As we approach the end of the school year, it is an important to address critical issues that can provide a proactive approach for the upcoming school year. PBIS/Culture/Climate teams can use this action planning
template to target and address school-wide, classroom and individual student behavior management support.

Teach/Pre-Correct expectations and routines for end of school year
Review and reinforce expected behaviors
Arranged events to celebrate successes of all students and staff

Review and evaluate office referral and/or discipline data for year
Revise/evaluate PBIS/Tiered Fidelity Inventory (TFI) action plans for year
Prepare proactive transition plans for at-risk (high risk) students
