    Examination/Athletic Permit Card) and the

    MMSD / WIAA ELIGIBILITY

    Eligibility - 0 F's = immediately eligible to

    play.

    1 F = 2 game suspension. 2 F's = 3 game suspension.

    3 F's = Not eligible to go out for a fall

    sport (Football). Incomplete = You have 10 days to

    complete or it turns into an "F".
