In 2005, S.R., 53 year old, single, from Taguig, Metro Manila, experienced irregular vaginal bleeding occurring every two to three months. There was no medical consult done until April 2008, when a pelvic ultrasound was done which revealed the following: Uterine myoma, and thickened endometrial stripe. Tranexamic acid and Mefenamic acid afforded temporary relief. Follow-up pelvic ultrasound done in July and October 2008 revealed an enlarging uterus, myoma and endometrial mass. In November 2008, vaginal bleeding persisted and S.R. was admitted for surgery which revealed a 12 weeks sized uterus, multiple peritoneal

seedings, ascites, and multiple pelvic lymphadenopathies. The surgical team proceeded with peritoneal fluid cytology, partial omentectomy, bilateral lymph node palpation, biopsy of peritoneal seedings and biopsy of the endometrial mass. Immediate post operative condition was unremarkable.

Surgical pathology report were as follows: Endometrioid adenocarcinoma, Grade II-III, Peritoneum positive for malignancy, Omentum negative for tumor. On day 1, S.R’s treatment plan was fully discussed to include, Chemotherapy, Second-look operation, and Alternative protocols. She decided however to go for oral immunoglobulin and parenteral immunomodulator protocols. She was placed on oral 150 mg immunoglobulin ( 1% IgG, marketed as IgCo) 3 x a day. In January 2009, S.R.’s skeptic relatives, insisted on letting her undergo other forms of treatment. A CT scan was requested by another physician which showed the following: Liver is normal in size. Gallbladder, pancreas, kidneys and adrenal glands within normal, gastro intestinal tract and colonic segments are unremarkable, NEGATIVE for Ascites and lymphadenopathies, Uterus is within normal size with small hypodense mass, and urinary bladder with no filling defect. After knowing her last CT scan, S.R. and even her relatives finally felt assured of faster and better recovery and continued the oral immunoglobulin treatment. As of this issue, she is on her 7th month of oral immunoglobulin and had just started her immunomodulator protocol. A cryosurgery for the uterine mass will follow. /jmsmd

Colostrum is a thick yellow fluid, rich in protein, growth factors, and immune factors. It is secreted by the mammary glands of all female mammals during the first few days of lactation. It also contains essential nutrients and prote-ase inhibitors that keep it from being destroyed by the processes of diges-tion. Humans produce relatively small amounts of colostrum in the first two days after giving birth, but cows pro-duce about 9 gallons (36 L) of colos-trum. AMY GOLETA- DY, born in the province of Camarines Sur, is married to Dr. David Y. Dy, the renowned Surgical Oncologist of St. Luke’s Medical Center. They are blessed with two beautiful and intelligent children, Catherine Lourdes and Christopher Lawrence. Dra. Amy, ( “doktora Amy” - a title commonly used for lady doctors and as most patients and friends would casually call her ) is the incumbent President of the Philippine Society of Pediatric Oncology and the Chair of the Philippine Board of Pediatric Oncology. She is also the first Filipino pediatrician who was trained in pediatric oncology at The Children’s Hospital in Sydney, Australia. She graduated Valedictorian at the Quirino High School in Quezon City in 1974. She completed her Bachelor of Science degree at the University of the Philippines in Diliman, Quezon City, in 1978, where she was a U.P. Zoological Society scholar. She was then awarded a Colombo Plan Scholarship at The Children’s Hospital in Sydney for additional training in pediatric oncology, which she completed in 1991. Thereafter, she also had a stint at the University of Texas M.D. Anderson Cancer Center in Houston, Texas. At present, she is the Vice-president of Cancer Treatment and Support Foundation, a non-stock organization aimed at helping cancer patients finance their chemotherapy. She also helps support the education of deserving high school students in Camarines Sur, being President of the Cesario Ll. Goleta Scholarship Foundation. She is a Clinical Associate Professor in Pediatric Oncology at the University of the Philippines College of Medicine; Chief of Pediatric Oncology at the Institute of Pediatrics of the St. Luke’s Medical Center and is an active consultant at the Manila Doctors Hospital. She is an active member of the American Society of Clinical Oncology, the International Society of Pediatric Oncology, and the Global Cancer Control Community (International Union against Cancer).
