Ferrucci1 Perspective Any doubt that the success of extracorporeal shock-wave lithotripsy (ESWL) for kidney stones could be transferred to gallstones has been dispelled by the landmark report of Sackmann et al. [1 ] from West Germany, which appeared in th e New England Journal of Medicine in February 1988. In a major expansion of their initial report of nine patients [2], which appeared 2 years earlier in the same journal, the present study documents successful initial fragmentation of gallstones in 1 74 of 1 75 patients, with negligi le complica- tions. The authors, most of whom are gastroenterologists and engineers at Dornier (Munich-Germering, W. Germany), treated all their patients by using a modified Dornier HM3 water-tank kidney lithotriptor specifically fitted with a real- time sonographic scanner for localization. However, since September of 1987, an equal number of patients have been treated with similar success on an improved, second-gener- ation non-water bath unit, the MPL-9000. The findings of Sackmann et al. [1 ] came as no surprise to afficionados of biliary lithotripsy, many of whom had visited Munich while the study was in progress. However, with the publication of this study as a lead article in the New England Journal of Medicine, the results assume immediate and wide- spread significance. For the one in 1 0 adults who will develop gallstones, an appealing new therapeutic option is now im- minent; a new segment of the medical equipment industry has been energized; and a decade-old but somewhat ignored medical discipline, chemolytic therapy of gallstones, has been redeemed. All of the above give strong impetus to the Food and Drug Administration-monitored, multicenter clinical trials being initiated by Dornier and several other manufacturers nationwide and promise to mark 1988 as a landmark year in the treatment of gallstone disease. How valid are the results of the Munich study? WilFthey be substantiated by studies in the United States? What technical and clinical issues are not answered? What are the implica- tio ns for r ad io lo gis ts ? Unique clinical details emerging from the study deserve emphasis. Strict eligibility criteria allowed only 28% of referred patients to be accepted for treatment. These included require- ments that patients have symptoms, have three or fewer gallstones, have stones no greater than 30 mm, have no calcification on a plain abdominal radiograph, have a function- ing oral cholecystogram, and have no other medical contrain- dications such as acute biliary obstruction, signs of cholecys- titis, pregnancy, or arrhythmia. Of the 1 75 patients selected for ESWL, 1 45 had a solitary gallstone with a mean diameter of 1 9 mm; the remaini g 30 patients had two or three calculi. Patients were treated in the prone position while partly im- mersed in the water bath with the shock waves entering via the ventral subcostal area. ECG triggering was used, and a total of 430 to a maximum of 1600 shock discharges were delivered in a range of 13-1 65 mm. Fragmentation was monitored sonographic lly until loss of the echogenic focus and acoustic shadow occurred. The limit of 160 shocks was chosen on the basis of published bioeffects data in ani al experiments. General anesthetic was used in the first 54 p tients, but more than half (56%) of subsequent patients were given only IV opiates, while the rest were given epidural anesthetic (22% ), general anesthetic (21 % ), or no anesthetic at all (1 % ). Each patient received adjuvant oral bile acid therapy to dis- solve residu l fragments. The medication regimen consisted of a combination of chenodeoxycholic acid and ursodeoxy- cholic acid administered in a single d se of 7.5 mg each at bedtime.
