The Chronic Illness Rider qualifies under section 101(g) of the Internal Revenue Code (IRC) and cannot be marketed or referred to as Long Term Care insurance.

Policy DetailsAccessLife+ is a whole life insurance product, with Single and Multi-pay plans including 5 and 10 year options, offering optional accelerated death benefits riders for chronic and terminal illness.

Issue ages:• Single pay: 18-75• Multi-pay: 18-70

Face amount limits:• Minimum: $25,000• Maximum: $250,000

📝 Note: Policy issuance and face amount will consider other in force coverage written or administered by National Guardian Life Insurance Company (NGL). At no time can in force coverage exceed $400,000.

Policy Premiums and Underwriting ClassesPremiums are banded based upon face amounts:

• Band 1: $25,000 - $99,999• Band 2: $100,000 - $250,000

Underwriting requirements and pricing vary by band. Policy premiums may vary depending on gender (except in MT), underwriting class and if they are a smoker or non-smoker. For more details see page 24.

Optional Accelerated RidersChronic Illness RiderChronically ill or chronic illness means that the insured has been certified within the preceding 12 months by a licensed health care practitioner as:

• Being unable to perform, without substantial assistance from another individual, at least two of the six Activities of Daily Living (ADL) due to loss of functional capacity for a period that is at least 90 consecutive days; OR

• Requiring substantial supervision to protect the insured or others from threats to health or safety due to severe cognitive impairment for a period that is at least 90 consecutive days.

Six Activities of Daily Living (ADL)1. Bathing - the ability to wash oneself or get in or out of the tub or shower without

substantial assistance.2. Continence - the ability to maintain bladder control, bowel function or the personal

hygiene necessary with this condition.3. Dressing - the ability to take off or put on all items of clothing and/or any artificial device

(ex. braces) that may be necessary.4. Eating - the ability to feed oneself or through intravenous delivery.5. Toileting - the ability to get to or from, or on and off a toilet without substantial assistance.6. Transferring - the ability to move in or out of a bed, chair or wheelchair without

substantial assistance.

Terminal Illness Rider Terminally ill or terminal illness means the insured has been certified by a physician as having a medical condition where the insured has a drastically limited lifespan of six months or less. The client has the ability to add the optional Terminal Illness Rider to the policy at the time of application, with no extra cost. Including this in the policy provides additional financial support when diagnosed with a terminal illness.

• This rider is automatically included if the client selects the Chronic Illness Rider.• No additional premium is charged for this rider.• The client may request an accelerated benefit up to 75% of the current face amount of the policy

subject to a minimum benefit of $1,000.• The accelerated benefit will be paid as a lump sum only.• An administration processing fee of $100 will be charged when the accelerated benefit is paid.

📝 Note: Use of the Terminal Illness Rider will terminate any use of the Chronic Illness Rider.

The Chronic Illness Rider includes:• An option to accelerate monthly 2% or 4% of the face amount which is selected at the time of

application. This cannnot be added after the policy is issued. • Clients can only accelerate up to 90% of the face amount, 10% is retained as a residual death

benefit.• Separate premiums which vary based on benefit option selected.• Once chronic illness benefits under the Rider have started to be paid, the policy premiums and

cash values on the base policy and applicable rider premiums will be proportionally reduced by the change in the base policy.

• If there is an outstanding policy loan balance, a portion of each chronic illness benefit payment will be applied toward repayment of the policy loan.

• This rider qualifies under section 101(g) of the Internal Revenue Code (IRC). » Clients should see their tax advisor prior to requesting benefits.

• Clients on disability or waiting for a disability claim should not apply for this Rider.• The Rider and the chronic illness benefit payments terminate on the earliest of:

» The date that the base policy is surrendered or terminated. » The date that the base policy face amount is reduced or goes to reduced paid-up status for an amount less than the minimum face amount as specified on the Base Policy Schedule Page found in your policy.

» The date the client elects to reduce the face amount or take a loan, if allowed under the base policy after chronic illness benefit payments begin.

• Upon payment of the Chronic Illness Rider benefit, the policy will remain in force but the required premium payable, the death benefit payable and the policy non-forfeiture values will be reduced proportionately based upon the amount of the accelerated chronic illness benefit.

AccessLife+ How to Fund

American consumers often misunderstand who will pay for care when needed. Health insurance and Medicare do not pay for extended care. Medicaid is a means tested public assistance program that generally requires you to divest yourself of assets.

Cost of services, according to PricewaterhouseCoopers (2017), can best be viewed as “a quarter of the time less than $25,000, another quarter of the time the cost is over $240,000.” This means that most of the extended care costs fall somewhere in between. How will you solve this problem?

There are several ways for your clients to fund an AccessLife+ policy, it can be as easy as writing a check, using a certificate of deposit (CD) as a source or accessing an older life insurance policy. Below are a few examples.

Dave and Lisa both age 60, in good health and non-smokers.

Many Americans purchase some form of permanent life insurance that accumulates cash value. However, unlike AccessLife+ older policies do not include accelerated benefits due to chronic illness. Internal Revenue Code §1035 allows an exchange life insurance policy for an improved policy with no fear of taxable gain. Clients can dual purpose their cash value accumulation from one or more policies by exchanging them for a paid-up AccessLife+ policy which provides guaranteed cash value, guaranteed death benefit and chronic illness rider for extended care.

Unless your client otherwise planned ahead for extended care due to a chronic illness, all resources come into play:

• Social Security• Retirement income from investments, savings, pensions • Home equity• Contributions from family members

Depending on where your client lives, the care they want or need may require an outlay between $4,000 and $8,000 per month.* If their regular monthly income from Social Security and retirement is less than what’s needed they will have a monthly shortfall. Where will the money come from? You can help them bridge the gap with AccessLife+.

Jeanne is in her mid-80s. Her Social Security and retirement income are $5,000 per month. She has been diagnosed with Alzheimer’s disease and needs to move to a memory care community that costs $7,000 per month. If Jeanne had purchased an AccessLife+ policy when she was 65 for $100,000 with a 2% chronic illness rider, she could receive $2,000 per month for 45 months to cover the shortfall after she satisfies her 90-day elimination period. Additionally, there’s $10,000 reserved for her final expenses.

Initiate Drop Ticket The Drop Ticket - Insured InformationIf you would like to review what information needs to be collected for the Drop Ticket, utilize the Form 4682 Drop Ticket to help walk you through these questions.

Insured Information:• Name, address and contact information• Owner must be the same as insured to issue a contract

(this can be changed after issue)• Gender• If the client’s birthday is within 30 days of application a

note will display that premium may change if underwriting process takes longer

• Entire Social Security number• Driver’s License number and state issued in• Marital status• Occupation and details • Power of attorney or guardianship information• Driving history• Criminal background• Aviation and hazardous sports background

BeneficiaryThe client can select up to four beneficiaries, including both primary and contingent when completing the Drop Ticket. The beneficiaries can be a person or estate/charity or both if they wish to have multiple beneficiaries. If they choose to have multiple beneficiaries they will need to select what percentage each will receive and total 100%.

• If an estate or charity, name and address information will be requested.• If an individual, name, relationship to the insured, date of birth, Social Security number, gender

and address will be requested.• If there is a need for additional beneficiaries, contact NGL immediately following approval to

recieve further instructions at 800.762.9883.

📝Note: A contingent beneficiary is someone or something that receives the benefits of the policy if the primary beneficiary is no longer able to accept the benefits.

ReplacementMany states have regulations concerning policy replacement. If the client answers “yes” to the first replacement question, you will have the option to enter in the required replacement information. If you choose to do so, the appropriate state forms will be populated and voice signatures will be collected during the Tele-App. A paper copy of the signed replacement form will be mailed to the proposed policyowner within three days of receipt of the application.

If you choose not to enter the required replacement information, you will need to complete a paper copy of the form and provide it to NGL and leave a copy with your client.

📝Note: Form 2337-AL+ Section 1035 Exchange Assignment with a wet signature is also required for replacements specifically involving a 1035 Exchange.

Face AmountInclude the amount of insurance requested and if the client would like a Single pay, 5 pay or 10 pay.

Selecting the Riders When completing the Drop Ticket, the client has the option of selecting the Chronic and/or Terminal Illness Rider. If the Chronic Illness Rider is selected, the client will automatically receive the Terminal Illness Rider. If the Chronic Illness Rider is not selected, the client may still select the Terminal Illness Rider. If neither rider is selected, a life insurance only policy will be issued.

For the Chronic Illness Rider, the client will need to select to accelerate 2% or 4% of the face amount. This amount is chosen at time of application and may not be changed throughout the life of the policy.

Physician InformationThe client will need to provide their physician name, clinic name and clinic address..

Submitting the Drop TicketUpon submission of the Drop Ticket you will receive a case number. The last six digits are needed for the Tele-App process and payment forms.

You can check the status of your cases by going into the Drop Ticket and selecting Dashboard. This will allow you to search by Case Number, Policy / Certificate Number, Applicant Name, Agent Name, Agent ID or Case Creation Date.

📝Note: Before the Tele-App begins please provide the client with all state disclosure forms, including the Notice to Insured (second page of Form 4677-AL+ Tele-App Prep). State specific client kits with all required forms can be found on the supply section of MyNGLConnect.com.

Required InformationPlease work with your client to help them prepare for the Tele-App process. They will be asked to provide:

• Health conditions and medical diagnoses• Names and addresses of physicians• Medications and herbal supplements • Employment status• Driving history• Hobbies, sports and/or aviation• Bank information• Policy information on policies being replaced (company name, address and policy numbers)

Verification of Client InformationTo start the interview, all client information collected in the Drop Ticket will be verified. Please be sure that the client has the required information with them for the interview. This will help the interview go smoothly.

Health Questions Your client will be asked questions about their medical history and health status and they should be prepared to discuss specific details.

Cognitive Screening (if required)The client may be asked to perform verbal activities to evaluate their cognitive condition if they are aged 60 and older or upon underwriting discretion. You cannot assist, listen or participate during the cognitive portion of the call.

Encourage the client to be in a quiet area where they can focus and take their time without interruption.

Tele-App - Scheduled CallIf the client wishes to schedule the Tele-App call, a number of forms should be left with the client. These forms can be found in the AccessLife+ kit, which is available on the supply section of MyNGLConnect.com, and includes the following information for completing the Tele-App process:

• Form 4677-AL+ Tele-App Prep - Includes the Notice to Insured• Form 4677-AL+ HQ Underwriting Questions• Form 4684-AL+ Premium Page• Required State Disclosures

Drop Ticket Follow Up for Tele-AppDrop Tickets will remain active for 30 days and we will follow up three times with the client to try to finish the process. Please follow up with your client to remind them about the Tele-App completion process. The follow up schedule is:

Day 0: Make initial attempt within 24 hours of receipt

Day 2: Make second attempt

Day 4: Make third attempt

Day 6: Contact the agent if the client is unavailable

Day 8: The Drop Ticket will be marked as “Inactive”

Day 30: The Drop Ticket will be canceled

Once the call is scheduled, please call your client the day before to remind them of the call and the required information they should have on hand.

After 30 days the drop ticket will be canceled. If your client decides to proceed, a new drop ticket will have to be started.

1035 Exchange ProcessThis option is available if choosing a Single pay policy. Since some carriers require the original 1035 Exchange paperwork, Form 2337-AL+ Section 1035 Exchange and Form W-9 Taxpayer Identification Number and Certification must be wet signed by the client and mailed to the contact information on the Section 1035 Exchange Form. These forms are available in the supply section of MyNGLConnect.com.

CoverageConditional receipts will not be provided. The policy will not take effect until all eligibility requirements have been met, including receipt of premium by NGL, and not until the issue date stated in the policy. The 30 day free-look provision begins when the client receives their policy.

Down and Multi-PayIf the client would like to fund part of the policy via lump sum check or 1035 Exchange and the remainder paid with a multi-pay plan, elect the appropriate plan option in the Drop Ticket (Single pay, 5 Pay and 10 pay). You will need to send both the check or the 1035 Exchange requirements (originals with wet signature) and Form 4684-AL+ Premium Page for the multi-pay plan. No payment information will be collected during the Tele-App. Two policy numbers will be assigned and two contracts will be issued.

• A separate $25,000 minimum face amount applies to both the Single pay portion and the Multi-pay portion of the plan.

• The total face amount of the two policies need to remain the same (cannot be increased). • The claim benefit amount (2% or 4%) will not be different between the two policies (there is only

one place to pick that in the Drop Ticket or Tele-App process). • The multi-pay premium needs to be received by NGL within 30 days of the Tele-App.• Each policy will have an effective date of when funds are received.

DeclinedIf you are present with your client at the end of the Tele-App, you will be informed of the underwriting decision. Please make your client aware of the decision. Your client will also receive a letter from NGL informing them of the decline. For additional information regarding the reason for decline, please contact our Underwriting department.

If you are not present at the end of the Tele-App, you will receive an email with the underwriting decision. Please inform your client of the decision as soon as possible. Checking the Status of Submitted Tele-AppsAccess your dashboard through MyNGLConnect.com, select the name of the agent(s) on the left and select the date range on the right. When your date range is selected click “Load for Selected Agents.” The table will auto-populate in the order business was received for the selected date range.

Click on the column names to sort by the corresponding columns.

New Business Status• Business that has been accepted will show a green check mark to the right. • Business that has an exception that needs to be addressed will show a red ‘x’ next to the status.

Modified Endowment Contract (MEC)By nature of the premium amounts, all policies may be classified as MECs. When a policy is classified as a MEC, withdrawals and loans and surrender value are taxable to the extent there is a gain on the contract. If taxable, NGL will automatically withhold 10% unless instructed otherwise. Additional tax penalties may be incurred if the policyowner is under age 59 ½. We advise the policyowner consults with a tax advisor on the possible tax consequences.

AccessLife+ Underwriting Guidelines

Field UnderwritingYou are the first and most important step in the risk selection process.

Determining eligibility for this product utilizing the tools provided will avoid an adverse decision and time spent by you and your client completing the Tele-App process.

The NGL Rate Calculator will assist in determining the Rate Class your client may qualify for based on your review of the List of Ineligible Impairments, a few questions regarding health history and Activities of Daily Living while factoring in build and smoker status. If in doubt regarding which class to quote, we suggest that you quote the Standard rate class or contact our Underwriting Team to further discuss. Assuring your client is prepared for the NGL EZ Apply process with the needed information such as doctor name/address, beneficiary information, policy number and carrier name for replacements, reason for prescription drug history, etc. will provide a better customer experience.

Smoker ClassificationNo smoking of cigarettes, marijuana, e-cigarettes/vaping, hookah, or consumption of smoking cessation products in the past 12 months qualifies as a non-smoker.

Applicant AgeThe applicant’s age is the actual age or that of their last birthday. Age will be determined based on the policy issue date. There is no provision to save age when applying for this policy. If your client’s birthdate is close to the application date, we suggest quoting both ages in the event of any delays. The minimum issue age is 18; maximum issue age is 70 for Multi-pay, 75 for Single pay.

Base Underwriting InformationWhen using the Rate Calculator, you will be asked a few health conditions in order to determine which rate class should be quoted:

• Asthma• Diabetes not treated with insulin• Depression or anxiety• Controlled high blood pressure• Epilepsy or seizure disorder

The outcome of these questions along with Build and Smoker Classification will determine the rate class of Preferred or Standard, Non-Smoker or Smoker, or Decline. If Decline is indicated, please do not proceed with a Drop Ticket. This is not a guarantee of the rate class outcome as the final decision will be determined after all underwriting requirements are reviewed. Standard rates are designed as up to Table 4.

Chronic Illness Underwriting InformationIf your client is applying for the Chronic Illness Rider a question regarding Activities of Daily Living will be asked to assist with determining the rate class quote of Preferred or Standard. Please remember if the rider is issued Standard, Preferred Life is not available. Standard rates are designed as up to Table 2.

What Could Delay the Tele-App Process? Technical difficulties during the Tele-App process could delay the completion of the process, including availability of external sources of data used in the underwriting process such as Medical Information Bureau (MIB), Rx, Motor Vehicle Registration (MVR), or cognitive testing. Should this occur, we will attempt to reschedule once the issues are resolved. We don’t anticipate this to happen often, but if it does we will do our very best to communicate on MyNGLConnect.com.

What Could Delay an Immediate Underwriting Decision?At times an immediate decision may not be available during the Tele-App process and may be referred to NGL Underwriters for further review. As an example an MIB code that doesn’t align with information provided by the client which may require additional information from either medical records or outreach to the reporting carrier to confirm the applicant’s identity.

Attending Physician Statement or Medical Records RequestIf an Attending Physician Statement or records are required, NGL will request that a “wet signed” HIPAA-Auth Form be submitted in order to expedite retrieval as many providers will not accept voice signed authorizations. NGL may be required to reach out to you further if the provider requires their own special authorization prior to release. Requests for records can delay underwriting decisions on average 30 days. NGL will request and pay for these records.

Additional Requirements may be Requested at Underwriter DiscretionOccasionally NGL may request requirements beyond the Age/Amount/Payment Mode if needed. As an example, if we received information regarding potential cognitive impairment from an external source used in the course of underwriting, we may request a cognitive test despite the age of the applicant.

What Do We Consider “Treatment or Advice”?Many questions within our application ask ‘have you consulted, received medical advice, or been diagnosed, treated, or prescribed medication by a member of the medical profession’ within a time period. Taking medication is considered treatment as are implanted devices as we consider that they are both treating a condition. Being advised to take medication is considered receiving medical advice. These examples would require affirmative answers.

Chronic ConditionsPlease be mindful that many conditions we ask about within the application are chronic conditions that don’t resolve or may not be ‘cured’ when considering look back periods. As an example, if a client advised that they had Chronic Obstructive Pulmonary Disease, but no longer have it, you should probe a bit further for any current medications, a pulmonologist, or any inhalers or breathing treatments.

Ineligible Conditions Our list of ineligible for coverage and prescription lists are not all inclusive, but intended as guides. Some conditions or prescriptions not listed as ineligible may be declined for complications or comorbidities not listed. The underwriters review and decision will govern in all cases.

Adverse DecisionIn the event we need to decline to offer coverage, we will provide the applicant with the reason(s) for our decision in writing with a copy to the agent.

Exclusion RiderAn exclusion rider may be offered as needed. Aviation other than commercial and hazardous sports are examples. Please see the Aviation and Hazardous Sports section on page 24.

Smoker ChangeSmoker classification change after issue will be available upon request. Please contact Life Product Team to inquire. A Smoker Questionnaire and evidence of insurability will be required. Any health history changes since original issue date will be considered in our decision.

Viewing Policy InformationAfter an application has been approved, additional reporting can be found on the “Dashboard” section of MyNGLConnect.com. The Dashboard makes it easy to:• Review the status of your new and existing business• Search policies for real time values• View important messages and alerts regarding your business• Review correspondence that has been mailed to your clients• View commission statements• View contact information• Reports• Address book

Policy DeliveryPolicies should be delivered to clients promptly and the delivery receipt should be signed and returned to NGL where required.

Information regarding declined applications must be communicated to the clients promptly. Never keep custody of any policy or contract for any period longer than absolutely necessary for purposes of analysis, record keeping or service review. Policies or contracts belonging to you or your immediate family are excluded.

ServiceAmendments From time to time signature amendments may be required to clarify a term of the contract, such as name spelling, the insured’s date of birth, or the policy face amount. NGL provides a return envelope along with each signature amendment included in the policy packet for the policyowner to mail to NGL. If delivering a contract with a signature amendment you may also fax the signed copy to 608.373.9508.

Changing the PolicyownerAll AccessLife+ policies will be issued with the insured person as the policyowner by default. To make a change to the owner, once active you may initiate the change by contacting customer service to have a form mailed to the current policyowner, or by providing the policyowner with the Policyowner Request for Service Form. Requirements:

• The current owner must complete Section 3 of the Policyowner Request for Service Form.• The form must be signed by both the current and new policyowner before a notary, the

document must contain a notary seal or stamp.• If the policy was purchased in a community property state (AZ, CA, ID, LA, NM, NV, TX, WA,

or WI) the current owner’s spouse must sign the form or a note must be included attesting that the current owner does not have a spouse or they are deceased.

• A completed and signed W-9 for the new policyowner must be completed.

Reinstating a Lapsed PolicyThe AccessLife+ policy may be reinstated within three years from the date the last premium was due. The insured must complete the reinstatement application to be reviewed for proof of insurability. All health questions must be answered by the insured. The reinstatement application must indicate which riders if any they would like to reinstate along with the life benefit. Please contact our Life Product Team to request a reinstatement application.

Cash Value LoansLoans may be taken against the cash value of the policy, see the schedule of cash values within the policy. If there is a loan against the policy and claim is filed against the CI Rider, a portion of each CI payment will be deducted to repay the loan. This deduction from each CI payment will not exceed the benefit payable by the CI Rider.

Premium/Loan Payment Methods Options• If your client wishes to change their payment on a loan, please complete a Bank

Authorization for Premium Payment Form and include a voided check. • Or contact our Life Product Team to setup an automatic payments of policy loans.

Policy MaturityThe AccessLife+ contract will mature when the insured reaches attained age 121, see the contract for maturity benefit options.

Beneficiary Changes• The policyowner may request a beneficiary change any time while the insured is living. • Indicate the change in Section 3 of the Policyowner Request for Service form. • Form must be signed by the policyowner and the signature should be notarized. NGL will

accept a witness signature in lieu of a notary if the owner’s physical signature is on file. The beneficiary elect may not act as witness.

• If the policy was purchased in a community property state (AZ, CA, ID, LA, NM, NV, TX, WA, or WI) the owner’s spouse must sign the form or a note must be included attesting that the owner does not have a spouse or they are deceased.

Policy SurrenderThe policy may be surrendered for the cash value, see the schedule of cash values for more information. Requirements for the owner to surrender the contract:

• Complete section 21 of the Policyowner Request for Service form.• Include withholding instructions in the tax withholding portion of this form. Note: NGL will

automatically withhold 10% if no withholding instructions are provided. • Return the original policy or complete section 6 of the form, check the box No Action

Required.• Form must be signed by the policyowner and the signature should be notarized. We will

accept a witness signature in lieu of a notary if the owner’s physical signature is on file. The beneficiary elect may not act as witness.

• If the policy was purchased in a community property state (AZ, CA, ID, LA, NM, NV, TX, WA, or WI) the owner’s spouse must sign the form or a noted must be included attesting that the owner does not have a spouse or they are deceased.

Face Amount ChangesIncreasing the face amount of the policy is not allowed after issue. To increase benefits the policy owner would have to apply for an additional policy. The maximum combined AccessLife+ face amount for all contracts is $250,000.

The face amount may be decreased as low as the minimum allowed for this product ($25,000). Requirements for the owner:

• Complete section 11 of the Policyowner Request for Service Form. • Form must be signed by the policyowner and the signature should be notarized. We will

accept a witness signature in lieu of a notary if the owner’s physical signature is on file. The beneficiary elect may not act as witness.

• If the policy was purchased in a community property state (AZ, CA, ID, LA, NM, NV, TX, WA, or WI) the owner’s spouse must sign the form or a note must be included attesting that the owner does not have a spouse or they are deceased.

Non-Forfeiture Options (NFO)Extended Term Insurance is the default NFO that each policy will be issued with. The policyowner may request to change the NFO option to Reduced Paid Up Insurance by completing section 7 and 14 of the Policyowner Request for Service Form. Note that Automatic Premium Loan appears on the form but is not available for this product.

Important Things to KnowInsurable InterestWhile the owner and insured must be the same person at time of application, there is an option to change ownership of the policy after it is issued. In order for a life insurance contract to be valid, the owner must have an insurable interest in the life of the proposed insured when the policy is issued. In most cases, the beneficiary must also have an insurable interest in the life of the insured. A person has an insurable interest in the life of the insured if the person is likely to suffer some loss or detriment if the insured dies.

The following situations meet the insurable interest requirement:• A blood relationship or family ties.• A debtor / creditor relationship (to the extent of indebtedness).• A corporation where an economic loss would occur if the partners, officer or key person

were to die.• When no other insurable interest exists, the estate of the insured should be named as the

beneficiary.

Policy Face Amount IssuancePolicy issuance and face amount is dependent on other policies in force with NGL. This means that while your client may have an approved decision from the Tele-App process, their accepted face amount is subject to our policy limit at NGL and may be reduced based upon existing coverage.
