Dear Friends of Us TOO,

Throughout 2016 Us TOO continued to focus on providing educational resources and support services to the prostate cancer community at no charge through a network of more than 200 prostate cancer support groups across the country and abroad, website updates, grassroots educational events, monthly newsletters and news feeds, social media, and fundraising.

Us TOO International Chairman Jim Rieder, Vice Chairman Jerry Deans, Treasurer C. Todd Ahrens and Secretary Bill Seidel provided consistent leadership through their ongoing service as members of the Executive Committee. In April, Chuck Strand moved from Director of Marketing and Communications to CEO, replacing outgoing President and CEO Tom Kirk who retired. We welcomed Stuart Gellman as a newly elected Us TOO board member who began serving in January 2016, and we were pleased with the continuation of service from board members Fred Allen, Tom Cvikota, Peter Friend, Keith Hoffman, Chad Little, Jim Naddeo and Jim Schraidt. We extend our thanks and appreciation to all current and former Us TOO board members for their guidance and dedication. Thank you, gentlemen!

Tim Mix joined the Us TOO staff as Communications Manager after working for several months with the team as a consultant. In addition to providing content development and graphic design, Tim has increased the distribution of Us TOO information through an increased social media presence on Facebook, Twitter, LinkedIn and the Inspire online community.

Terri Likowski was promoted to Program Director Support Group Services. To help maximize the effectiveness of Us TOO support group leaders, the support group leader password-protected web page was updated with comprehensive content to address the process of starting and growing a support group by highlighting best practices. Terri began moderating monthly conference calls open to all support group leaders to help identify challenges and share best practices with content posted on the web page.

The Us TOO Community Conversation on Prostate Cancer event series format was updated to provide an overview on informed decision making throughout all aspects of prostate cancer – from the value of PSA testing and the potential option of active surveillance, to various treatment options and management of side effects. The event in Detroit was broadcast live online to allow remote access to the event and submission of questions and comments. Video from the event was edited into short segments, which were listed by topic and accessible on the Us TOO website.

Us TOO also expanded upon industry collaborations by partnering with the Patient Access Network (PAN) Foundation. Beginning in early 2017, prostate cancer patients who receive financial assistance from PAN can opt in to receive additional information on prostate cancer education and support services. An Us TOO team member will contact interested patients to offer Us TOO resources and services at no charge.

On behalf of the Us TOO staff, Board of Directors, those we have helped in their battle with prostate cancer, and those we will help – Thank You! We sincerely appreciate our corporate sponsors, individual donors, support group leaders, volunteers and event participants for the ongoing support for the work that we do at Us TOO International.

Best regards,

Jim Rieder Chuck StrandChairman of the Board Chief Executive OfficerUs TOO International Us TOO International

The US Prevenve Services Task Force (USPSTF) is updang its controversial guidance about prostate cancer screening, and a final research plan was published online last week. The plan will guide a systematic review of the available evidence on prostate cancer screening. In turn, the systematic review “will form the basis of the Task Force’s updated recommenda-tion statement on this topic,” according to the USPSTF website. In 2012, the organizaon formally recommended against rou-ne PSA-based prostate cancer screening for healthy men, re-gardless of age. However, the document le room for use of the test in the clinic. “Clinicians should understand the evi-dence but individualize decision-making to the specific paent or situaon,” read the final document, which was published in Annals of Internal Medicine (Vol. 157, pp. 120-134, 2012). Nonetheless, recent reports indicate that the use of the PSA test has dropped, especially among primary care providers. In their research plan, the USPSTF will be looking at mulple “key quesons.” The very first queson addresses higher-risk men: “Does the effecveness of PSA-based screening vary by subpopulaon/risk factor (e.g., age, race/ethnicity, family his-tory, and clinical risk assessment)?” “But the question might not be fully answerable,” said Richard Hoffman, MD, MPH, an internist at the University of Iowa, and an expert in shared decision-making about prostate cancer screening. “Finding high-quality data to answer this will be chal-lenging,” he told Medscape Medical News. None of the major

Androgen deprivaon thera-py (ADT) is offered to men with prostate cancer who have a rising PSA aer cura-ve therapy (PSA relapse [PSAR]) or who are consid-ered not suitable for curave treatment; however, the opmal ming for its intro-ducon is uncertain. We aimed to assess whether immediate ADT improves overall survival (OS) com-pared with delayed therapy. In this randomised, mulcen-tre, phase 3, non-blinded trial, we recruited men through 29 oncology centres in Australia, New Zealand,

and Canada. Men with pros-tate cancer were eligible if they had PSAR aer previous aempted curave therapy (radiotherapy [RT] or sur-gery, with or without postop-erave RT) or if they were not considered suitable for curave treatment (because of age, comorbidity, or local-ly advanced disease). We used a database-embedded, dynamically balanced, ran-domisaon algorithm, coor-dinated by the Cancer Coun-cil Victoria, to randomly as-sign parcipants (1:1) to im-mediate ADT (immediate (Connued on page 4)

Metastases develop in a small proporon of men placed on acve surveillance (AS) for prostate cancer (PCa), but the risk is signifi-cantly higher in some men than others, such as those with Gleason 7 tumors, re-ported a new study pub-lished in The Journal of Urol-ogy (Vol. 195, pp. 1409-1414, 2016). In a study of 980 men placed on AS – 769 with low-risk and 211 with intermediate-risk PCa – a team led by Laurence Klotz, MD, of Sunnybrook

Health Sciences Centre in Toronto found that 30 men (3%) progressed to meta-static disease at a median of6.3 years aer diagnosis.Metastases developed in 13 (10%) of 133 patients withGleason 7 disease. Metastases occurred in 16 low-risk and 14 intermediate-risk patients, and developed in bone in 18 patients and lymph nodes in 13. Of the 30 men, 15 died from PCa, four died of other causes, and 11 are alive with metastases. Men with intermediate-risk disease were at higher risk for metastasis, but those with Gleason score 6 and PSA

levels greater than 10 ng/mL were not at increased risk. In multivariate analysis, a PSA doubling time less than three years vs. greater than three years was associated with a 3.7-fold increased risk of me-tastasis. Gleason score 7 com-pared with 6 was associated with a three-fold increased risk. The presence of three or more positive biopsy cores was associated with a 2.7-fold increased risk compared with fewer positive cores. The authors concluded that AS appears safe in men at low-risk and in select men at

Advocacy InitiativesUs TOO continued its advocacy outreach with attendance at events such as the ZERO Patient Advocacy Summit. Outreach was extended to constituents through email, social media, and other platforms, with hopes to positively impact legislative initiatives affecting prostate cancer research.

Educational MaterialsMultiple prostate cancer educational brochures and books continued to be made available to the prostate cancer community. All materials were acccessible digitally through PDF documents housed on the Us TOO website. In addition, Us TOO mailed and distributed thousands of printed pieces in response to hundreds of requests for the material, which was provided at no cost.

Inspire Online Community Us TOO continued to provide additional support services to the prostate cancer community through hosting the Inspire online prostate cancer communities (UsTOO.inspire.com) with 16 prostate cancer discussion groups specific to the following topics:

• Newly diagnosed• Treatment options• Active surveillance• Managing side effects• Genomic testing• Exercise and nutrition• Wives, family, friends and caregivers• Prostate cancer and intimacy• Screening and early detection• Recurrence/advanced disease• Military veterans/Agent Orange• Clinical trials• Advocacy• Local support group leaders• In Memoriam• Prostate cancer news

Support ServicesUs TOO support groups provide an invaluable forum for peer-to-peer interaction to exchange information and personal perspective specific to effectively navigating the prostate cancer journey. Each Us TOO support group functions independently and is led by local volunteer, which could be a prostate cancer survivor, caregiver or medical professional. Groups hold regular meetings for men living with prostate cancer and their families. In addition to emotional support, the meetings provide unbiased educational information related to prostate cancer treatment options for newly diagnosed men or those managing advancing disease, solutions for addressing side effects, and nutrition. Us TOO group meetings are free and open to the public. In order to offer improved “support to the supporters,” Us TOO initiated regular support group leader calls and launched a webpage specifically to address their needs and materials.

Awareness and Fundraising

12th Annual SEA Blue Chicago Prostate Cancer Walk & RunSEA Blue is Chicago’s largest prostate cancer fund raising and awareness event. More than 1,200 people attended the SEA Blue Prostate Cancer Walk & Run on Sunday, September 11th in Chicago’s Lincoln Park. This year, on the anniversary of 9/11, we recognized local first responders. Prostate cancer survivor and baseball legend Ken Griffey Sr. was our special guest star who met with attendees, posed for pictures, and offered words of wisdom.

In 2016, “SEA of Blue” lemonade stands were once again created to help promote the SEA Blue Chicago Prostate Cancer Walk & Run, and to bring prostate cancer awareness to local communities. Kids dressed up in blue and served blue lemonade to promote the SEA Blue tagline, “Blue is to prostate cancer as pink is to breast cancer.” Several stands were held throughout the Chicago area, for various news outlets, and on television.

The Second Annual SEA Blue Ribbon Walk in Rochester, NYAttendance doubled from the previous year at the Second Annual SEA Blue Rochester Ribbon Walk on Saturday, August 20th. The family-focused event was organized by Us TOO Rochester Chapter Leader Patrick Fisher and featured a walk through the park, refreshments, and kids’ activities. Vital funds and local awareness were raised for Us TOO Rochester.

We thank all sponsors, donors, volunteers, and participants.
