Come join us in San Francisco for the 19th Annual Congress of the American College of Phlebology, November 10-13, 2005.

This year’s meeting will build on 20 years of success in providing education in the diagnosis and treatment of venous disease. The Pre-Congress day will feature live patient evaluations followed by treatment discussion and demonstrations from an expert panel. Workshops throughout the Congress will be supported by video feed of live cases, complemented by instructional videos, designed to provide maximum viewing for all workshop attendees. The Scientific Abstract sessions have been expanded to allow audience and panel discussion of these latest advances in research. New additions to our always popular Symposia will include a special session by the American Venous Forum on endovascular interventions for deep venous disease. Due to overwhelming demand, this year’s Interactive Sessions will be limited in size but not in scope, covering hot topics from foam sclerotherapy and endovenous ablation, to proper coding, setting up a phlebology practice, and “other than legs.” You will want to register early for these. And finally, we are honored and excited to have Dr. Peter Gloviczki, Chief of Vascular Surgery at Mayo Clinic and world-renowned expert on venous disease, as this year’s keynote speaker.

The strength of the American College of Phlebology has always been the diversity of its membership all united towards the common goal of advancing the study of Phlebology. San Francisco is a city rich in culture and beauty, made up of bustling Chinatown, the gentrifying Mission, clubby SoMa, hippie Haight-Ashbury and faux-hemian North Beach. The stunning Embarcadero waterfront in the heart of this mosaic of a city will be the perfect setting for this years meeting.

General InformationRegistration Information Advance registration is required to ensure that badges and tickets to sessions and social activities will be available when you arrive at the Congress. Please use the registration form provided or register online. Advance registration closes October 28, 2005. Any registrations received after this date will be processed at the on-site registration rate. On-Site registration will be available, however only a limited number of courses and social activities will be available.

Pre-Congress Day Registration Fees (Thursday, November 10, 2005) – Optional

The purpose of the Pre-Congress Symposium is to provide an intensive review of the core concepts of phlebologic evaluation and treatment. These include venous anatomy, physiology, ultrasound evaluation, surgical and non-surgical treatment modalities and nursing participation. We have divided the day into tracks to specifically accommodate those who have specialized interestin surgery, ultrasound and nursing. While the Symposium is oriented toward providers who seek introduction to Phlebology, it is comprehensive enough to provide a very thorough review for even the most experienced practitioner.

This year we are changing the format of the afternoon workshops to be more practical and problem oriented. Rather than focusing on individual specific treatment methods we will be looking at a variety of real patients with venous pathology; proceeding through the steps of appropriate evaluation, then presenting different possible treatment strategies which will include compression, sclerotherapy, laser, ambulatory phlebectomy, and endovenous ablation. Participants will come away with a foundation of knowledge upon which they can build through the ensuing days of the Congress. (Patients will be used as diagnostic models, however no treatment per se will be demonstrated.)

Continuing Medical Education (CME) CreditAmerican Academy of DermatologyThe American College of Phlebology’s 19th Annual Congress is recognized by the American Academy of Dermatology fortwenty-nine (29) hours of AAD Category I CME credit and may be used toward the American Academy of Dermatology’s Continuing Medical Education Award.

The Pre-Congress (Thursday, November 10) qualifies for eight (8) hours and the ACP Congress (November 11-13) qualifies for twenty-one (21) hours.

American Academy of Family PhysiciansThis activity has been reviewed and is acceptable for up to twenty seven and a half (27.50) Prescribed Credits by the American Academy of Family Physicians.

The Pre-Congress (Thursday, November 10) qualifies for seven and a half (7.5) prescribed credits and the ACP Congress (November 11-13) qualifies for twenty (20) prescribed credits.

General InformationSociety for Vascular UltrasoundThis program meets the criteria for twenty-four and a quarter (24.25) SVU-CME credits which are accepted by the American Registry of Diagnostic Medical Sonographers (ARDMS) towards recertification, the American Registry of Radiologic Technologists (ARRT) for Category A credit, and the Inter-societal Commission for the Accreditation of Vascular Laboratories (ICAVL) for accredi-tation.

The Pre-Congress (Thursday, November 10) qualifies for five and a quarter (5.25) credits and the ACP Congress (November 11-13) qualifies for nineteen (19) credits.

Wild Iris Medical EducationThis Educational Program is sponsored by Wild Iris Medical Education which is approved as a Provider by the Washington State Nurses Association. The Washington State Nurse Association is accredited as an approver of Continuing Education in Nursing by the American Nurses Credentialing Center Commission on Accreditation, #PA-5/Feb/08, California Board of Registered Nursing (BRN) Provider #12300, Florida Board of Registered Nurses #NCE3403. This program qualifies for twenty-eight (28) Continuing Education Units.

The Pre-Congress (Thursday, November 10) qualifies for eight (8) Continuing Education Units and the ACP Congress (November 11-13) qualifies for twenty (20) Continuing Education Units.

Additional organizations may recognize these programs. Appropriate credit for attendance should be ascertained and reported by the individual to the particular state or medical society to which he or she belongs. A certificate of attendance will beprovided to all registrants on-site upon completion of the program and return of their evaluations.

Syllabus MaterialsNew This Year: In order to reduce the amount of materials each attendee carries during the Congress, some syllabus materials will be provided to you at the Congress via CD-ROM only. This format applies only to materials presented in the early morning interactive sessions, the afternoon instructional courses, and all symposia. However, if you also want printed copies of thesematerials during the Congress, they may be ordered in advance (details below).

Note that some printed materials will be provided to all registrants, including: the daily program schedule, material from thekeynote presenter, text from all abstracts to be presented, as well as exhibitor and exhibit hall information.

You may order printed copies of the “CD-ROM only” syllabus materials if you desire. The order must be placed in advance of the Congress (on or before October 28, 2005). There is a nominal charge of $35 to cover the printing and preparation costs of the hardcopy materials. Please be sure to check the appropriate box on your registration form if you would like to receive a printed copy of the syllabus materials upon your check-in at the Onsite Congress Registration Desk.

Evaluation FormsEvaluation forms are provided in the meeting packets. Please fill out the forms completely and deposit them at the ACP registration desk in order to receive your CME certificate.

Meeting PacketsMeeting packets will be available at the ACP Registration Desk, which will be located in the Grand Ballroom Foyer. Your packet will contain your badge, printed abstract materials, CD-ROM of additional material (see above), interactive session ticket(s), instructional course ticket(s), and evaluation forms. If you pre-ordered printed syllabus material, they will be provided as well.

Located at the cornerstone of the Embarcadero Waterfront, the Hyatt Regency is one of the most impressive landmarks found in the city skyline. Its architecture features a seven-sided design with a tiered side soaring 17 stories, making its Atrium Lobby not only one of the most unique and photographed, but also the world’s largest. Overlooking the San Francisco Bay and the City’s dynamic waterfront neighborhood, the Hyatt is adjacent to the Financial District, and nearby Chinatown, Fisherman’s Wharf and Union Square. Each guest room features wireless high speed internet access (wi-fi)* at approximately $10 per day, 2-line phones with local and long distance service, data ports for dial up access, voicemail, speakerphone and conferencing capability; work stations with enhanced lighting, hairdryer, iron/ironing board, and views of either City or the Bay.

Room reservations must be made directly with the hotel either by phone or via the Internet. Group rates for ACP are $179single / double (plus 14% state and local taxes). Identify yourself with the American College of Phlebology when makingtelephone reservations.

Online registrations can be made via the ACP website, www.phlebology.org, using the “Hotel Registration Link” on the Meetings page. You cannot make discounted reservations via the general www.hyatt.com website.

All room reservations must be made no later than October 10, 2005. However, the group room block is limited in size and will be filled on a first-come first-served basis. Reservations received after the room block is filled or after October 10, 2005 will be accepted on a space-available basis and prevailing hotel rates. Check in time is 3:00 PM and check out is 12:00 noon.

Note: All ACP group reservations will be crosschecked against the Congress registrations. Those found to be not registered for the Congress will be notified to do so immediately. Non-registrants will be removed from the group room block shortly after notification, so please register for the Congress as soon as registration opens to avoid forfeiting your group room rate.

Transportation InformationThe Hyatt Regency is 14 miles from San Francisco International Airport (SFO). Cabs are approximately $35 each way; van shuttle is approximately $10 each way. General InformationRoommate LocatorACP offers the Roommate Locator service as a way to reduce your hotel expense. Just check the appropriate box on the registration form. Your name, telephone number, address, arrival and departure dates, gender preferences, and whether or not you smoke will be provided to others who have requested this service. You will receive a list of potential roommates in advance of the meeting. Use this list of potential roommates to locate a roommate with similar preferences and arrival and departure dates. Once you find a roommate, please contact the ACP Headquarters Office to have your information removed from the list. We strongly recommend that you make a hotel reservation as soon as possible. If a roommate is located and both individuals have reserved a room, one reservation can be cancelled. (Users of this service accept full responsibility of any consequences arising there from.)

Program, Errata and AddendaThe ACP reserves the right to modify the agenda and speakers listed in the Preliminary or Final Program as circumstances dictate. The Final Program will only be available online in pdf format. No Final Programs will be mailed. If necessary, an Errata and Addenda Sheet will be included with your meeting packet. The final program schedule and room assignments will be included in your printed syllabus materials.
