Why Meds2Home…. Meds2Home was created to assist Medicare Skilled
Nursing Facilities or Hospitals with Discharges to home, independent living or assisted living facilities.
We know Medications, their use, and/or failure to continue usage as properly prescribed by their physician contributes significantly to hospital re-admissions after skilled nursing discharge.
We know the role of nursing homes has changed. Facilities are increasingly dedicated to transitioning older adults from the hospital back to their own homes. Short term stays has grown tremendously in the last 10 years.
What We Know…. 20% of Medicare patients discharged from hospitals to
skilled nursing facilities are “re-hospitalized” for conditions that could have potentially been avoided.
40% of these patients being “re-hospitalized” are due to medication errors.
MEDPAC is currently tracking the “re-hospitalizations” rates for nursing home residents, and are monitoring the “rates for residents with potentially avoidable conditions”
As a part of the Affordable Care Act, MEDPAC wants to target and penalize skilled nursing providers with aberrant patterns of “re-hospitalizations” Specifically duel eligible's.
What We Know…. According to MEDPAC, Medicare payments to PAC providers
totaled $59 billion in 2013, more than doubling since 2001, and the agency spends nearly as much on post-acute care and readmissions in the first 30 days after discharge as it does for the initial hospital admission.
As a part of the Protecting Access to Medicare Act of 2014, skilled nursing facilities are required to begin public reporting of re-admission rates starting October 1, 2017.
That CMS will begin withholding 2% of all SNF Medicare payments starting October 1, 2018, and CMS will then re-distribute 50-70% of the withheld back to SNF’s by way of incentive payments to high performing facility providers with reduced hospital re-admissions.
How Meds2Home Can Help…. Meds2Home is intended to make handling and directing
resident medications upon discharge easy, simple and cost effective.
Meds2Home will work directly with the facility discharge planner, nursing department or other designee.
Meds2Home shall communicate and coordinate with the resident’s home health care provider to insure a complete and accurate medication listing, providing a MAR and a drug interaction report upon discharge from a skilled facility.

How Meds2Home Can Lower Cost…. Skilled Nursing and Hospitals Lower the risk of re-admission due to medication errors PAC,
lowering the risk of being penalized. Run drug interactions before sending medications, lowering the
risk of an adverse effects of medications. No more 3 day medication supplies during discharge. Meds2Home
will send medication to the Hospitals, SNF, or patients home day of discharge.
Home Health Companies Meds2Home will decrease the time it takes the nurse to do medication
reconciliation post discharge. Med Reconciliations can take up to an hour post discharge, with
Meds2Home this can take as little as a few minutes giving the nurse more time to further access the patient and increase quality of care.
The MAR provided by Meds2Home will alert the HHC if there are any discrepancies before they get to the home.
