sollte ein TDM für die wichtigsten
b-Lactame etabliert werden
o Aufgrund PK/PD-Überlegungen ist eine prolongierte oder kontinuierliche Infusion von b-Lactam-Antibiotika der intermittierenden Gabe hinsichtlich des Therapieziels, möglichst dauerhaft die MHK der Erreger zu überschreiten, überlegen.

7. Risks of antibiotics in the ICU: toxicities and collateral damage 7.5. In practice
The use of antibiotics, in “at risk” patients (renal dysfunction), should consider the benefice-risk ratio
for efficacy/toxicity, especially when no alternative exists. Where possible, TDM may help minimising
toxicity but exposure thresholds should be clearly identified.
7.6.What is next
In the near future, antimicrobial optimal dosing regimens should not only be defined based on PK/PD
targets associated with clinical and microbiological efficacy, but also on PK/toxicodynamics targets.
The use of dosing softwares may help achieving these goals.

Applikation ist sinnvoll!!
• Standarddosierungen von antiinfektiven Arzneistoffen bei Intensivpatienten sind potentiell problematisch!
o Insbesondere bei b-Laktam-Antibiotika o Veränderte pharmakokinetische Rahmenbedingungen (krankheitsbedingt)
- Arzneistoffelimination, Verteilungsvolumen etc.
o Große Gefahr der Unterdosierung oder Überdosierung - Eingeschränkte Wirksamkeit, Toxizität, UAW
• Therapeutisches Drug-Monitoring ist vor allem auch bei b-Laktam-Antibiotika o Möglich, inzwischen käufliche Kits für die HPLC am Markt o Praktikabel, in jedem Labor mit einer HPLC prinzipiel durchführbar o Sinnvoll, Optimierung der Wirksamkeit, Reduktion von Toxizität/UAW
• Unser Konzept: CADDy + TDM -> Individuelle Dosierung, Kontinuierliche Applikation (b-Laktam-AB, Vancomycin, Linezolid)
o Sicheres Erreichen PK/PD Zielwerten (> 60% der Zeit ≥ 4 x MHK/ECOFF)
o Bessere Gewebepenetration o Verringerung der Gefahr von Toxizität u./o. Resistenzbildung
