Have a voice in your health care

(Sanford Tracy)End-of-life care: It’s a tough

topic for a lot of us. In fact, a recent study found many people know very little about it.

In the American Journal of Preventive Medicine, researchers looked at the results of a national health survey. Nearly 8,000 adults answered questions about their lifestyle and well-being. They were also asked about end-of-life care. Only 26 out of 100 people said they had any such plans in place. Almost one-third said they had no interest or knowledge about

the topic.But understanding your

choices and then making them known can ensure you get the care you want if you ever become seriously hurt.

“When it comes to your end-of-life care, make sure your rights are upheld,” says Angela Deadrick, LICSW, licensed independent clinical social worker at Sanford Tracy. “You have the right to make your own decisions about your health and life.”

End-of-life care is also known as advanced care planning. It starts with a legal document called an advance directive. This document has directions for your doctor, loved ones, and others about the type of care you want if

you can’t tell them yourself. Two key parts of an advance

directive are a living will and a durable power of attorney for health care.• Livingwill:Awritten

statement that details the type of care you want (or don't want) if you are unable to speak for yourself. A living will has no connection to the conventional will or living trust used to leave property at death. It is strictly a place to spell out your health care preferences.• Powerofattorneyfor

health care: A document that designates someone you trust to be your health care agent. This person can make any necessary health care decisions for you and ensure health care

providers give you the type of care you wish to receive.

Deadrick, who is also an advance care planning provider, adds that not only are you giving yourself peace of mind, you are giving a gift to your family by creating an advance directive. In Minnesota, providers will refer to the next of kin for health care decisions when an advance directive is not in place.

“These are very important decisions, and it can be quite stressful and burdensome for a child – or children,”

says Deadrick. “It can also cause disagreements between families, if no one has been designated to speak for you. If you have more than one child, all children will be asked to decide what is best, which can lead to problems and leave families struggling with guilt.”

But with an advance directive, these problems can be prevented, as your wishes are specifically spelled out.There are no unanswered questions for your family or friends to determine, leaving youandthemfeelingconfidentyour wishes are being met, no

matter what happens.“We all plan for major life

events such as weddings and babies – and end-of-life care should be considered the same way,” Deadrick says. “Don’t leave this very important life event to chance. Make sure your wishes are known with an advance directive.”

Advance directives are important

(Sanford Tracy)If you are 18 or older, you

can – and should – complete an advance directive. You never know when you may have a medical emergency, and having a plan in place can alleviate a lot of unnecessary stress for you and your loved ones.

An advance directive doesn’t need to be long and complicated. It simply needs to explain your wishes. It also doesn’t have to be a formal legal document. Most providers will honor patient decisions even if they are handwritten.

“Writing down some of your thoughts and desires is a good place to start,” says Angela Deadrick, LICSW, advance care planning provider at Sanford Tracy. “And simply talking with your loved ones about some general points can get the conversation started.”

When considering end-of-life care, keep these points in mind:• Make sure your

advance directive follows your state’s laws on end-of-life care. Some states call for such forms

or papers to be notarized.• Complete your

advance directive when you are healthy and of sound mind. During a health crisis or other stressful situation is not the time to make these decisions.• Give a copy to your

doctor, family members, and any others who you think should know your wishes. • Carefully choose

your health care proxy. Talk with that person about your decision. Make sure he or she agrees to the role. Your spouse or a close relative may not always be the best choice. He or she may be too clouded by grief if you are close to death.• Reviewyouradvance

directive from time to time. You can update it as needed if you change your mind. Be sure to give a revised copy to everyone who received the previous version.

“It’s important to review your advance directive to ensure it represents your current viewpoint and situation,” Angela recommends. “As we age or go through life events, our

opinions and values may change.”

Lynn says a good rule of thumbs is to follow the “5 Ds” for re-evaluating your advance directive: 1. Death of family or friend

2. Divorce3. New diagnosis4. Decline in health5. Every decadeMost importantly, know

that you do not have to begin this process alone. Sanford Tracy has two advance care planning providers available to answer any questions or help you complete your advance directive.

“We are here to help you begin the process, give you tips on talking with your family or understanding all the different decisions that need to be made regarding your advance directive,” Deadrick says. “We understand how difficult itcan be to think about end-of-life care, but we also know howbeneficialandcomfortingit can be for your own peace of mind as well as those you love.”
