The Active Lifestyles Programme aims to provide the whole community with the opportunity to live a more active and healthy lifestyle.
Learning more about physical activity and healthy eating has never been easier.
Do you want a stepping-stone to get active and learn new lifestyle skills?
We have a number of options depending on your health, level of fitness and personal goals. For referral onto any of these options, please refer to the back page of this leaflet (how do I access) or contact the Active Lifestyles team on 0151 934 2352 for further details.
There are a number of programmes available to help improve your health & wellbeing including something available for the whole family. Following referral the Active Lifestyles Team will choose the most appropriate level for you.

Supported Programme14-week supported physical
activity programme, under the guidance of a GP referral qualified
Fitness Instructor at your most convenient Active Sefton
leisure facility.
Discounted UseHeavily discounted use of the
facilities during the programme with reduced priced memberships
after completion.
Ongoing SupportOngoing motivational support, continued for 12 months on completion.
LeveL 1EARly IntERvEntIOn PROgRAmmE (AgE 18+)
Aimed at people with low level health problems, which could be improved by leading a healthy lifestyle.
Health ScreeningHealth screening before, during
and after the programme, including blood pressure, weight and waist circumference checks.
Individualised ProgrammePersonalised programme with
access to a full range of activities, including walking and cycling,
exercise classes, gym based sessions, swimming and 6-week
weight managementprogrammes, tailored to meet
your desired goals.
LeveL 2DIEtS DOn’t WORK PROgRAmmE (AgE 16+)
Level 2 is available for those needing weight management support and for those suffering with risk factors for heart disease such as high blood pressure, angina or diabetes.
This level has two options:-Part (a) 12-week Diets Don’t Work Programme (age 16+)(Self referral is also available for this part of the programme)
Group SessionsFree 12-week group
weight management programme.
KnowledgeIncrease your knowledge
and skills necessary to achieve and maintain a
healthy weight.
Behaviour ChangeInvolves not only
nutritional advice but also covers habits, coping
mechanisms, relapses, behaviour change and
weight management tips.Goal SettingIncludes dietary analysis
and goal setting.
Availability Available throughout the
borough of Sefton.
group & One to One Support
Both group and one to one advice available, with motivational support and 3-month follow ups for 12 months after completion.

Support Programme14-week supported programme of
physical activity under the specialist guidance of an Active Lifestyles
Development Offi cer.
Health ScreeningIncludes full health screening and
physiological assessment before, during and after the programme.
Specialised ProgrammePhysical activity programmes are
designed to improve health conditions and are aimed at meeting your
desired goals.
Discounted RateAllows access to a number of physical
activity programmes at a heavily discounted rate including gym, swim, exercise classes, walking and cycling.
Extra HelpGives access to specifi c Active Lifestyles
sessions to encourage group support and help you to meet new friends.
Ongoing Support12-month motivational support and advice given, with access to reduced price memberships on completion of
the programme.
LeveL 3DIEtEtIc SuPPORt (AgE 18+)
ReLAX & RevIve mEntAl WEllBEIng PROgRAmmE (AgE 18+)
Relax & Revive is a programme designed for individuals and groups to help them achieve a positive mental wellbeing through an increase in physical activity. Increased physical activity helps to alleviate feelings of anxiety, stress and depression, improves feelings of confidence and motivation as well as providing opportunities for social interaction and improved social skills.
Relax & Revive offers: -
Drop in SessionsDrop in Sessions that include a variety of Sports Hall activities & relaxation sessions
(tai chi etc) across the borough.
Cost The cost of the drop in
is FRee
Group SessionsWe can organise 12 week block programmes (any sport or activity) for any group that
would like to increase their physical and mental wellbeing.
Specialised SupportOne to one dietetic support through an advanced dietician, for patients needing
specialised dietary advice.
AdviceFree advice and follow up sessions.
Additional GuidanceAvailable to those who have accessed
level 2 and need further specialised support or who have a high level of
obesity.
Group SessionsReferral onto group sessions available.
Access to Physical ActivityStill gives you access to exercise referral to ensure physical activity is built into
lifestyle changes.

MOve ITcHIlD HEAltHy lIFEStylE PROgRAmmE (AgE 7-16)
Do you have children aged 7 to 16 years old? Are you worried that they might be unhealthy or above their ideal weight? Why not register onto the mOvE It programme…
• mOvE It Encourages children and their
families to adopt a healthy lifestyle by using vital elements such as family involvement, healthy eating advice, behaviour change as well as fun ways to increase physical activity.
• Who can access it children aged 7 – 16 that are above
their ideal weight and their families.
• Available venues Available at leisure centres & schools
across the borough.
• Programme consists of FREE 6-week course comprising
of one 90 minute session per week. This includes one hour of fun activities and 30 minutes of nutritional discussion.
• Active parents Parents are also encouraged to get
active in the fitness suites or through taking part in walking and cycling programmes.

How do I access the Active Lifestyles Programme?
Option 1Visit your GP / Health Professional for a lifestyle discussion and ask to be referred onto the Active Lifestyles Programme. (Includes all parts of the programme).
Option 2Self refer onto the Diets Don’t Work Programme (LEVEL 2 part a – Nutrition advice only), simply call 0151 934 2352.
Option 3Self refer onto the Relax & Revive Programme(individually) or to set up a 12 week block programme(any sport or activity), simply call 0151 934 2352.
Option 4To register for the MOVE IT Programme, simply call 0151 934 2352.
