What is it?

• Community-based care health care model

• Provide primary care services to bridge health care delivery gaps

• Targeting 73 rural and remote communities throughout BC

• Services Provided:

• Home Visits

▫ Falls Risk Screening

▫ Vitals Check

▫ Blood Pressure / Oxygen Saturation Check

Population Profile

• Seniors living in remote and rural BC

▫ Referred by their doctor or primary care health provider

• Living with chronic conditions:

▫ Congestive Heart Failure (CHF)

▫ Chronic Obstructive Pulmonary Disease (COPD)

▫ Diabetes

Program Objectives

• Contribute to the stabilization of paramedic staffing in rural and remote communities.

• Bridge health service delivery gaps in the community, identified in collaboration with local primary care teams.
