Wakefield District• Population of 332,000 expected to rise to 338,000 in 2018.

• 65th most deprived district in England, with 40,000 people in the district living in neighbourhoods amongst the top 10% most deprived in England.

• Currently has three council owned ‘Connecting Care’ health and social care hubs.

Challenges and advice• Challenges / issues

– Interoperability (IT systems, records, pathways, etc)– Health and social care cultures are different – Inequality of resources– Conflicts of interest (commercial bidding)

• Advice – Necessary to develop a shared vision– Effective working relationships are key and require investment– Must work towards establishing shared premises as soon as possible– Joint governance is essential to integrate workstreams (e.g. joint operational delivery group)– Shared approach to organisation development (e.g. team of teams)
