COURSE DESCRIPTION Emory’s Ski and Learn Dental Seminar will provide timely and practical information that you can take back to your office for immediate application. REGISTRATION DATES AND TIME Saturday, 1/18/2014 – 7:00am – 9:00am Saturday, 1/18/2014 – 3:00pm – 5:00pm Sunday, 1/19/2014 – 6:30am – 7:00am (Course material and name badges will be available at registration) REGISTRATION FEES All registration fees include tuition, course materials, continental breakfasts and a welcome reception on Monday at 5:00 pm. TUITION $575 Dentist / $425 Auxiliary /Other Health Professionals Tuition Refund Policy If a course is filled or cancelled, Emory will refund the full tuition. Cancellations prior to January 1, 2014 will be refunded less a $100 administration fee. No refunds will be given after this date. WHO SHOULD ATTEND Dentists, dental hygienists, and dental assistants. All members of the dental team will benefit from this clinically focused program. EDUCATIONAL METHODS Illustrated lectures and discussion. CONTINUING DENTAL EDUCATION CREDITS Emory University School of Medicine is an ADA CERP Recognized Provider. ADA CERP is a service of the American Dental Association to assist dental professionals in identifying quality providers of continuing dental education. ADA CERP does not approve or endorse individual courses or instructors, nor does it imply acceptance of credit hours by boards of dentistry. Emory University School of Medicine designates this live activity for 16 continuing dental education credits. At the base of Beaver Creek Mountain in Colorado, it is one of the world’s top ski resorts. Enjoy ski in/ski out convenience. For more details go to the web site: www.beavercreeklodge.net ACCOMMODATIONS We are holding reservations for a block of rooms at the Beaver Creek Lodge at the following group rate of $329 (plus tax), King Suite Valley View. You may make room reservations by calling 1-800-525-7280 between the hours of 8:00am – 5:00pm, Mountain Time, Monday – Saturday. To insure you receive the group rate, you must identify the group name Emory 34th Annual Ski and Learn Dental Seminar when making reservations. If making reservations online, go to www.beavercreeklodge.net and select the “Reserve Online” link on the left hand side of the page, then select group from the drop down list next to “Rates and Packages” and enter the code “Emory34”. All reservations must be received before Thursday, December 19, 2013 to receive the group rate. Reservations received after this date will be accepted on a space/ rate available basis only. A one night non-refundable deposit is due at the time of confirming the reservation. The balance will be due 30 days prior to arrival and is non-refundable. The demand for rooms is high during our program and may sell out prior to December 19, so be sure to make your reservations early. Hotel rooms in the Emory Ski and Learn room block are reserved for the Emory Ski and Learn program registrants. To assure that our room block is available for meeting registrants, Emory CDE will contact persons reserving rooms to confirm their meeting registration. MEETING MATERIALS In accordance with Emory University’s sustainability initiative, the course handouts/syllabus will be available on an usb flash drive. We are offering handouts online ahead of time so that you have an opportunity to see them before the meeting. This information will be emailed to you. Conference handouts can be viewed, downloaded and printed from this page. Registrants will receive an email announcement as to when handout material will be emailed. No printed handouts will be available onsite at the meeting, digital only. DISCOUNT LIFT TICKETS You can obtain your ski lift tickets at the advanced purchase rate directly online at www.beavercreek.com/lifttickets. Course participants will receive a discount coupon to be used towards the purchase of individual lift tickets. Lift ticket coupons, which will guarantee you the PEAKS Rate, will be available to you during meeting registration hours. *Group lift tickets will not be available for purchase through Emory University.
