A statistic survey was carried out on the patients， diseases， and operations experienced

at the urological ward of Kyoto University Hospital during the years of 1977 to 1982．

は じ め に

前回（1973年8月～1976年12月）の入院患者臨床統

計1）にひきつづきその後の6年間の臨床統計をおこな

ったのでその結果を報告する．

対象ならびに方法

1977年1月～1982年12月までの6年間に，京都大学

医学部附属病院泌尿器科に入院した患者2，392名に対

し，年齢別，性別頻度，疾患別頻度，主要疾患頻度，

入院期間中死亡者数，剖検例数，手術件数，手術内容

別頻度を入院台帳をもとにして調査した．

なお，年度にわたる患者は第1年度のみに算入し

た，

結 果

1）入院患者数

1977年～1982年の6年間の総患者数は2，392名で，

年間平均患者数は399名であった．前回報告の1974～

1976年の3年間の記聞平均患者数は339名で大幅な増

加を示しているが，1979年をピークとして1980年にや

や減少し，以後ほぼ“回している．

性別では，今回報告の2，392例中，男1，929例，女

463例で男女比は4． 17：1であった，前回報告では，

男女比は，3．14：1であり，男の入院頻度が増加して

いる．

Table 1は10歳年齢階級別，性別入院患者数を示

し，括弧内に年齢階級別頻度を比率で示してある．

Fig．1に5歳階級別，性別頻度を実数で示した．

2）疾患別頻度

疾患別頻度をTable 2～13に示し，腫瘍，結石，

奇形の主要疾患の占める比率をTable 14で示した．

なお，1患者で複数の疾患を有す場合，おのおのの項

に含めた．

腫瘍：（Table 2）

副腎腫瘍，神経芽細胞腫は別表に呈示した．今回報

告では神経芽細胞腫はなかった． Extragonadal

germ cell tumorは睾丸腫瘍の項に含めた．前回報

告に比し腫瘍例数の著増を認めた．とくに，腎細胞癌，

膀胱癌，前立腺癌，前立腺肥大症の増加が顕著である．

結石＝（Table 3）

前立腺結石，恥垢石はこの項に含めた．前回報告と

著変はない．

奇形：（Table 4）

た．腎嚢胞もこれまでの慣例上この項に含めた．

通過障害尿流停滞＝（Table 5）

尿管狭窄は他科領域の悪性腫蕩の浸潤あるいは術後

の停滞によるものである．膀胱憩室，神経因性膀胱も

この項に含めた．

循環障害＝（Table 6）

．腎動静脈疲が3例ありlAsk－UPmark．症候群も

1978年に1例あり，腎性高血圧の項に，含め灯．．

結核：（Table 7）

尿管，膀胱結核併発例は単に腎結核の項に含めた、

非特異性感染症＝（Table 8）

膀胱腫瘍との鑑別が困難で精査入院した慢性膀胱炎

の患者もこの項に含めた．

男性不妊症：（Table 9）

乏精子症は精系静脈瘤を合併したものが主である．

腎不全：（Table 10）

血液透析のため，入院したものである．nai1・patel－

la症候群は奇形の項にも含めた．

副腎および他の内分泌疾患：（Table 11）

外傷，異物：（Table 12）

その他＝（Table 13）

主要疾患頻度：（Tablc 14）

泌尿器科領域で頻度の高い尿路性器腫瘍，尿路結石，

尿路性奇形の占める比率を示した．腫瘍の占める割合

は例年40％内外であるが，1982年50％を超えているの

が目立つ．

3）死亡，剖検例（Table I5）

入院期間中死亡者数，剖検数，死因につながった原

疾患を，分母に死亡者数を，分子に剖検数をしるし

た．6年間の総死亡者数は114名で，・．そのうちフ4例に

剖検を施行し，剖検率は65％であった．1．．

4）手術（Table 16，17）

年間の手術件数をTable l6にしるした．ただし，

外来でおこなった小手術は含めていない．

手術の対象となった臓器の組み合わせで，術式を分

類し，内容別頻度として，．Tab！e工7に示した．

手術1件につき複数の術式を施行したものは各項に

含めた．ただし，尿路結石に対し尿管切石，腎孟切石

を同時におこなった場合や，腎孟・腎切石をあわせて

おこなった場合はその主なる術式を採用した．また，

広汎腎摘出術は腎摘の項に含めた．

つぎに，項目別に注釈を加える．

1．腎：腎切石術が1978年より頻度が高くなってい

る．これは田口法による腎実質一層縫合術を取り入れ

たことによる．Fistelonephrolithotomyは腎痩設置

患者で腎結石を併発した例で，腎縷孔より直接腎結石

を除去した症例である．自家腎移植は単腎の腎血管性

高血圧症例である．経皮的弓嚢胞穿劇術によるアルコ

ール注入療法は最：近の2年間に頻回におこなわれてお

り良い成績を残している．

VI．尿管・膀胱：逆流防止術は1976年以降急増して

おり前期はHutch法，最近はPolitanQ－Leadbetter，

Lich－Grcgoir， Glenn－Anderson法がおこなわれて

いる．

X皿．前立腺：前立腺肥大症に対する当科での手術は

この6年間で大幅な変化を示している．まず第1に，

恥骨上式から恥骨後式前立腺被膜下摘除術へと1979年

を境にして一変していること，第2に凍結手術が19フ9

年号ら激減していることである．かわって経尿道的前

立腺切除術が急増している．

経会陰式前立腺露出術は，前立腺癌に対する術中開

創照射術をおこなったものである．

XW．経尿道的手術：膀胱腫瘍に対する経尿道的手

術はTUR－Btが確実に増加しており，焼灼術のみは

1980年より急減しており，かわってレーザー光線によ

る焼灼が増えてきている．

XW．その他；腸管合併手術が増加してきている．

虫垂切除術のほとんどは，膀胱全接，回腸導管造設術

時に併行しておこなったものである．

結 語

1977年に報告した，）1973年8月～1976年12月の臨床

統計にならい，1977年1月～1982年12月にいたる6年

間の：京都大学医学部附属病院泌尿器科入院患者臨床統

計をおこなった。

6年閲の全症例2，392例を，年齢別，疾患別に表示

した．また主要疾患の比率，入院期間中死亡者数と剖

検数（剖検率），手術別頻度もあわせて調査した．
