Registration link can be found on the school website at

https://www.pmschools.org/domain/46. You will need to click on the Before and After

Care/Summer Recreation tab. This will bring you to the Summer Enrichment link.

Submit one registration form per class/per child. Each class will be waitlisted until we have

the minimum number of students required. Once we have the minimum number of students

registered you will receive an email confirmation and your payment will be charged.

Resident Students of Patchogue Medford School District are invited to participate in a selection of

enrichment opportunities, including, Athletics, Arts & Crafts, Culinary and STEAM. Programs are age-

appropriate; indicated by the grade the student will enter beginning September 5, 2019. Sessions will be

held at Tremont Elementary School, 143 Tremont Avenue, Medford, NY 11763 (Directions can be found

on our website), Monday through Friday from 9:00am to 12:00pm.

Registrations are processed in the order in which they are received. PLEASE REGISTER EARLY,

CLASSES FILL QUICKLY AND WE TREAT THEM ON A FIRST COME, FIRST SERVE BASIS.

Registration ends close of business June 14, 2019. Everyone will be placed on a wait list when they sign

up for a course. As soon as the required minimum is met you will receive an email stating that your child

has been approved and your payment will be charged.

The registration link can be found on the school website (www.pmschools.org) under the Parent

Information tab, Before and After Care/Summer Recreation. You will need to click on Summer

Enrichment. Each class registration and/or child must be on a separate form. Children wishing to sign up

for more than one course will need an additional form for each class. The fee per course is $125.00 and

may be subject to an additional $25.00 material fee, course narrative will state accordingly. If a

course is full or minimum enrollment not met, all registration fees will be returned. Refunds will not be

made after the start date of the session. Confirmation for your registration acceptance will be sent via e-

mail, to the email address you provide. BE SURE YOU INCLUDE YOUR PROPER EMAIL

ADDRESS. If you do not have an email, you may call our office at (631) 687-8105 after July, 1, 2019.

Please be respectful of others in the course sessions by arriving on time with all required materials.

Students are encouraged to bring a small healthy snack and a bottle of water for break time.

Please be mindful when completing the registration form with regard to designating adults who may pick

your child(ren) from the Summer Enrichment Program. We will only release your child(ren) to the

designee on record with proper photo identification. Children will be signed out by the parent/guardian or

by his or her designee on record. Everyone picking up a child, from Summer Enrichment, including

parents or guardians, must present a valid driver’s license or government issued photo identification.

Session 1 Enrichment Courses

July 8 - 19, 2019

Sports Mania: Grades 1-3 Children will have the opportunity to compete in games of basketball,

soccer, volleyball and other athletic activities. They will be building on

the basics of the sport, while learning sportsmanship and team loyalty.

(Maximum 20 students)

Games and Sports: Grades 4-6 Children will learn the rules of various sports, while interacting with peers. Come play baseball, kickball,

soccer, and other fun activities and learn the art of healthy competition. Bring your sneakers and a positive

attitude! (Maximum 20 students)

Craft Quest: Grades 1-3

Our quest for this crafting journey takes us through the seasons. Children will be making

unique crafts that are creative and enjoyable! They will also experience the fun of

journaling adventures, listening to music and performing art experiments. From mason

jar surprises to string art and our old time favorite; quilling! This course has a $25

material fee, which is due at time of registration. (Maximum 12 students)

Craftin’ Creatures: Grades 2-4

Cone join us on a wild adventure crafting for creatures! Grab some goods and get going, keeping creatures

of the world growing strong. Through this two week session your child will be in a classroom setting where

“Wild Things” dwell creating sand art, smoothies sun catchers, masks and more! They won’t want to miss

this great opportunity to become an eco-savvy creature! This course has a $25 material fee, which is due

at time of registration. (Maximum 12 students)

Full S.T.E.A.M. Ahead: Grades 1-6

Students will have fun learning how to integrate Science, Technology, Engineering, Art and Math into

interesting crafts they will design and take home. They will sharpen their critical thinking skills, while

creating interesting works of art. They will work with a variety of diverse materials such as straws, jars,

clothespins, tubes, etc. This course has a $25 material fee, which is due at time of registration.

(Maximum 12 students)

Cartooning: Grades 4-6

Students will be able to design and illustrate a cartoon character of their choice. They will learn the art of

cartooning and how to re-create their own cartoon cell. They will also learn how to work with different

types of mediums such as tempera paints, watercolors, and colored pencils. (Maximum 12 students)

FUNtastic Adventures in Science: Grades 1-3 Create, explore, learn and play with this hands-on science course. Your

child will experience the silly side of science by getting their hands messy,

getting curious and being creative. Activities will be indoor and outdoor for

a wide range of learning and experiences. Your child will be a scientist every

day of this program; maybe even an astronaut, an archeologist, a bird expert,

detective, nature lover, and craft maker. Your child’s love of exploration and

curiosity will flourish. This course has a $25 material fee, which is due at

time of registration. (Maximum 12 students)

Once Upon a Chef: Grades 1-3 This course combines fun in the kitchen with literacy! Children will work

together to make delicious themed snacks and create crafts based on popular

children’s books. This course has a $25 material fee, which is due at time

of registration. (Maximum 12 students)

Groovin’ in the Kitchen: Grades 4-6

Summer is here! How would you like to spend your day jumping into a yogurt

parfait? Keep cool making snacks and friends. Students will be the chefs by

learning how to make fun, healthy snacks. They will also learn kitchen safety and

unlock their creativity while working on their cooking skills. So, if you want to

become a Cool Kid in the kitchen this is the place to be. Come learn how to make

a fishbowl or a beach scene for your summer BBQ and pool parties, your guests

will be so impressed! This course has a $25 material fee, which is due at time of

registration. (Maximum 12 students)

Tech Kids: Grades 4-6

Students will be able to utilize technology to create fun comic strips, digital storytelling, videos, music

and so much more through multiple engaging applications such as Microsoft Office 365, Toontastic,

Inkle, Bubblr, and so much more. We will also incorporate hands on activities such as creating your own

games, t-shirt designing, scrapbooking and more. This course has a $25 material fee, which is due at

time of registration. (Maximum 12 students)

Love art? Love reading? Love fun? Want to try something new? Come join us! Make a book come

alive. Learn to retell a story by making one of these or more: Pop-up books, Accordion Book, Upside

Down Book or a Folded Flip Book. We’ll have fun making and sharing

books, and maybe a game as well. This course has a $25 material fee,

which is due at time of registration. (Maximum 12 students)
