What is a CCG?CCG = Clinical Commissioning Group

• Local GPs, nurses, consultants, pharmacists, dentists, opticians working together to drive change in health care

• Leading the commissioning (planning and buying) of health services from 2013

How a CCG differs from a PCTClinicians have direct contact with people and their lives and collectively see the ‘big picture’ The Government believes clinicians have a better understanding of local needs and are better placed to make commissioningdecisions.

Facts and figures• 185,000 patients

• 31 GP practices

• Covering Mirfield, Spen, Batley, Birstall, Birkenshaw, Dewsbury

• 86% of patients choose Mid Yorks NHS Trust hospitals

• By 2030 20% of our population will be over 65

Key local issues

• Infant death rates – reducing, but still too high

• Smoking related illness

• Obesity / lack of physical activity – especially amongst children

• Dewsbury – 1 in 3 adults have a long term illness.

Making an impactWe’ve already made an impact by getting someservices closer to patients:

Blood tests – now available in GP surgeriesDermatology – delivered in the community24 Hour blood pressure monitoring – went live in JuneReforming the ‘patient journey’ Working with partners across the Mid Yorkshire areaon health service transformations

Our challengesFinance – the NHS budget is shrinking so there’s less money to go round at a time when we’re living longer and health needs are increasing. This will also affect our ability to offer grants

Balancing between being a GP providing healthcare and being a commissioner (buying services)

Working with Mid Yorkshire Hospital Trust to help it resolve its financial difficulties

What we want to achieve• Bringing health and social care together seamlessly

Working with Mid Yorkshire Hospitals NHS Trust• Health issues are changing; so health care needs to be

delivered differently • the NHS is changing - so we can do things differently and create higher standards, better patient care and improved safety• Increasing specialisation gives better results• £20bn savings in the NHS means we must do more with less

Meet Doris• 85 yrs old lives alone but independently • falls on a Friday evening • goes to A&E via ambulance• no clinical information about Doris at casualty• has tests – results apparently abnormal• it’s now 11.30pm so Doris is admitted

... and there’s more• Doris could go home next morning but there are concerns about support and she’s transferred to an acute ward

• Doris is hospitalised for several days until care and support in place

• she becomes confused, loses confidence and independence

• Doris needs to go into a home
