To all Incoming Freshies:

Congratulations and welcome to the University of the Philippines VisayasTacloban College (UPVTC)! Inasmuch as you now have the privilege of studyingat UPVTC, you need to accomplish the following important instructions:

PRIORITY THINGS YOU NEED TO DO1 Confirm your intention to enroll at UPVTC for the First

Semester, AY 2019-2020 starting May 3 to May 24, 2019.2 Prepare and submit all the documents required for

enrollment on or before July 10, 2019.3 Attend the Freshies orientation.4 Follow the enrollment procedure.

Stated below are the specific steps and reminder that you need to accomplish:

STEP I. CONFIRMATION OF YOUR DECISION TOENROLL AT UPV TACLOBAN COLLEGE

Confirm your decision to enroll at UPVTC both at the Office of the CollegeSecretary (OCS), UPV Tacloban College and ONLINE at http://111.125.83.65or crs.upv.edu.ph . Your confirmation should be done in the following manner:

A. Confirmation at the UPVTC Office of the College Secretary

There are four (4) ways to confirm your decision to enroll at UPVTC. This procedure starts with the accomplishment of the attached Confirmation/ Reply Slip (UPV Form C1) which may be submitted in person, by courier, by fax, or through e-mail.

1. If in person: Visit the Office of the Office of the College Secretary andsubmit the accomplished UPV Form C1.

2. If by courier: Send by courier (e.g. LBC, JRS) the accomplished UPVForm C1 to:

3. If by fax: Fax the accomplished UPV Form C1 to the Office of theCollege Secretary through fax number (053) 832-2897

4. If through e-mail: Send the accomplished UPV Form C1 to the officiale-mail address of the Office of the College Secretary at [email protected]

B. Confirmation Online (schedule to be announced)

After you have submitted the Confirmation/Reply Slip to the Office of the College Secretary, you must also confirm your decision to enroll online. Follow the steps indicated below:

1. Locate your Student Number and PIN in your Notice of Admission.

2. Access the UPV-CRSIS online Confirmation System by using the URLhttp://111.125.83.65 or crs.upv.edu.ph and look for theFreshman Login link on the Registrar’s Corner at the upper left ofthe homepage.

3. Log-in by using your Student Number as your username and your PINas your password. (If you encounter problems logging in to the CRSIS,please contact the Office of the College Secretary through e-mail [email protected])

4. Carefully follow the instructions found in the website.

STEP II. PREPARATION FOR ENROLLMENT/SUBMISSION OF REQUIRED DOCUMENTS

1. Physical-Medical and Dental Examination

a. Accomplish the following laboratory examinations: chest x-ray, stoolexam, urinalysis, CBC, blood typing and hepatitis Bscreening. You may have your laboratory exams taken at anydiagnostic center or hospital of your choice. (A laboratory request fromthe UPVTC physician is attached to this notice).

b. For those with dental problems, have yourselves checked by yourdentist for proper treatment and management prior to your dentalexamination at UPVTC.

c. Prepare the following requirements to be submitted to the UPVTCClinic:

1) original and photocopies of your laboratory results2) one (1) long size, white, ordinary folder3) one (1) 2x2 ID picture4) one (1) photocopy of your Notice of Admission

d. Proceed to the UPVTC clinic for your Physical-Medical and Dentalexamination. The UPVTC Clinic is located beside the Office of StudentAffairs (OSA). Medical and dental examination will be held from May02 to June 28, 2019 (except May 10, May 23, May 30, June 19 andJune 20, 2019) at 8:30 to 11:30 a.m.

d.1. Fill up the Physical-Medical/Dental Forms (These forms are available at the Clinic).

d.2. Submit your laboratory results and other requirements to the the UPVTC Physician.

e. A Medical Certificate will be issued by the UPVTC Physician afterexamination which will qualify you for enrollment. This certificateshould be submitted to the Office of the College Secretary togetherwith your credentials.

2. Submit the following documents to the UPVTC Office of StudentAffairs (OSA):

a. One (1) 2x2 ID pictureb. One (1) photocopy of your PSA Certificate of Live Birthc. One (1) long size, red expanding enveloped. One (1) photocopy of your Notice of Admissione. Ten (10) sheets of short bond paperf. Certificate of Good Moral Characterg. Accomplished Background Information Sheet. This form should be

obtained from the OSA.

NOTE:

A guidance certification will be issued by the Guidance Counselorafter you have submitted the said requirements to the OSA and thisguidance certification will be submitted to the Office of the CollegeSecretary (OCS) together with your entrance credentials.

3. Submit the following Entrance Credentials to theOffice of the College Secretary on or before July 10, 2019 :

a. Original and two (2) clear photocopies of your Notice of Admission.

b. Two (2) identical 2x2 ID pictures (with plain background) with yourname and signature at the back.

c. Original copy and two (2) clear copies of your Senior High SchoolCard (Form 138) duly signed by your Principal and bearing thestatement: “Graduated and eligible for admission to College”.

d. Original and two (2) clear copies of your Junior and Senior HighSchool Permanent Record (Form 137-A from Grade 7 to Grade 12)duly signed by the Principal with the remarks “Copy for UP VisayasTacloban College” and “Graduated and eligible for admissionto College.”

Note: Form 137-A with incorrect remarks will not be accepted. Pleasevisit or email the UPVTC Office of the College Secretary for theissuance of letter-request for Form 137-A.

e. Original copy and two (2) clear copies of your PSA Certificate ofLive Birth.

f. Accomplished Student Directory. This form may be obtained fromthe Office of the College Secretary.

g. Medical Certificate issued by the UPVTC Clinic.

h. Guidance Office Certification.

i. Obtain your Admission Slip from the Office of the College Secretaryafter you have submitted the aforementioned documents. Failure tosubmit the required credentials and documents may result to thecancellation of your slot.

Note: Please keep your Admission Slip for enrollment purposes.

4. Your UPV-CRSIS Personal Account

A database of all UPV students is maintained by the UPVComputerized Registration and Student Information System (UPV-CRSIS).Update your Log-in Data and Personal Information at the said system.

Access the UPV-CRSIS by using the following URL:http://crs.upv.edu.ph or http://1 11.125.83.65 . In doing so, observe the followingprocedures:

a. Click the “STUDENT” on the UPV-CRSIS homepage. This button islocated on the top right portion of your monitor.

b. On the space “STUDENT ID” enter your student number.

c. On the space “PASSWORD” enter your PIN then click the “LOG IN”button. If the computer asks you if you want it to remember yourpassword, click “NEVER ON THIS SITE”. (If you encounter problemslogging in to your CRSIS account, please contact the Office of theCollege Secretary through e-mail at [email protected])

d. Please read the UP Data Privacy Notice and then click the “I Agree”button in order to proceed.

e. Change your password once you have accessed your personal account.Click the “EDIT MY LOG-IN DATA” button. On the box opposite“PASSWORD” delete the original entry and enter your new password.The password may be a combination of letters and numbers. Choose apassword that you can easily remember. Note that the system is case-sensitive when it checks your password. Hence, remember exactly howyou typed your password noting whether characters have been typedin capital or small letters. It is however recommended that you useonly lower-case (or small) letters. Re-type your new password in thebox that immediately follows it. When finished, click the “UPDATE”button.

f. Remember: keep this password only to yourself and do not under anycircumstance share it with anyone.

g. Click the “EDIT MY PERSONAL DATA” button. Supply all therequired information in the window that is displayed paying particularattention to boxes marked with red asterisks. When finished, click the“UPDATE” button. This step is necessary in order for the Office of theCollege Secretary to print your Student Directory.

h. After completing the process, “ LOGOUT” from the UPV-CRSIS byclicking the “LOGOUT” button found on the upper right portion ofyour monitor. Do not leave the computer unit without loggingout of the system especially if you have accessed the UPV-CRSIS at aninternet café.

STEP III. FRESHIES ORIENTATION

All first year students with their parents are required to attend the Freshies Orientation scheduled on August 2, 2019 at UPV Tacloban College (specific venue: to be announced). The orientation provides vital information to guide incoming first year students on the challenges and opportunities for growth in their college life at UPV Tacloban College.

STEP IV. REGISTRATION FOR INCOMING FIRSTYEAR STUDENTS ON JULY 29, 2019

Please come in person and bring your Admission Slip. Follow the enrollmentprocedure that will be posted at the Office of the College Secretary and bulletin boards.

Please be informed that the UP Visayas shall implement the Universal Access to Quality Tertiary Education Act (RA 10931) and its Implementing Rules and Regulations.

a. Return Service – those who opt to avail of the free tuition shallrender a return service. The guidelines for the return service system will be posted at the crs.upv.edu.ph, as soon as it is available.

b. Opt-out – as part of the Implementing Rule and Regulations of the RA 10931, a student may voluntarily forego for Free Tuition in UP Visayas Tacloban prior to enrollment and instead, pay the full tuition and miscellaneous fees. The Opt-out forms can be downloaded at the crs.upv.edu.ph, as soon as it is available.

V. OPENING OF CLASSES

The Opening Exercises and Start of Classes is on August 05, 2019.
