Indigenous Outreach Program Improving access to primary care

As the Indigenous Outreach Worker for the Gold Coast, my main focus is improving Aboriginal and Torres Strait Islander community access to mainstream primary care services. This is achieved by working with general practitioners, practice nurses, practice staff, Aboriginal Community Controlled Services and directly with the Aboriginal and Torres Strait Islander community. Types of activities included in my role:

• Support at, and transport to and from, appointments for clients as required.

• Ensuring access to medications through Close The Gap annotated prescriptions, promote use of Webster packs, and medication delivery as required.

• Identification of barriers experienced by the local Aboriginal and Torres Strait Island community and facilitation of solutions with relevant service providers to increase access to primary health care.

• Encourage and support community member’s to self-identify with primary health services.

• Promote existence of local Indigenous services available and facilitate the referral process (Mungulli, Indigenous Heart and Eye Health Clinics).

• To network with the Aboriginal and Torres Strait Hospital Liaison Officers at Gold Coast hospitals and Care Coordinator for CCSS Care Coordination and Supplementary Services.

Wow … one quarter of the way through the year already. It seems only a few days ago we began writing 2016 on our communications.

Easter signals the beginning of the flu vaccine season. Government supplies will roll out next month, and I know it is a hot topic for all sorts of reasons. We also know that pharmacists will again be offering vaccination; a common reason given for not attending General Practice for this purpose is to do with waiting times. Perhaps this is an opportunity to review fast track options for these patients?

The next networking event is scheduled for May. Look for information on this in the GP Bulletin and Generally Speaking (May editions). This format has been popular with the first event held in Mudgeeraba in the third week of March. GPs who have not attended meetings for some time are rediscovering old acquaintances. And new GPs and Registrars have appreciated the opportunity to meet outside the very clinical environment we all live in. We have extended the invitation to partners, often left at home while the GP half of a couple goes out.

I recently attended a meeting in Canberra with the Department of Health and with other Primary Health Networks (PHNs) to discuss new program directions including the mental health and suicide prevention reforms, including the requirement to commission alcohol and other drug treatment services. Gold Coast Primary Health Network’s (GCPHN) role will be to work with the sector and with the Gold Coast Hospital and Health Service (GCHHS) to develop a regional plan to incorporate these areas.

Our aim will be to commission services to address gaps and to better coordinate these for efficiency and develop pathways to assist GPs to navigate services for their patients. Our interim needs analysis was considered by the Board at the March Board meeting. Priorities for 2016/17 will be to commission youth, suicide prevention and Mental Health Nurse Incentive Program services based on 2015/16 funding levels. In relation to alcohol and other drug services, we are still waiting on further advice from the Department, however it is planned that we will be commissioning additional treatment services during 2016/17 with a focus on the Indigenous community.

A Regional Mental Health and Alcohol and Other Drug (AOD) Steering Committee consisting of GPs, acute public and private services, community mental health and AOD services and emergency response services will inform the development of the regional plan and new services.

The relaunch of the My Health Record was also discussed. GCPHN has developed a strategy to support GPs in meeting the new requirements of the digital health PIP and you will receive more information on this shortly to enable practices to meet these requirements.

GCPHN Clinical Council Update

GCPHN Primary Care Opinion Survey The Gold Coast Primary Care Opinion Survey is currently being collated, with 240 responses from health professionals across the Gold Coast. We will be providing details of the survey in future editions. GPs, practice managers and practice nurses were asked about their satisfaction with Generally Speaking. Some of their findings included:

• GP satisfaction: satisfied/very satisfied - 75%• Practice nurse satisfaction: satisfied/very satisfied - 85% • Practice manager satisfaction: satisfied/very satisfied - 91%

Main reasons why respondents were satisfied:• Informative, relevant, useful, concise• Professionally produced, timely• Covers important information• Information is local and relevant for General Practice

The main areas respondents enjoyed were:• Links to read further information• Seminars, education and training events• Good array of topics relevant to the whole practice, practice

tips, service updates• Industry insights• Important announcements, changes in legislation, practice

incentives, what’s happening in health• Job vacancies• Disease outbreaks and notifications

Suggestions about improvements and future articles: • Latest information on referral guidelines• Practice support topics• Monthly tips in specialty areas eg mental health, chronic

disease etc• Showcase GCPHN website• New specialists on the Coast• Specific health topics• Updates from hospital departments (services provided,

The Gold Coast Primary Health Network (GCPHN) Clinical Council held a discussion around discharge summaries and improving communication between GPs and the GC Hospital and Health Service (HHS) and it has been identified as a priority of the HHS board and the General Practice Liaison Unit. Most recent improvements were noted to the completion rate of discharge summaries (>90% in February 16) and strategies discussed to improve these further. Recruitment is currently being finalised for the General Practice Liaison Officer, and once appointed, it is envisaged that further action will be taken. It was noted that it was a commonly identified issue in the initial results in the Gold Coast Primary Care Opinion Survey.

There was a general discussion regarding launch of My Health record and the requirements for GPs to upload patient records. The Clinical Council discussed the level of support that will be required to assist GPs and practice staff. The potential duplication of the My Health Record and My Aged Care Gateway were discussed with a proposed solution to ensure that these systems can be linked to each other.

In relation to the Gold Coast Primary Health Network draft strategic plan, Clinical Council members felt some areas are still needing further clarification in terms of the five year goals and more specific targets and measures. The Clinical Council has recommended that reference be made to the involvement of allied health in supporting coordinated patient management.

As part of the discussion around end of life planning, some issues regarding current palliative care arrangements were discussed and raised. It was suggested an education event/forum be held for all healthcare professionals involved in palliative services across the Gold Coast.

A Clinical Advisory Committee is being established to provide guidance and advice on the risk stratification pilot and Dr Peter Meulman was nominated as the Clinical Council representative.

GPs can refer older residents with a severe and enduring mental illness for assessment as part of the CNAP 65+ panel, which brings government, community and private organisations together to create pathways to support older people with complex needs and mental illnesses, where all past and current services have identified unmet needs. CNAP aims to:• Assist coordination of appropriate care and services• Support older people and their families to remain independent in

the community

How Does the Panel Work?The referred older person must consent to their information being presented to the panel. At the panel, service providers will meet to share resources and information about services and make suggestions that could assist the older person. After the panel meeting, the referring service provider gives feedback to the older person and their family about options available to them. The older person and the family maintain control of any recommendations or options that are suggested. Referral CriteriaThe older person being referred to the panel will meet a combination of the following criteria:• Be aged 65 years or older with severe and enduring mental illness

requiring intensive case management• Live within the Gold Coast area• Have a current care worker providing care coordination or case

management• Consent to the process and information being shared• Have multiple support needs that cannot be met by current services• Have significant impairment in social functioning and require

ongoing support to maintain/enhance their independence in the community

• Have limited or no family and social support networks• Co-morbidities ie. substance use and/or physical/cognitive decline• Is more likely to disengage from support services.

This Active Learning Module has been approved by the RACGP QI&CPD Program - 40 Category 1 points. 6 CPD hours through Australian Practice Nurses Association. It features:

• Comprehensive, six-hour Active Learning Module• A greater understanding of risk assessment, suicide prevention intervention strategies and patient support and management• Demonstrates a strategy to provide assistance to a patient who may be considering suicide, understanding the time constraints of general practice consultations

Skills training for Practice Managers and Practice Staff Friday May 13 2016, 6pm-9pm

As part of practice accreditation staff are required to be able to triage patients. This workshop enables development of a strategy or protocol to meet the needs of general practices, with additional support provided by the identification of local organisations to assist practices, staff and patients. It features:

• Two-hour tailored seminar• Build awareness and skill staff to recognise ‘at risk’ signs, and to respond appropriately.

Access to Allied Psychological Services (ATAPS) Programs for Older PeopleIt is thought that between 10 and 15 per cent of older people experience depression and approximately 10 per cent experience anxiety. Rates of depression among people living in residential aged-care facilities are believed to be much higher, at around 35 per cent. Unfortunately, many people over 65 still seem to feel there is a stigma attached to depression and mental health conditions, viewing them as a weakness of character rather than a health problem. Older people are also more hesitant to share their experiences of depression with others, often ignoring symptoms over long periods of time and only seeking professional help when things reach crisis point.

Older persons living in the community, who could benefit from short term psychological intervention, are financially disadvantaged and not residing in a Commonwealth Funded Aged

Care facility may be eligible to access psychological services through the ATAPS Program. A standard ATAPS referral provides up to 12 individual counselling sessions per calendar year with an accredited ATAPS mental health provider.

The Access to Allied Psychological Services (ATAPS) Program is a Commonwealth Government funded Program providing services for all age groups, including children under 12, youth, adults and the elderly.

Dealing with Difficult Patients An appropriate approach towards difficult patients is by using positive language to deescalate the situation. This gives the patient no opportunity to overreact. For instance, "It seems you have a different viewpoint on this situation. Let me explain our position." This statement allows the patient to be heard and allows you to state your side of the argument.

Queensland seniors staying on their feet This month is a timely reminder for GPs and patients about a wide range of useful resources available on the Queensland Health website for patients and health professionals, to help seniors to stay on their feet and avoid a fall. Statistically, every day, 133 older Queenslanders have a fall requiring medical attention, even though falls are mostly preventable. Managing depression: the optionsNPS MedicineWise is inviting practices to participate in an educational visit to look at selecting the most appropriate management approaches for patients diagnosed with depression including non-pharmacological options. An NPS MedicineWise educational visit can help GPs find out about the place in therapy for supportive approaches, psychological treatments and antidepressants, along with the latest information about duration of adequate treatment trials. Two types of educational visits are available at no charge. Both are facilitated by NPS MedicineWise’s expert Clinical Services Specialists.

• For GPs – one-on-one tailored discussion (30 minutes duration)

• Small group case-based meeting for up to 10 health professionals – GPs, practice nurses and pharmacists (1 hour duration)

Immunisation Catch Up TipsReceptionists • All immunisation appointments are screened and booked as

either routine or catch up appointments• Catch up appointments should be allocated a longer

appointment and not all required vaccines may be available. • Request patients to bring all vaccination records to the
