First time users accessing the aged care services will need to register
with
MY AGED CARE 1800 200 422
This will involve an over the phone assessment which will determine
eligibility and access needs.
If anyone is in financial difficulty please ask about financial
assistance at the office. All requests for financial assistance will be
held in the strictest confidence.
The aim of this service is to assist frail aged people remain in their
own homes as long as possible by helping to make their homes
safer and more accessible. The area covered by our service is the
Local Government area of Coolamon.

MEALS ON WHEELS
Allawah Community Care provides a Meals on Wheels frozen meal
delivery service.
Some people prefer to pick their meals up from the Allawah
Community Centre however, weekly deliveries can be made
within Coolamon and to surrounding towns within the Shire.
Volunteers deliver meals to those who are unable to come to
Allawah Community Care Office and collect meals for themselves.
A list of meal choices and heating instructions is available from the
Allawah Community Care office.
Heating instructions are clear and if those using the meal service
have problems with their vision and find it hard to use the
microwave or oven, volunteers may be able to provide assistance
with heating.
Invoices are sent out monthly and Volunteers will ask you to sign
a delivery sheet when they bring meals to your home.
The frozen meal service is meant to compliment a nutritious diet
and all those who use the frozen meals service are encouraged to
eat a healthy breakfast and a good balance of fruit, vegetables and
salad with the meals they prepare for themselves.
If you find there are too many meals in your freezer please ring
and cancel your meals for a few days. Remember to do the same
if you are going away or having family members come to visit.
Frozen meals can only be compared to frozen meals. We have
selected a local company who are committed to providing a
quality frozen product, using fresh produce and seasonal
vegetables.
If you find the vegetables are not cooked enough for your taste,
feel free to cook them a little more so that they are to your liking.
If you have any comments or complaints about the meals provided
we will periodically ask your feedback via service surveys. Please
do not hesitate to note on the survey or alternatively contact or
call at the office.
COMMUNITY TRANSPORT
Allawah Community Care Community Transport provides a service for
Residents of the Coolamon Shire and regularly travel between Wagga
Wagga, Junee, Narrandera & Temora as well as locally.
Services are provided by volunteer drivers who will pick you up at
home and take you to your destination, then return you home as
required.
The Service
Enables eligible people to attend:-
Health/Welfare appointments
Shopping
Social Activities
Appointments etc
Bookings
Bookings may be made during business hours between 8.30am
and 3pm by phoning the Allawah Community Care office on
6927 3521. A message may be left on the answering machine at
any time and we will return your call.
You will need to give at least 48 hours notice for local trips and it
is greatly appreciated if you could give at least four days notice
prior to out of town appointments.
All Bookings are to be made through the Allawah Community Care
Community Transport Office, not with volunteer drivers.
Cost
Service Users accessing Community Transport will be asked to
make a contribution and this will vary depending on the
destination.
SOCIAL SUPPORT
The purpose of the Social Support Service is to provide a range of
basic support services to the elderly to enable them to remain
independently in their own homes.
Allawah Community Care Social Support services are provided by
a companion (paid worker or volunteer), either within the home
or while a person is accessing services or facilities within the
community.
The Social Support Service is primarily directed towards meeting
the person’s need for social contact and/or a companion in order
to participate in community life.
Social Support enables people who are frail & elderly to enjoy life
more fully and develop friendships with people in the wider
community through social interaction and community based
experiences
The Service
Enables eligible people to attend:-
a social event
Shopping (groceries, new clothes, hairdresser appointments, post office etc)
Sharing an interest with a small group of people, supported by volunteers or paid staff.
Visiting friends or having someone visit you in your home
Letter writing / phoning friends or relatives for the vision impaired
Bill paying and banking where the client is accompanied
Accompanying people to medical appointments
Other assistance as requested
CARERS RESPITE
From time to time your care provider may need to attend an
appointment or would appreciate some assistance in the home to
assist with daily tasks, or would just like to go and visit a friend but
does not wish to leave you alone.
The Service
Allawah Community Care can provide a suitably qualified care
worker to provide care in your home between the hours of 9am
and 4pm. Whether it be 1 hour or 5 hours this can be determined.
The care worker can:-
Assist with preparing the meal
Play board games / cards, be interactive with client
Provide some domestic services whilst there
HOME MODIFICATION SERVICE
Examples of Home Modification work that the service can provide
include:
Installation of handrails, handheld showers and tap turners.
Installation of ramps
Rebuilding of steps to accommodate walking frames etc.
(Ramps and remodeled steps are provided at one point of access
per home, usually either the back or front door)
Applications for more extensive modifications for eligible residents.
How to Access the Home Modification Services
Contact My Aged Care for an initial assessment. They will then
forward the referral onto Allawah Community Care at your
request.
The Process
Once a request for Home Modifications is received, a home
assessment is arranged with you and an Occupational Therapist at
a mutually convenient time.
Occupational Therapists are highly trained health professionals.
You can be confident of their ability to make recommendations
specific to your requirements.
Cost
Limited funding allows only the supply of functional items deemed
necessary by the Occupational Therapist. You will have to pay full
cost for any additional items supplied at your request.
The Occupational Therapist will provide drawings of the necessary
work. At this stage you will have three choices:-
1. Take the drawings and request the help of friends and family to get the work done, according to the plans. If you choose this option the Home Modification Service will not contribute to your costs.
2. Contact your preferred builder and manage the project yourself. If this is your choice the Home Modification Service will not contribute to your costs.
3. Allow Allawah Community Care to get quotes from licensed contractors and come back to you a quote for works.
If Allawah Community Care arranges to have the work carried out
on your behalf, you will be required to sign a document indicating
your willingness to pay for the contribution cost.
Funds from the Home Modifications Service will cover the
remaining costs. Terms can be arranged in cases of extreme
financial hardship. As is the case with all client information, this
will be treated confidentially.
When the work is completed Allawah Community Care will contact
you to discuss the completed work and you will be asked to
complete a service survey.
HOME MAINTENANCE
About the Service
Allawah Community Care provides a suitably qualified person by
Coolamon Shire Council approved contract.
They will provide basic yard maintenance:
Lawn mowing
Whipper snipping
Gutter cleaning
Weeding
They may also
Change tap washers
Adjust water systems
Bookings
Once Allawah Community Care receive your referral from My Aged
Care we will contact you to arrange service details.
Note:
Funding for this service is limited. You may be placed on a wait list.
If a funded service is at capacity you may be placed on a wait list
or where the wait list are deemed long we may refer you to
another provider.
