TRC Assignment & Gaps in Care Closure

Therapeutic Resource Centers (TRCs) provide specialized pharmacists to support

patients with complicated conditions

TRCs strive to increase adherence and close “Gaps in Care” – patients not receiving

optimal treatment for their condition

Example:

• Prescription correct for diagnosis, but based on missing refills, patient may not be compliant

Gaps associated with increased risk of disease progression and

complications

TRCs help close gaps by working with patients and physicians to

understand and address issues leading to non-compliance
