Before the end of the working day (shift) the employee is to notify the immediate Supervisor of the accident/injury As soon as practicable after an accident/injury occurs, unless the accident/injury is serious or disabling, the Supervisor is to notify: i. The designated Occupational Safety & Health Representative, and ii. Occupational Safety & Health Manager SERIOUS OR DISABLING [Loss Time Injury - more than 1 day] INVESTIGATION Investigation is to be carried out by the immediate Supervisor in consultation with: i. the injured employee, ii. Occupational Safety & Health Representative, iii. the HR Account Manager iv. Other employees as required, (witness) and v. OS&H Manager If a "Notifiable Accident" OS&H is to advise WorkSafe Western Australia INVESTIGATION If required, investigation is to be carried out by the immediate Supervisor in consultation with: i. injured employee, and ii. Occupational Safety & Health Representative if available ACTION 1. Supervisor with the assistance of OS&H completes and forwards a copy of the accident report to: i) Occupational Safety & Health Representative, ii) Occupatuional Safety and Health, and iii) Retains a copy for record 2. Supervisor implements c trols on 3. Supervisor advises Dean, Director, Head of School, Manager of the accident and what preventative procedures have been implemented 4. If appropriate, OS&H notify: i) Vice-Chancellor, and ii) WorkSafe Western Australia ACTION 1. Supervisor implements controls 2.
