Epidemiological week no. 37 (9 - 15 September, 2011)

• 77 districts and 3 agencies provided surveillance data to the DEWS this week from 2,480 health facilities.

• A total of 910,590 consultations were reported through DEWS of which 19% were acute respiratory infections (ARI), 12% Skin disease, 12% acute diarrhoea, and 7% suspected Malaria.

• Over all diarrhoea has increased by 2% as compared to the previous few weeks.

• A total of 181 alerts with 42 outbreaks were reported in week-37, 2011: Altogether 71 alerts were for DHF; 65 were for AWD, 14 for NNT and Tetanus, 12 for Measles, 4 for Pertussis, 3 each were for bloody diarrhoea and Leishmaniasis; 2 each for acute diarrhoea and suspected malaria, while 1 each for CCHF, Meningitis, Scabies, Mumps and Chicken pox.

• National Polio Eradication Initiative reported 5 new type-1 confirmed polio cases this week: These include 3 from Balochistan (one each from districts Quetta Loralai and Pishin); 2 from Sindh (Hyderabad and Umerkot). As of 15th September, 2011, Pakistan has reported a total of 89 polio cases (88 type-1 cases & 1 type-3 case) from 34 districts/towns/tribal agencies/areas.

Since July 29, 2010, approximately 44,565,363 patient consultations have been reported to the DEWS from five provinces and three agencies and state of AJK. In this week 37, 2011 (9 to 15 September, 2011) reports were received from 2,480 health centers and the major causes for seeking health care by the communities in almost all of the provinces were diarrheal diseases 106,771 (12%), acute respiratory infections 170,530 (19%), Skin diseases 106,109 (12%) and suspected malaria 64,093 (7%). The above graph shows the trend of acute diarrhoea cases out of total consultations reported to DEWS in 2009, 2010 and 2011 up until now.

During the week 6 cases of DF with 2 deaths were reported from 5 different locations. The patient presented with fever, headaches and body aches. Lab investigations revealed thrombocytopenia and leucopenia. 5 blood samples were taken and all found positive for IgM Dengue. The patient was referred to BBS Hospital Rawalpindi but died on 15 Sep. Epidemiological and environmental assessments were carried out in the area. A session on DF spread and prevention was conducted for the affected household. Health session was carried out during Jumma Prayers in the area. Fogging was done and bed nets were provided by EDO in the area.


Lower Dir Abbottabad DHQ (Township, plot #6, Lahore)

Mansehra Village Timber Near Phulra

Patients were shifted to Dengue ward, LLINS applied around the bed to prevent spread of infection. WHO team along with CDCO and entomologist visit the area, IRS plus insecticide spray of the household and in the vicinity carried out. Dengue Prevention session was given to the community. EDO Health was informed and requested to update the provincial CDC.

Alert for DF, 10 cases were reported from Nishter hospital. The cases were shifted to isolation ward. The details and the line list were shared with the EDOH Multan. WHO SO along with Health department team visited the areas and conducted house to house survey and found no more cases. two dengue awareness sessions were conducted in the feild and distributed IEC material regarding dengue prevention. Fogging of the whole union council is done by a CDC team from the health depart‐ment.

14‐Sep AWD Sindh Naushahro Feroze

Nearest health facility was informed of the suspected cases and staff were briefed on case definition and early referral of the cases. EDO Health and Malaria control program officer was informed. Province Khyber Pakhtunkhwa:

• This week 18 districts reported to DEWS from 651 health facilities with a total of 156,196 patients con-sultations.

• The proportional morbidity of AD showing decline during the last two weeks after a consistent higher proportion (16%) for last almost 10 weeks.

• 40 alerts were reported in this week, 21 were for DHF, 11 for AWD, 7 for Measles, while one for neo-natal Tetanus.

Province Sindh:

• This week 18 districts reported to DEWS from 707 health centers with a total of 383,488 patient consul-tations during the reporting period of week 37, 2011.

• In Sindh, AD cases reported were 47,396 (12%), 1% higher proportional morbidity as compared with last week.

• 62 alerts were reported from Sindh in this week: 39 were for AWD; 8 for NNT and Tetanus; 7 for DHF; 3 for Measles; while one each for BD, Leishmaniasis, Malaria, Meningitis and Mumps.

ince, 451 health facilities reported a total of 213,482 patient consultations during this week.

• AD cases reported were 17,632 and stabilized be-tween 7% - 8% from last four weeks.

• 61 alerts were reported in this week from Punjab; 41 were for DHF; 12 for AWD; 3 for Neonatal Tetanus; 2 each were for acute diarrhoea and bloody diar-rhoea; while one was for Pertussis.

State of Azad Jammu and Kashmir:

• In this week, weekly report received from 111 health centers from 10 districts, with a total of 39,885 patient consultations.

• ARI cases reported were 5,548 (14%), while Acute diarrhoea re-ported 2,608 (7%) cases.

• 3 alerts were received and responded this week; One each for AWD, DHF and Measles.

• This week 52 health centers in 3 agencies reported from FATA, with a total of 10,745 patient consultations.

• ARI cases reported were 1,911 (18%), while Acute diarrhoea re-ported 1,128 cases (11%)

• Two alerts for Neonatal Tetanus was received and responded this week.

Gilgit Baltistan:

• In this week, 79 health centers from 7 districts in Gilgit Baltistan sent weekly report with a total of 29,065 patient consultations.

• ARI cases reported were 4,739 (16%), while Acute Diarrhoea 8,778 (30%).

• 4 alerts received and responded this week, one each for AWD, Measles, DHF, Pertussis.

FATA:

Province Balochistan:

• In this week, 16 districts reported to DEWS from Balochistan province. 429 health centers reported a total of 77,729 patient consultations.

• AD reported in 8,371 (12%) of the total consulta-tions, same proportional morbidity as compared with last week.

• 9 alerts were reported in this week from Balochistan, 2 for Leishmaniasis; while one each for AWD, Ma-laria, CCHF, Scabies, Pertussis, Mumps and Chicken pox.

Distribution of Wild Polio Virus cases Pakistan 2010 and 2011

Year 2010: The total number of polio cases reported in 2010 is 144 includ‐ing 120 type‐1 cases and 24 type‐3 from 40 infected districts/towns/agencies. Year 2011: The total number of polio cases confirmed by the laboratory is 89 (88=type‐1, and 1=type3) till date from 34 districts.

Focus on: Dengue Fever

What is Dengue? Dengue is the most widespread mosquito‐borne infection in human beings, which in recent years has become a major international public health con‐cern. Over the last 15 years, we have witnessed a dramatic increase in the global incidence of dengue and its severe manifestations such as dengue hemorrhagic fever. Dengue fever is a severe, flu‐like illness with fever, headache and muscle and joint pains that affects children and adults, but seldom causes death. Dengue hemorrhagic fever (DHF) is a potentially deadly complication in about 1‐3% of cases that begins with a sudden rise in temperature which usually continues for two to seven days and can be as high as 40‐41°C. In moderate DHF cases, all signs and symptoms abate after the fever subsides. In severe cases, the patient's condition may suddenly deteriorate after a few days of fever when the temperature drops, followed by signs of circulatory failure. With modern intensive supportive therapy, the fatality rates can be reduced to less than 1%. How Dengue spreads? Dengue spreads through the bite of infected Aedes Aegypti mosquito. The mosquito gets the virus by biting infected person. The symptoms appear of the disease about 5‐7 days after the infected bite. There is no way to tell if a mosquito is carrying the Dengue virus. Therefore, people must protect them‐selves from all mosquito bites. How can we prevent Dengue? Prevent mosquito bites

• Dengue mosquitoes bite during the daytime, protect yourself from the bite.

• Wear full‐sleeve clothes and long dresses to cover the limbs. Avoid wearing black color.

• Use repellents. Care should be taken in using repellents on very young children or the elderly

• Use mosquito coils and electric vapor mats during the daytime

• Use insecticide treated nets (ITNs) to protect young children, pregnant women, old people, in addition to others who may rest during thy day.

• Curtains (cloth or bamboo) can also be treated with insecticide and hung at windows or doorways, to repel or kill mosquitoes.

Where do mosquitoes live and breed? Dengue mosquitoes breed in stored, exposed, water collection systems. The favoured breeding places are: barrels, drums, jars, pots, buckets, flower vases, plant saucers, tanks, discarded bottles/tins, tyres, or water coolers, and other places where rainwater collects or is stored.

• Drain out the water from window air coolers when not in use, as well as draining all unused tanks, barrels, drums, and buckets.

• Remove all objects containing water such as plant saucers from the house.

• All stored water containers should be kept covered at all times.

• Collect and destroy discarded containers in which water collects, such as bottles, plastic bags, tins, tyres, etc.

• Efficient disposal of all solid waste/garbage. How should we take care of a Dengue patient?

• Keep body temperature below 39oC. Give the patient paracetamol (not more than four times in 24 hours). Fever can also be reduced by sponging patient with tepid water.

• Do not give the patient Aspirin or Ibuprofen (Brufen) or similar medi‐cine for fever or pain. Give only paracetamol.

• Give large amounts of fluids (water, soups, milk and juices) along with the patient’s normal diet

• The patient should rest

• Immediately consult your physician if any of the following manifesta‐tions appear: Red spots or points on the skin; bleeding from the nose or gums; frequent vomiting; vomiting with blood; black stools; sleepi‐ness; constant crying; abdominal pain; excessive thirst (dry mouth); pale, cold or clammy skin; or difficulty in breathing.
