzheimer’s/Dementia residents. Not only does our staff receive specialized Alzheimer’s/Dementia training, but our facility is fully equipped to provide quality care for our residents.

Our 50-bed home ensures that your loved one will not get overwhelmed in the bustle of a larger facility. We have a spacious wandering circle that allows residents a functional outlet during wandering stages. Our outdoor courtyard allows our residents to visit with their family in a secure environment.

Rehabilitation Services Our Physical and Occupational Therapists are trained in the unique rehabilitation challenges facing recent amputees, including wound healing, stump shaping and re-learning how to perform activities of daily living.

Orthotic and Prosthetic Services Our partner, Mid Atlantic Prosthetics, visits weekly to evaluate potential and current clients. A certified Prosthetist oversees the design and fit of each custom-made orthotic and prosthesis.

Mental Health Services To address adjustment issues, altered self-image, depression and the grieving process, as well as other issues that may surface during rehabilitation.

Dietitians To ensure dietary requirements are being met for optimal recovery.

Wound Care Specialist A collaborative approach with weekly evaluations by a Wound Care Physician, along with daily visits by a certified Wound Care Nurse ensure treatments are completed in a timely manner and recovery is streamlined.

Pain Management A Physician certified in pain management is available to address specific pain issues associated with recent limb loss.

Physiatrist A board-certified Physiatrist rounds weekly at our center and consults with attending physicians and rehabilitation staff on appropriate plans of care for our amputee patients.

Specially Trained Clinicians All of our nursing professionals, as well as our general staff, have been trained in the specific rehabilitation needs of our amputee population.

Fayette Health and Rehabilitation Center is a 174-bed facility located in the heart of Baltimore City, minutes from the Inner Harbor and within a 10-mile radius of seven major hospital centers. We are Medicare and Medicaid certified, and have contracts with many leading private insurance carriers. We care for individuals who may not be able to tolerate the intensive rehabilitation of an acute rehabilitation center, but would benefit from the specialized services of a dedicated inpatient amputee rehabilitation program.

Our Community

Billy had been living independently with his wife when a fall led to a brief hospital stay. From the hospital he was discharged to a nursing community where Billy’s wife was told to plan funeral arrangements because his prognosis was poor. Within two months of extensive therapy and dedication, Billy was able to return home to his wife, walking independently and completing all of his self-care with minimal verbal cuing. His wife describes his journey to recovery as one “from despair to hope. It was like seeing someone almost dead come back to life!”

David began to show significant signs of a decline and was no longer able to stand or ambulate safely. He subsequently began participating in physical and occupational therapy. David worked hard, and slowly began to show improvement in strength and balance. With this improvement, he began to express his desire to be able to ambulate independently, and this quickly became a new goal. The therapists trialed David for several weeks without his wheelchair, and upon his discharge, he was released and his wheelchair was removed. His wife, Judy, is overjoyed. Judy has stated how wonderful it is for David to be able to walk up and give her a hug when she comes to see him.

Stroke Recovery Program:The Stroke Recovery Program has been created to provide intensive rehabilitation and quality care with evidence-based practice to optimize outcomes for stroke survivors.

A stroke is a disease of blood vessels in the brain and can result from obstructed blood flow to the brain (ischemic stroke) or a ruptured vessel or bleed in the brain (hemorrhagic stroke). Suffering from a stroke is a life-altering event in which the rehabilitation process is of the utmost importance in recovery.

Our rehabilitation team is dedicated to finding workable solutions to the most difficult situations. This task is accomplished by approaching every patient and family with ingenuity, perseverance and creativity, while motivating survivors to relearn basic skills that a stroke takes away. Our Team provides strategies to survivors and their caregivers so that they can address and overcome stroke-related disabilities.

The overall goal of the rehabilitation process is to improve function so the stroke survivor can become as independent as possible. Preservation stands for preserving the resident’s current abilities and identity. Care is “person-centered” and special attention is given to ensure that the individual’s past is still a part of their present. The environment is home-like and familiar; stimulating, yet calm. And as a secured unit, it offers free mobility and sensory activity.

Purposeful activities will be provided daily by the Life Enrichment Coordinators. These include, arts, games, reminiscing, Montessori/tasks, sensory stimulation, food prep/programming and socials. Innovative activities vary day to day.

Valley Care Center is the Preservation,

Purpose, and Pace Program (PPP). It’s a

program that is ever changing to meet

the needs of our residents. The program

creates an “ebb and flow” to the day,

promoting times for physical and mental

stimulation and also downtime and rest.

Pace stands for the “flow” and routine of the day, the fusion of activities of daily living and daily leisure. Personal care and activities are intermeshed to fit the resident’s needs. The Memory Lane staff works as a team to ensure a rhythm that is comfortable and flexible. The activities change day to day, even hour to hour to ensure a flow that is most therapeutic.

Care Transition Manager Services

• Balance patient goals with healthcare coverage.

• Review and reconcile medication lists received in multiple settings.

• Assist with patient-physician communication.

• Communicate with primary care physician during transitions and schedule follow-up appointments.

• Provide patient education to increase literacy of the patient’s diagnosis and medication.

• Coordinate care between providers of home- and community-based services, such as Home Health, Durable Medical Equipment, Meals on Wheels and more.

• Schedule follow-up phone calls to support patient and family for 30 days after transition back home.

What is Transitions?Transitions by CommuniCare is an initiative whereby a dedicated nurse focuses on the coordination of care for patients who are undergoing short-stay recovery and rehabilitation in a skilled nursing center. A care transition manager (CTM) is dedicated to ensuring that his/her patient’s goals are best achieved to reach the highest level of independence.

How Does it Work?The CTM works with the Care Team to provide support, education, coaching, care management and care coordination of the patient as it relates to the transition of care from a hospital to a post-acute center, or from a post-acute center back home into a community-based setting.

Our main goal is to provide patient-centered, coordinated care for the patient and family. The CTM’s focus is on assisting with communication and coordination between physicians, service providers, care providers and families for healthy transitions between multiple settings.

Success StoriesWooster and his cowboy bootsWooster* decided he was ready to go home (wearing cowboy boots, making his gait unsteady) even though he had not tackled the stairs in physical therapy. Wooster was a retired marine and not used to having his plans altered. The staff did not want this patient to return home against medical advice, yet wanted to respect patient choice. The CTM intervened and reviewed Wooster’s goal with him: to be able to climb up six steps in order to enter his home. On reflection, he decided to stay until his personal goal of getting up six steps was reached, and even gave up his cowboy boots for more practical footwear! The CTM saw him later in the week, and he told her he had accomplished two steps and was looking forward to going home when he could complete all six steps.Transitions assisted Wooster in making a healthy decision, so that when he got home, he could stay home. Patty spikes a feverPatty transitioned home from a CommuniCare nursing center with a solid plan in place for home health service, completed medication reconciliation and a physician follow-up appointment made. The CTM made a call to the patient 24 hours after transition home, and the patient reported symptoms of chills and a fever. The problem was that her follow-up physician appointment was scheduled for one full week later, and the home health nurse would not visit her for another 24 hours. Patty was alone. The care transition manager contacted the doctor immediately to report Patty’s symptoms to him directly, and got Patty an appointment that same day!Thanks to Transitions, early intervention successfully helped Patty avoid an unnecessary trip to the emergency room, just one day after returning home.

Veronica got her two weeks of careVeronica’s insurance company planned a cut date for her care on Tuesday; however, her CTM was not comfortable with her returning home because her personal goals were not attained. The CTM felt that Veronica was not stable enough to transition back home. The CTM called the specialist and was unable to get an appointment with the specialist until that Thursday. The insurance company thereafter agreed to cover her stay until after the appointment. Once Veronica saw the specialist, it was determined that indeed she needed additional care, and her care was extended two additional weeks for a safe transition home! Thanks to Transitions, Veronica received the extra care she needed to get back home and stay home safely.

Beacon 360

Smoking Rewards

For those patients who enjoy smoking, Beacon 360 has designed a program that uses smoking to help patients make the right choices. The program also helps patients with cigarette

management.

The Beacon Shop

When patients exhibit positive behavior, they earn “Beacon Bucks,” pretend money that can be used to purchase items in

the Beacon Shop.

The Amenities Room

Patients may also use Beacon Bucks, to buy time in the Amenities Room, which may include exercise equipment, reading materials, a game console,

computer access and the like.

Special Field Trips

As patients show improvement in their ability to make healthy choices, they will earn the opportunity to participate in community events like theatre, bowling, picnics, dance parties etc.

Unit Graduation

It is our goal to give each patient his or her highest quality of life and independence. As patients continue to build their life skills toward functioning independently, they can graduate from Beacon 360.

Beacon 360 is a clinical program designed to assist those with

behavioral impairment in maintaining the highest quality of life possible,

Medical CareIt is not uncommon for patients with behavioral impairments to experience other medical conditions. In this case, it is necessary for patients to receive skilled medical care to enable them to live in a community setting.

Beacon 360 is designed to care for the complex medical needs that arise in conjunction with behavioral management challenges. It is our goal to restore the functionality of each patient to the most independent, least restrictive environment possible.

Medical Complexities and Skilled Nursing Needs

Beacon 360 is staffed with skilled nurses who tend to all patients’ complex medical conditions. From wound care and respiratory care to medication management, all patients can receive medical care that goes hand-in-hand with their behavioral care.

Advanced Specialty Hospitals Collateral

Pulmonary/Ventilator Program Our pulmonary/ventilator program provides a patient with respiratory failure the opportunity to be successfully weaned from ventilator dependence and achieve his or her highest functioning level. The extended, continuity of care provided by this program allows many of our patients to return home breathing on their own.

Medically Complex Program Our medically complex program provides integrated treatment for patients with more than one condition. Life support treatments and services include: telemetry, parenteral nutritional support, cardiac monitoring, respiratory monitoring and dialysis.

Complex Wound Programs Our complex wound programs provide specialized treatment for patients with pressure ulcers, serious wound complications, and those who are recovering from extensive surgery.

Low Tolerance Rehabilitation Program Our low tolerance rehabilitation program offers physical therapy, occupational therapy, and speech-language pathology geared to patients who are unable to participate in more than three hours of rehabilitation a day.

How to Make a Referral

Anyone can initiate a referral to Advanced

Specialty Hospitals of Toledo. Our clinical

liaison will complete an on-site screening

to determine the patient’s medical needs.

Admissions

Who is eligible to become a patient?

Advanced Specialty Hospitals of Toledo

accepts patients who are 18 years of age and

older with various medical conditions and

physical disabilities. Patients are typically

referred by a physician, nurse, case manager,

social worker, family member, physician’s

office, skilled nursing facility or home

healthcare agency and agent. In most cases,

patients come from a hospital-based

intensive care unit.

Upon approval for admission, a clinical

evaluation is conducted by an Advanced

Specialty Hospitals care team. Our services

are covered by Medicare, many health

maintenance organizations, insurance

companies and state-approved health

plans. Because there may be charges not

covered by insurance, it is best to check

individual policies for details on specific

benefits. Our team will secure insurance

pre-certifications and check hospital levels

of care benefits prior to admission.

LONG-TERM ACUTE CARE HOSPITAL (LTACH)

LTAC Hospitals fill an important role in the

continuum of patient care and address the needs

of a small, but growing, patient population who

cannot be effectively treated in conventional

healthcare settings. Patients are referred to the

Advanced Specialty Hospitals of Toledo for the

continuation of acute medical care because the

severity and complexity of their conditions

precludes them from being accepted into

a rehabilitation hospital or a skilled nursing

facility. From elderly persons with pre-existing

conditions that complicate their courses of

treatment to technology-dependent individuals

requiring ongoing intensive nursing care, every

patient at Advanced Specialty Hospitals of Toledo

is treated with dignity, respect and compassion.

About UsAdvanced Specialty Hospitals of Toledo is a 40-bed, Long-Term Acute Care Hospital (LTACH- pronounced L-tack) providing care for patients who are medically complex, stable and require extended hospitalization. We regularly admit patients from hospital Intensive Care Units, Step-Down Units, Cardiac Units and short-term acute care hospitals that are no longer appropriate or cost-effective. LTACH care differs from nursing homes and sub-acute care in many ways. Our patients are medically complex and require some or many of the following: daily physician visits, critical care ACLS trained nurses, respiratory therapists, pharmacists, dieticians, social workers, discharge planners, dialysis technicians, physical therapists, occupational therapists and speech-language pathologists. Regardless of needs, our unique, progressive LTACH environment is prepared to provide for any patient’s acute medical care in a professional, university-quality setting that maximizes recovery options.

A Statement FROM THE CEO

To All CommuniCare Employees and Affiliates:

Dear Friends;

Each one of us has a core of beliefs that makes us uniquely who we are.

This belief system is our guide for what is right and wrong. Through the

years we’ve discovered that we all have certain beliefs in common, for

example honesty, getting and giving respect, cooperation, etc. We all

want others to be honest with us, give us respect and treat us like the

valuable and important people we are. Unfortunately, in the heat of battle

it’s easy to forget that things we value with respect to how others behave

to us are exactly the same things that we must do to others. As a wiseman

1. Where can I find a usable copy of the logo? Please contact Studiothink at [email protected] or call 877.309.5534 and let us know which file format you need and what it is being used for.

2. What can the logo be used for? As a member of the CommuniCare Family of Companies, you can copy and paste the low-res logos only into your internal communications, such as handbills, flyers, etc.

3. Who is our print and promo vendor?Print collateral is handled by Studiothink and their print partner, and the promotional store is handled by Proforma company. You can access the print store and company store from the CommuniCare website at communicarehealth.com

4. Can I use logos from the website to print promotional or other materials?

No. These logos are low-res and will result in poor reproductions, especially with embroidering, silk screening or professional printing.

5.Does the tagline have to appear under the logo at all times?

No. In cases of tight space restrictions, the tagline is not required.

6. How do I order badges? Employee badges may be ordered at http://studiothink.net/communicare/orders/human-resources/. Simply input the requested information and follow the instructions for ordering and delivery. Name badges are produced at the corporate office. They can be reached at 800.989.7337.

7. How do I login to the print or company store? In order to login, you must be provided the appropriate credentials. Only human resources director, executive director and admissions director are allowed access to the print store. Login access can be requested by the facility’s ED or corporate office HR, by emailing [email protected]. Studiothink will need to know facility name, employee’s full name, title and email address. Login information will be then emailed to that employee directly.

8. Where do I order promotional items? Promotional materials may be purchased at http://www.chspromos.com/. Simply visit this website, input the requested information and follow the instructions for ordering and delivery. You can contact Proforma at 859.485.8895. You can also access this store from the CommuniCare website.

9. What is standard production time?Studiothink requests 7-10 working days to provide you with a proof pending appropriate materials are provided. Additional time will be added for print/production and shipping after approval.

10. Where do I order print collateral materials? You can access the print store from the CommuniCare website or type in http://studiothink.net/communicare/orders/marketing/. Simply visit this link and fill out the required project request form. Make sure to select the correct approver for your region (your regional marketing director - RMD). Your request will then be sent to your RMD for approval. Pending approval, the project will go into Studiothink’s production schedule. We request that you give us 7-10 working days to provide you with a proof. Upon file approval, additional time will be added for print/production and shipping. 14. Can I re-use old materials, if they do not reflect current brand changes?

No. Uniformity is crucial in CommuniCare’s developing and maintaining a strong brand identity. If we release materials that are outdated, we run the risk of alienating the progress we have made. If we do not remain consistent in our deliveries, we will end up sending mixed messages to our audiences. 15. What if I have additional needs, such as PR or social media assistance?

16. When sending or utilizing images, does the resolution really matter?

Yes. Think of resolution as a measure of quality. The higher the resolution, the higher the quality. In the case of most ads and flyers, a lower resolution would work just fine due to the size of the space. However, it’s better to be safe and send the highest resolution version available.

17. Does every CommuniCare employee have to adhere to these identity guidelines?
